Pasaulē nav vienotas taktikas ceļa locītavas septiska artrīta ārstēšanai. Ir vairāki protokoli, kuros pie septiska artrīta izvēlas punktēt ceļa locītavu, tādējādi nodrošinot drenāžu. Cits protokols paredz artroskopisku skalošanu vairākas reizes, neuzrādot konkrētu skaitu. Ja augstāk minētās darbības kopā ar antibakteriālu terapiju nedod vēlamo efektu, dažkārt tiek veikta vaļēja sinovektomija vai pat locītavas endoprotezēšana.
Traumatoloģijas un ortopēdijas slimnīcā nav vienota protokola ceļa locītavas septiska artrīta ārstēšanai. Parasti cenšas 3 reizes izskalot ceļa locītavu ar troakāriem, izmantojot 6 litrus fizioloģiskā šķidruma. Tad tiek izvērtēti iekaisuma rādītāji dinamikā un lemts par turpmāko ārstēšanas taktiku.
Šī pētījuma mērķis bija noteikt faktorus, kas ietekmē ceļa strutainā artrīta ārstēšanas rezultātu un pārbaudīt, cik labi sāpju NRS, EQ-5D-5L un KOOS skalas var raksturot ārstēšanas rezultātu.
Laika posmā no 2010. līdz 2013. gadam Traumatoloģijas un ortopēdijas slimnīcā ar ceļa locītavas septisku artrītu ārstējušies 52 pacienti, no kuriem intervēti (iekļauti pētījumā) tika 42. Ceļa locītavas strutainā artrīta iemesli: jatrogēni – 26 (13 operācijas un 13 intraartikulārās injekcijas), nezināmi – 8, artropātijas – 5 (t.sk. 3 kristālu artropātijas), traumas – 2, roze – 1. Lielākoties – 38,1% gadījumos – izsaucējs bija Staphylococcus aureus, taču 35,7% gadījumos izsaucējs netika konstatēts.
No visiem pētījumā pārbaudītiem faktoriem, kas varētu ietekmēt ārstēšanas rezultātu, t.i. vecums, ĶMI, sistēmisko risku esamība, laiks no sūdzību parādīšanās līdz operācijai, iekaisuma epizožu skaitu pirms operācijas, laboratoriskas analīzes, osteoartrīta un hondropātijas pakāpe, locītavas skalošanu skaits, vienīgi ĶMI statistiski ticami ietekmēja ceļa strutainā artrīta ārstēšanas rezultātu.
Pacientiem ar pilnīgu izārstēšanos bija labāki iznākuma rādītāji, tomēr EQ-5D-5L skala nav tik laba ceļa strutainā artrīta rezultātu izvērtēšanai, kā sāpju NRS un KOOS skalas. The world has no single procedure for treatment of septic arthritis of a knee. There are several protocols where in case of septic arthritis is chosen to stipple the knee joint, thus ensuring the drainage. Other protocol provides arthroscopic lavage several times without showing a specific number. If the above actions with antimicrobial therapy do not give the desired effect, sometimes open synovectomy or even arthroplasty of a joint is being performed.
Hospital of Traumatology and Orthopaedics has no single protocol for treatment of septic arthritis of a knee joint. Usually it is tried to rinse knee joint three times with tunnelers by applying 6 litres of saline solution. Then the inflammatory indicators are evaluated in dynamics and it is decided on the future treatment strategy.
The aim of this research was to determine factors what influence treatment outcome of suppurative arthritis of a knee joint, and to check how well the pain scales NRS, EQ-5D-5L and KOOS can describe treatment outcomes.
During the period from 2010 to 2013, the Hospital of Traumatology and Orthopaedics has treated 52 patients who suffered from septic arthritis of knee joint, 42 of whom were interviewed (included in the study). Causes of suppurative arthritis of a knee joint: iatrogenic - 26 (13 surgeries, and 13 intra-articular injection), unknown - 8, arthropathies - 5 (including 3 crystal arthropathies), trauma – 2, erysipelas - 1. Mostly – in 38.1% of cases – the cause was Staphylococcus aureus, however in 35.7% of cases the cause was not established.
From all in the research tested factors what could affect treatment outcome, i.e., age, BMI, presence of systemic risks, the time between appearance of a complaint and surgery, number of inflammatory episodes before surgery, laboratory tests, degree of osteoarthritis and chondropathy, number of joint lavages, only BMI had statistically credible significant impact on treatment outcome of suppurative arthritis of a knee joint.
Patients who recuperated completely had better outcome indicators, however EQ-5D-5L scale is not as good for evaluation of suppurative arthritis of a knee joint as pain NRS and KOOS scales.
