Vienlaicīga abpusēja gūžas locītavas endoprotezēšana

Abstract

Starp daudzām dažādām konstrukcijām, kuras šobrīd izmanto traumatoloģijā, rekonstruktīvajā ķirurģijā un ortopēdijā, endoprotēzēšanai ir īpaša vieta. Galvenais iemesls lielo locītavu endoprotezēšānas straujai attīstībai ir tas, ka tā ir joprojām vienīgā metode, ar kuru var atgriezt normālā dzīvē simtiem tūkstošu pacientus ar traumu sekām, infekciju sarežģījumiem, aseptiskām nekrozēm, reimatoīdo artrītu, iedzimtām lielo locītavu deformācijām. Mūsdienās pilnīga gūžas locītavas endoprotezēšana ir īpaši bieža operācija rekonstruktīvā ķirurģijā. Pasaulē katru gadu ap 800 000 pacientiem veic šādas operācijas. Pēc ārzemju autoru aprēķiniem, šobrīd vairāk ne kā 2,5 milj. cilvēku dzīvo pilnvērtīgi ar gūžas kaula endoprotēzi. Materiāli prospektīvam pētījumam „Vienlaicīga abpusēja gūžas locītavas endoprotezēšana” tika iegūti Vācijā, traumatoloģijas un ortopēdijas slimnīcā „Sana-Klinik ZollernalbGmbH” Robert-Koch-Straße 26, 72461 Albstadt. Pētījumā tika apkopoti dati par 150 pacientiem no „Sana-Klinik-Zollernalb” slimnīcas. Apskatītas 75 pacientu slimību vēstures ar pacientu datiem, kuriem veikta Bilaterāla gūžas locītavas endoprotezēšana, un 75 pacientu slimību vēstures ar Unilaterālo endoprotezēšanas pieeju. Slimnieki sadalīti 2 grupās.

Pētījuma darba mērķis ir izvērtēt bilaterālas endoprotezēšanas pieeju, salīdzinot to ar unilaterālo pieeju, kā arī veikt pacientu ārstēšanas datu apkopojumu, bilaterālas un unilaterālas endoprotezēšanas pielietošanas gadījumos. Noslēgumā veikt iegūto rezultātu analīzi.

Pēc pētījuma rezultātiem tiek secināts, ka bilaterālas endoprotēzēšas rezultāti un komplikācijas būtiski neatšķiras no unilaterālas endoprotezēšanas, praktiski pēc visiem parametriem.

SUMMARY Among the many different structures, which are currently used in traumatology, reconstructive surgery and orthopedics, arthroplasty has a particular position. The main reason of rapid development of big joint replacement is the fact, that it is still the only method by which it can return to normal life hundreds of thousands of patients with the effects of trauma, infectious complications, aseptic necrosis, rheumatoid arthritis, congenital deformations of big joints. Full hip arthroplasty is particularly frequent surgery reconstructive surgery today. There are 800 000 approximately for this type of surgery every year all over the world. According to calculations of foreign authors, there are more than 2.5 million people live with hip endoprosthesis. Materials of research "Simultaneous bilateral total hip arthroplasty" were collected in Germany, Traumatology and Orthopedics Hospital ‘’Sana-Klinik ZollernalbGmbH "Robert-Koch-Straße 26, 72461 Albstadt. The study included 150 patients from the "Sana-Klinik-Zollernalb’’ hospital. Study focused on 75 patients' medical history of bilateral replacement, and 75 patients with histories of unilateral replacement approach. Patients are divided into 2 main categories accordingly. The main target of research work is evaluation of the bilateral arthroplasty approach comparing to the unilateral approach, perform patient treatment data collection, bilateral and unilateral replacement practices, to analyze results.

According to results of research work is concluded that the bilateral arthroplasty results and complications do not differ significantly from unilateral replacement in almost all parameters.
