Diplomdarba tēma ir „Nozokomiālo stafilokoku rezistence pēc endoprotezēšanas operācijām Traumatoloģijas un ortopēdijas slimnīcā”.
Darba mērķis ir izpētīt endoprotezēšanas locītavu mikrobo iekaisumu etioloģiskos aģentus, kā arī noteikt to jutību uz antimikrobajiem līdzekļiem.
Darbā izmantotās pētījuma metodes ir mikrobu izolācija, to identifikācija, izmantojot tīrkultūras izdalīšanas metodi un bakterioskopiju, bioķīmisko īpašību noteikšana ar dažāda veida testiem – katalāzes, lecitināzes, koagulāzes, β-laktamāzes un Staph, kā arī tiek noteikta jutība pret antimikrobajiem līdzekļiem, izmantojot disku difūzijas testu (Bauer-Kirby metode).
Diplomdarbā analizēti 2009.gada laikā veikto endoprotezēšanas operāciju bakterioloģijas žurnāli, kuros tika reģistrēti mikrobi, kas tika izdalīti no sastrutojošām brūcēm pēc endoprotezēšanas operācijām, kā arī tika noteikta mikrobu jutība uz antimikrobajiem līdzekļiem, izmantojot standartmetodes saskaņā ar metodiskajiem norādījumiem „Bioloģiskā materiāla mikrobioloģiskās izmeklēšanas metodes”. Kopā tika izdalīti 99 mikrobi, no kuriem 76 (76,8%) sastādīja Gr+ kultūras, bet 23 (23,2%) – Gr- kultūras. No Gr+ kultūrām 70 bija stafilokoku grupas mikrobi un 31 no tiem bija S. aureus, bet 39 bija koagulāzes negatīvie stafilokoki (KoNS). Veicot pētījumu, tika konstatēts, ka no visiem izdalītajiem S. aureus mikrobiem, tikai 2 bija cefoksitīna (meticilīna) rezistenti, kas liecina, ka pārsvarā visas komplikācijas pēc endoprotezēšanas operācijām neizraisīja slimnīcas infekcijas. Zīmīgs arī tas atzinums, ka otrs biežāk sastopamais mikrobs ir S. epidermidis, ar kuru, tā rezistences dēļ pret pieejamiem antimikrobajiem līdzekļiem, ir ļoti grūti cīnīties. Pavisam tika izdalīti 17 S. epidermidis mikrobi, 9 no tiem bija rezistenti pret cefoksitīnu (meticilīnu). The subject of the current research paper is „Nosocomial staphylococcal resistance to the replacement operations in the Hospital of Traumatology and Orthopedics”.
The aim of the research is to explore the replacement of joint inflammation etiological microbial agents, and to determine their susceptibility to antimicrobials.
Following methods were used to implement research: isolation of microbs, their identification, using a pure culture of evolution and bacterioscopy, different types of biochemical tests - catalase, lecitinase, coagulase, β-lactamase and Staph, and is determined susceptibility to antimicrobial agents using disc diffusion test (Kirby-Bauer method).
In the diploma paper obtained analysis of bacteriological journals, where are registered all the isolated microbes from wounds after replacement surgery operations in the year of 2009, and was determined antimicrobial susceptibility to antimicrobial agents, using standard methods in accordance with the guidelines to "A biological material for microbiological methods of investigation". Together were distributed 99 microbes, of which 76 (76.8%) accounted for Gr + culture, while 23 (23.2%) - Gr-culture. Of Gr + 70 cultures were staphylococcus microbe groups and 31 of them were S. aureus, 39 were coagulase-negative staphylococci (CoNS). A survey showed that of all excreted S. aureus microbes, only 2 were cephoxitin (methicillin) resistant, indicating that most of all complications following endoprosthetic operations did not result in hospital infections. Important to mention the second most common microbe S. epidermidis, which due to its resistance to available antimicrobials is very difficult to fight. Together were distributed 17 S. epidermidis, 9 of them were resistant to cephoxitin (methicillin).
