dc.description.abstract | Pēdējās desmitgades laikā intrakraniālo aneirismu ārstēšana ir kļuvusi vairāk invazīvi-radioloģiska nekā neiroķirurģiska. Tomēr joprojām nav vienota viedokļa, kura no šīm metodēm ir ilgtermiņā labvēlīgāka pacientam. Mērķis - noskaidrot, vai vēlīnais iznākums pacientiem pēc aneirismas ruptūras atšķiras atkarībā no pielietotās ārstēšanas metodes – endovaskulārās koilēšanas un tiešās klipēšanas.
Retrospektīvi tika analizēti pacienti (n=175), kas 2010./11. gadā ārstējušies Paula Stradiņa Klīniskās universitātes slimnīcas Neiroķirurģijas klīnikas 22./30. nodaļā. Pacienti ar aneirismas ruptūru (n=109) tika aptaujāti, izsūtot pa pastu anketas. Rezultāti: „koilēšanas” grupa: 0-1 ballei pēc modificētās Rankina skalas atbilst 63%(24/39) pacientu, 2-3 ballēm - 29%(11/39), 4-6 ballēm - 13%(4/39). „Klipēšanas” grupa: 0-1 balle - 52%(12/23), 2-3 balles - 35%(8/23), 4-6 balles - 8%(3/23). Secinājumi: statistiski nav nozīmīgas atšķirības vēlīnajā iznākumā, salīdzinot koilēšanu ar klipēšanu. | en_US |
dc.description.abstract | For the last decade the treatment of intracranial aneurysms has become as much radiological, as much neurosurgical problem. However there is still no agreement which of these approaches is more beneficial for patients.
Purpose: To assess and compare the late outcome of endovascular and neurosurgical treatment of ruptured intracranial aneurysms.
A total of 175 patients were retrospectively analysed, who were hospitalized in the Pauls Stradins Clinical University Hospital, the Neurosurgery Clinic (department no. 22./30.) in year 2010./11. The patients who were diagnosed with ruptured aneurysms (n=109) were selected for the survey and they received a questionnaire via postal mail.
Results: the „coiling” group: grade 0-1 according to the modified Rankin scale was 63%(24/39) of the patients, mRS 2-3 - 29%(11/39), mRS 4-6 - 13%(4/39). The „clipping” group: mRS 0-1 - 52%(12/23), mRS 2-3 - 35%(8/23), mRS 4-6 - 8%(3/23).
Conclusions: there was no significant difference in the late outcome. | en_US |
