Darba mērķis un problēmas būtība
Darba mērķis ir analizēt onihomikožu epidemioloģiju Latvijā 2013./2014.gadā, noskaidrot tās galvenos izraisītājus, klīniskos un sociālos aspektus, kā arī izstrādāt anketu onihomikožu epidemioloģisko rādītāju apkopošanai un rekomendēt to ieviešanu Latvijas dermatoveneralogu praksēs. Zinātniskās publikācijas vēsta par onihomikožu izplatību, diagnostiku un ārstēšanas metodēm pasaules mērogā, tomēr trūkst epidemioloģisko pētījumu Latvijā, kā arī nav izstrādātas vadlīnijas slimības diagnostikai un ārstēšanai.
Materiāli un metodes
2013.gada 27.novembrī Latvijas Dermatovenerologu asociācijas sēdē tika izdalītas un elektroniski izsūtītas 105 anketas, 54 dalībnieki piekrita aizpildīt aptaujas anketas, uz elektroniski izsūtītajām atbildēja 8 speciālisti. Kopā tika analizētas 62 anketas. Anketa sastāv no 10 jautājumiem (skatīt pielikumā). Tika iegūti un analizēti arī dati no Zāļu valsts aģentūras par antimikotisko zāļu patēriņu statistiku 2014. gadā.
Rezultāti
Sakarā ar to, ka trūkst vienotu vadlīniju, Latvijā praktizējošiem dermatologiem nav vienota uzskata onihomikozes diagnosticēšanā un terapijā. Lielākā daļa (69%) aptaujāto ārstu atzīmē dermatofītus kā onihomikozes biežākos izraisītājus, kas atbilst šā brīža situācijai pasaulē un Latvijā. Tikai 13% ārstu atzīmē, ka terapiju pēc onihomikozes diagnozes uzstādīšanas uzsāk 75-100% pacientu. No tā var secināt, ka joprojām pastāv mīts, ka onihomikoze ir slimība, kuru var neārstēt, tādejādi tā turpina izplatīties. Tikai 48% aptaujāto diagnosticēšanai pielieto “zelta standartu” (klīniska izmeklēšana + tiešā mikroskopija + mikoloģiskais uzsējums). Pēc Valsts zāļu aģentūras datiem pārdotākie pretsēnīšu medikamenti ir bezrecepšu topiskie antimikotiskie preparāti.
Secinājumi
Latvijā biežākie onihomikozes izraisītāji ir dermatofīti (T.rubrum un T.interdigitale). Ir nepieciešams veikt plašu onihomikožu epidemioloģisko pētījumu Latvijā, kā arī izstrādāt un ieviest vienotas vadlīnijas "Onihomikožu diagnostika un ārstēšana Latvijā". Pēc onihomikozes diagnosticēšanas, ārstēšanu uzsāk tikai 50% pacientu, jo terapija ir dārga un nav valsts apmaksājamo zāļu sarakstā. Tā kā pārdotākie medikamenti ir topiskie bezrecepšu medikamenti, tad varam secināt, ka liela daļa onihomikozes pacientu neārstējas atbilstoši rekomendējamai terapijai, bet lieto topiskos pretsēnīšu medikamentus ar kuriem nav iespējams izārstēt onihomikozi. Study objective
The aim of the research was to clarify the Latvian experience of the practicing dermatologists in diagnosing and treating onychomycosis in 2013-2014 as well as compare it with the data described in the scientific literature. The scientific publications report on the prevalence, diagnosis and treatment methods of onychomycoses on a global scale, however, there is the lack of data on the situation in Latvia.
Materials and methods
On November 27, 2013, 105 questionnaires were given out to the participants of the meeting of the Latvian Association of Dermatovenerology and sent out electronically. 54 dermatovenerologists agreed to fill them in and 54 questionnaires were returned. Within a month, 8 specialists replied to the questionnaires sent out electronically. In total 62 questionnaires were analyzed. The questionnaire consists of 10 questions (annexed). The data of the State Agency of Medicines on the statistics of the consumption of the antifugal medicines were collected and analyzed for the year 2014.
Results
Due to the lack of uniform guidelines, the practicing dermatologists in Latvia do not have a single view on the diagnosis and therapy of onychomycosis. Most (69%) of the surveyed physicians note dermatophytes as the top pathogenic agents of onychomycosis, which corresponds to the current situation in the world and in Latvia. Only 13% of the physicians note that after a diagnosis of onychomycosis the therapy is initiated by 75-100% of patients. From this, it can be concluded that there is still a myth that onychomycosis is a disease that may not be treated, so it continues to spread. Only 48% of the respondents use the "golden standard" (clinical examination + direct microscopy + mycological plating) for diagnosing the state. According to the data of the State Agency of Medicines, the first place on the most selling antifungal medication list is taken by non-prescription topical antifungal medication.
Conclusions
The top pathogenic agents of onychomycosis in Latvia are dermatophytes (T.rubrum un T.interdigitale). In Latvia, it is necessary to do epidemiological researche and develop strict guidelines “The diagnosis and treatment of onychomycosis in Latvia”. After the diagnosis of onychomycosis, therapy is initiated only by about half of the patients because of the expensive therapy, which is not supported from the government. As the best-selling anti-fungal medication is non-prescription topical medication we conclude that a part of onychomycosis patients are being unseccessfully treated without a dermatologist’s advice.
