Refraktīvas lēcas implantācija pēc dabiskās lēcas ekstrakcijas

Tēmas aktualitāte: Pēc Pasaules Veselības Organizācijas datiem pasaulē ir ļoti daudz cilvēku ar nekoriģētu refrakcijas kļūdu un kataraktu. Viena no šo pacientu ārstēšanas iespējām ir refraktīvas lēcas implantācija pēc dabiskās lēcas ekstrakcijas.
Mērķis: Noskaidrot refraktīvas lēcas implantācijas pēc dabiskās lēcas ekstrakcijas efektivitāti pacientiem ar lēcas apduļķošanos (katarakta) vai bez lēcas apduļķošanās (presbiopija, miopija, hipermetropija, astigmātisms).
Materiāli un metodes: Pētījuma veikšanai tika izstrādāts protokols, lai varētu izanalizēt refraktīvas lēcas implantācijas pēc dabiskās lēcas ekstrakcijas rezultātus pētījumā iekļautajiem multifokālas un monofokālas gupas pacientiem. Pētījums tika veikts SIA “Dr. Solomatina acu rehabilitācijas un redzes korekcijas centrs”. Kopumā pētījumā iekļauti 60 pacienti jeb 86 acis, no kuriem 11 pacienti jeb 21 acs ietilpst multifokālu pacientu grupā, 49 pacienti jeb 65 acis monofokālu pacientu grupā. Multifokālu pacientu grupā ietilpst difraktīvu (5 pacienti jeb 9 acis), difraktīvu – refraktīvu (3 pacienti jeb 6 acis) un torisku (3 pacienti jeb 6 acis) pacientu grupas. Iegūtos datus apkopoja un analizēja izmantojot “IBM SPSS Statistics” 20 programmu un MS EXCEL datorprogrammu.
Rezultāti: Multifokālu pacientu grupa – vidēji 48 gadi, bez rezultātu ietekmējošām acs blakusslimībām. Pēcoperācijas periodā nenovēro stabilas komplikācijas un netiek veiktas papildus operatīvas manipulācijas, taču ir subjektīvu sūdzību.
V palielinās, taču būtiskas atšķirības starp multifokālu (difraktīvu, difraktīvu – refraktīvu) un monofokālu pacientu grupām nenovēro. Sfēras amplitūda samazinās. Daļai acu tiek lietotat V korekcija. Visi iegūtie dati nav statistiski ticami, jo ir nepietiekošs acu skaits pētāmajā grupā noteiktā laika periodā.
Secinājumi: Darbā izvirzītā hipotēze neapstiprinās un lai iegūtu ticamus rezultātus nepieciešams veikt klīniski randomizētu pētījumu lielākā laika periodā. Importance of the subject: According to data of World Health Organisation there is a vast number of people worldwide with non-adjusted refraction error and cataract. One of the forms of treatment for such patients is refractive lens exchange.
Objective: To identify the efficiency of refractive lens exchange for patients with cataract or without cataract (presbyopia, myopia, hypermetropia, astigmatism).
Materials and methods: For the purposes of the study a protocol was drawn up in order to analyse the results of refractive lens exchange for monofocal and multifocal patients included in the object of the research. The study was conducted at the SIA Dr. Solomatina acu rehabilitācijas un redzes korekcijas centrs. The study objected included 60 patients or 86 eyes, 11 of which or 21 eye belonged to the group of multifocal patients, but 49 patients or 65 eyes belonged to the group of monofocal patients. The group of multifocal patients included diffractive patients (5 patients or 9 eyes), diffractive – refractive (3 patients or 6 eyes) and torical patients (3 patients or 6 eyes). The obtained data were summarised an analysed by using IBM SPSS Statistics 20 and MS EXCEL computer software.
Results: Group of multifocal patients – on average 48 years old, without additional eye sickness affecting the outcome. No stable complications were observed during post-surgery period and no additional surgical manipulations were performed, however, subjective complaints were voiced.
The sharpness of the sight increases, nevertheless significant differences between multifocal (diffractive, diffractive-refractive) and confocal groups of patients were not observed. The range of field decreases. A sharpness of the sight was performed for a part of the eyes. All obtained data are not statistically reliable due to the fact that there were an insufficient number of eyes within the studies group over the set period of time.
Conclusions: The hypothesis set in the course of the work did not confirm and in order to obtain reliable data it is required to conduct a clinically random research over a longer period of time.
