Meticilīnrezistentais Staphylococcus aureus (MRSA) izraisa gan sadzīvē iegūtas, gan ar ārstniecības iestādi asociētas (HI-MRSA) slimības. Lai atšķirtu epidemioloģiski nesaistītus MRSA uzliesmojumus un jaunus klonus, izmanto antibiogrammu kombinācijā ar molekulāro tipēšanu.
Darba mērķis bija novērtēt MRSA molekulāro epidemioloģiju Paula Stradiņa Klīniskajā universitātes slimnīcā (PSKUS) pēdējos trīs gados un identificēt iespējamos jaunos klonus. Ir veikta MRSA izolātu fenotipiska novērtēšana un 27 MRSA izolātu genotipiska analīze, izmantojot tipēšanu pēc spa gēna.
Noskaidrots, ka PSKUS pēdējos trīs gados ir sastopami četri radniecīgi spa tipi, kas pieder iepriekš identificētam HI-MRSA celmam SCCmecIII-ST368. Trīs no tiem līdz 2011. gadam nebija identificēti. Viens no spa tipiem agrāk nav identificēts. Sastādīts hipotētisks jauno spa tipu evolūcijas ceļš.
PSKUS ir vērojama pakāpeniska MRSA multirezistento celmu nomaiņa uz mazāk rezistentiem MRSA celmiem, kas saskan ar kopējām tendencēm Eiropā. Methicillin-resistant Staphylococcus aureus (MRSA) can cause community-acquired and hospital-acquired (HA-MRSA) infections. An antibiogram in combination with molecular typing is used in order to distinguish epidemiological unrelated outbreaks and new clones of MRSA.
The aim of this work was to estimate molecular epidemiology of MRSA over the last three years in Pauls Stradins Clinical University Hospital (PSCUH) and to identify potential new clones. Phenotypic assessment of MRSA isolates and genotypic analysis of 27 MRSA isolates using molecular typing by spa gene were carried out.
It was determined four related spa types which belong to the previously identified HA-MRSA strain SCCmecIII-ST368 were observed over the last 3 years in PSCUH. Three of them have not been identified till 2011. One spa type have not been identified before. A hypothetical evolutionary path of new spa types was created.
A gradual transition from multiresistant strains of MRSA to less resistant strains of MRSA is observed in PSKUS which is in line with common tendencies in Europe.
