Sargmezgla biopsijas izmantošana pacientēm ar agrīnas stadijas invazīvo krūts vēzi

Problēmas aktualitāte. Sargmezglu biopsija ir minimāli invazīva reģionalās kontroles metode, kuras izmantošana var mazināt morbiditāti, saistītu ar citu, līdz 1994.gadam vienīgo, reģionālas kontroles metodi krūts vēža pacientēm - paduses limfmezglu disekciju. Pētījuma mērķis. Izprast korelāciju starp sargmezglu biopsiju un metastāzēm paduses limfmezglos pacientēm ar agrīnas stadijas invazīvu krūts vēzi. Materiāli un metodes. Pētījums ir aprakstošs retrospektīvs. Tika atlasītas un analizētas agrīno stādiju (I, IIA, IIB) krūts vēža pacientšu slimības vēstures Latvijas Onkoloģijas Centra un Patoloģijas Centra arhīvos, kuras laika posmā no 2010.gada janvāra līdz 2012.gada decembrim saņēma ķirurģisku ārstēšanu Latvijas Onkoloģijas Centra krūts ķirurģijas nodaļā, un kurām tika veikta sargmezgla biopsija. Dati tika apkopoti, apstrādāti IBM SPSS programmā, rezultāti salīdzināti ar medicīniskās datu bāzēs pieejamiem līdzīgiem pētījumiem. Ruzltāti. Pētījumā iesasistītas 132 agrīnas stadijas invazīva krūts vēža pacientes vecumā no 30 līdz 83 gadiem (vidējais vecums ir 56.02 ± 10.14 (SE = 0.89) gadi). Sargmezgli tika iezīmēti 71 (53.8%) pacientei ar kombinēto metodi un 61 (46.2%) pacientei ekskluzīvi ar izosulfāna zilo. 40 (30.3%) pacientēm sargmezgla biopsijas atradne bija pozitīva un veikta paduses limfmezglu disekcijas, kura rezulāts ir sekojošs: 31 (77.5%) pacientei metastāzes konstatētas tikai sargmezglā, 8 (20.0%) gan sargmezglā, gan paduses limfmezglos, 1 (2.5%) pacientei konstatēts viltusnegatīvs rezultāts. Secinājumi. Pētījums apstiprina iepriekš publicētus datus par to, ka kombinēta sargmezglu iezīmēšanas metode ir drošāka un tai jādod priekšroka, salīdzinājumā ar iezīmēšanu ekskluzīvi ar izosulfāna zilo. Sargmezglu biopsijas sensetivitāte pētījuma ietvaros sastāda 97.5%, viltusnegatīva rezultāta incidence - 2.5%. Īpaši atlasītai pacienšu grupai ar pozitīvu sargmezgla biopsijas atradni ir iespējams neveikt paduses limfmezglu disekciju, aprobežojoties ar sargmezgla disekciju. Literatūrā pieejāmie kritēriji augšminētai atlasei, iespējami, nav pietiekoši jūtīgie, ir jāmeklē papildus kritēriji. Actuality of the problem Sentinel lymph node biopsy is a minimally invasive method of regional control, wich enables us to diminish morbidity associated with another (used exclusively up until 1994) method of regional control - axillary lymph node dissection. Objectives To understand the correlation between sentinel lymph node biopsy and metastasis in axillary lymph nodes in early-stage invasive breast cancer patients. Methods Actual study is descriptively retrospective. We have collected medical a history of early-stage (I, IIA, IIB) invasive breast cancer patients, who underwent sentinel lymph node biopsy followed by appropriate surgical procedures (breast-conserving operative treatment, modified radical mastectomy, breast amputation) in breast cancer department, Oncology Centre of Latvia, between January 10th 2010 and December 20th 2012. The resulting data was analyzed using SPSS software, all the results compared with similar studies available throughout medical data bases. Results 132 patients, ages 30-83 (average - 56.02 ± 10.14 (SE = 0.89) y.o.) were enrolled according to criteria of the study. For lymphatic mapping in 71 patients (53.8%) a combined method was used, whereas, in 61 patients (46.2%) only blue dye was used. In 40 patients (30.3%) sentinel lymph node biopsy was positive. All of these patients underwent axillary lymph node dissection, and the results were as follows: in 31 patients (77.5%) metastases were found in sentinel lymph node only, in 8 patients (20.0%) - both in sentinel lymph node and in axillary node, in 1 patient (2.5%) skip metastasis was detected. Conclusion The current study confirms results of previous studies, namely, that combined lymphatic mapping method is more reliable than mapping using solely blue dye. For that reason it is more preferable. Within the framework of the study, sensitivity of sentinel lymph node biopsy was 97.5%, the false-negative rate was 2.5%. In conclusion, it is possible to select sentinel lymph node positive patients, in whom only sentinel node dissection may be used as a regional method of control, without further axillary lymph node dissection. The criteria for this selection found in literature appears to be not sensitive enough, that’s why a new set of examinations might be necessary in order to identify new criteria for selecting sentinel lymph node patients among whom only sentinel lymph node dissection could be performed.
