Pepsinogēna un gastrīna-17 nozīme autoimūna gastrīta diagnostikā

Ievads: AG ir pārāk reti diagnosticēts stāvoklis ar augstu KV attīstības risku. Ilgstošas asimptomātiskas gaitas dēļ to ir grūti diagnosticēt, tāpēc nepieciešamas jaunas, efektīvas un mazāk invazīvas diagnostikas metodes.
Mērķis: Darba mērķis ir analizēt seruma biomarķieru (pepsinogēns, G-17) nozīmīgumu AG diagnostikā.
Materiāli un metodes: Pētījuma modelis ir nejaušās izlases princips, iekļaujot pacientus, kuri laika periodā no 2004. - 2014. gadam Gremošanas veikuši augšējā gastrointenstinālā trakta endoskopiju. Lai izmērītu seruma biomarķieru (pepsinogēns un G-17) līmeni, pacientiem tika ņemti asins paraugi. Balstoties uz Sidnejas klasifikāciju, tika ņemtas biopsijas.
Rezultāti: Pētījumā tika iekļauti 806 pacienti, histopatoloģiski 44 (5%) pacientu tika diagnosticēts AG, 36 (4%) AA, 6 (1%) TA, 720 (89%) NA. Lai saistītu AG pacientu PI/II attiecību ar G-17, tika izveidotas viena pētījuma grupa. Vienā grupā novēroja G-17 pieaugumu un PI/II attiecības krišanos. Tātad AG gadījumā novēro tiešu korelāciju starp šiem abiem raksturlielumiem. Tika novērota izteikta negatīva korelācija starp G-17 un P I/II attiecību, kas bija statistiski ticama (rs = -.793, p = .000).
Vidējā PI/II attiecība AG pacientiem (44) ir 5.41 (ST±5.36). Bet AG negatīviem pacientiem vidējā PI/II attiecība ir (761) 8.42 (ST±4.22). Vidējā atšķirība ir 3.01 (ST± 0.82). Ir statistiski ticama PI/II attiecības atšķirība starp AG pozitīviem un negatīviem pacientiem (0.001sig.2-tailed).
Vidējais G-17 līmenis pacientiem ar AG (44) ir 25.96 pmol/L (ST±21.64) un AG negatīviem pacientiem (761) tas ir 8.49 pmol/L (ST± 13.08). Vidējā atšķirība ir 17.46 pmol/L (ST± 3.29). Ir statistiski ticama G-17 līmeņa atšķirība starp AG pozitīviem un negatīviem pacientiem (0.00 sig.2-tailed).
Secinājumi:
Seruma biomarķieri ir nozīmīgi AG diagnostikā atrofisku dziedzeru gadījumā. Ir skaidrs, ka AG gadījumā G-17 pieaug un PI/II attiecība krītas.

Background: AG is an underdiagnosed condition with a high risk for GC. It is difficult to diagnose as it remains asymptomatic for long time and it is important to find new efficient and less invasive methods of diagnosis.
Objectives: The objective of the study is to analyse the relevance of serum biomarkers (pepsinogens, G-17) in diagnosing AG.
Materials and Methods: The study model is a random sampling that includes patients who came for an upper GIT endoscopy between the years 2004-2014. Blood samples were taken from the patients to measure the levels of serum biomarkers (pepsinogens and G-17). Also biopsies were taken based on the Sydney Classification.
Results: The total sample of patient includes 806 patients, based on histopathology we have 44 (5%) patients with AG, 36 (4%) patients with AA, 6 (1%) TA, 720(89%) NA patients.
When correlating the ratio of PI/II with G-17 in autoimmune positive gastritis patients,one major group was formed. In one group G-17 is increasing and subsequently the ratio of PI/II is decreasing. Thus, there is a strong negative correlation between G-17 and PI/II ratio, which was statistically significant, rs = - .793, p = .000.
The average ratio of PI/II in the AG patients (44) is 5.41 (ST±5.36). But in negative AG patients the average ratio of PI/II is (761) 8.42 (ST±4.22). The mean difference is 3.01 (ST± 0.82). There is a statistically significant difference of PI/II ratio between the positive patients with AG and negative patients with AG (0.001sig.2-tailed).
The average level of G-17 in patients with AG (44) is 25.96 pmol/L (ST±21.64) and in negative AG patients (761) is 8.49 pmol/L (ST± 13.08). The mean difference is 17.46 pmol/L (ST± 3.29). There is a statistically significant difference of G-17 between the positive patients with AG and negative patients with AG (0.00 sig.2-tailed).
Conclusions:
The serum biomarkers are relevant in AG diagnosis in the presence of atrophic glands. It is clear that G-17 is increasing and PI/II ratio will be subsequently decreasing in the presence of AG.
