Kolorektālā vēža skrīninga mērķa grupas atsaucību ietekmējošie faktori

Aktualitāte. Latvijā kolorektālais vēzis (KRV) ir otra visbiežāk sastopamā onkoloģiskā saslimšana, sekojot krūts vēzim. Katru gadu Latvijā ar kolorektālo vēzi saslimst vairāk nekā 1000 iedzīvotāji, no kuriem lielākā daļa tiek diagnosticēti novēloti, līdz ar to mirstība sastāda aptuveni 700 cilvēku gadā. Kopš 2009. gada Latvijā ir ieviests KRV skrīnings, diemžēl atsaucība Latvijā ir zema - 2014. gadā atsaucības līmenis bija 10.59%. Eiropas Komisijas noteiktas vēlamais minumums ir 45% no populācijas. Darba mērķis ir noskaidrot un raksturot faktorus, kas veicināja iedzīvotājus piedalīties organizēta kolorektālā vēža skrīninga programmā. Materiāli un metodes. Diplomdarba dati tika iegūti no pētījuma "Organizēta kolorektālā vēža skīninga pilotpētījums Latvijā" gala ziņojuma datiem. Pētījuma kopa bija 15 000 nejauši atlasīti Latvijas Republikas iedzīvotāji, vecuma grupā 50-74 gadi. Pētījumā iekļautos iedzīvotājus sadalīja trīs grupās, katrā pa 5000 iedzīvotājiem un viņiem randomizēti tika izsūtīta viena no trim pētījumā izmantotajām testsistēmām slēpto asiņu noteikšanai fēcēs. (Kvantitatīvais imunoloģiskais tests OC- Sensor, kvantitatīvais imunoloģiskais tests FOB Gold, Gvajaka tests - gFOBT HemoCare). Dalībniekiem tika izsūtīta arī aptaujas anketa, ar jautājumiem par skrīningu un iemesliem, kas pamudināja piedalītites tajā. Diplomdarba ietvaros tika analizēti, to iedzīvotāju dati un aptaujas anketu rezultāti, kuri piedalījās kolorektālā vēža skrīninga pilotpētījumā. Rezultāti. Pētījumā kopumā no 14 656 izsūtītajiem testiem, atpakaļ tika saņemti un izpildīti 6023 (41.1%) testi. Pētījumā lielāka atsaucība bija sieviešu vidū - piedalījās 3937 (46.1%; TI 45.1-47.2) sievietes un 2086 (34.1%; TI 32.9-35.3) vīriešu. Vismazākā atsaucībā bija no iedzīvotājiem vecuma grupā 50-54 gadi - 35.3% (n=1323; TI 33.8-36.8). Vislielākā atsaucība bija vecuma grupā 65-69 gadi - 45.4% (n=1089; TI 43.5-47.4), 70 un vairāk gadi - 44.8% (n=1098; TI 42.9-46.8). Vislielākā atsaucība bija no tiem iedzīvotājiem, kuri bija saņēmuši imunoloģiskos testus - OC-Sensor - 47.4% (n=saņemtie 2307/nosūtītie 4871; TI 45.9-48.8) un FOB GOLD- 44.7% (n=saņemtie 2191/nosūtītie 4899; TI 43.3-46.1) (skat.2.attēlu). Savukārt vismazākā atsaucība bija no tiem iedzīvotājiem, kuri saņēma Gvajaka testu - 31.2% (n=saņemtie 1525/nosūtītie 4886; TI 29.9-32.5). Aptaujas anketā lielākā daļa no iedzīvotājiem - 56.2% (n=2809) minējuši, ka testu pamudinājusi veikt saņemtā vēstule, 41.2% (n= 2061) - rūpes par savu veselību, 39.7% (n= 1986) - iespēja testu veikt mājas apstākļos neapmeklējot ģimenes ārstu vai citu speciālistu. Secinājumi. Pētījumā lielāka atsaucība bija sieviešu vidū, iedzīvotājiem vecuma grupā 65-69 gadi, kā arī tiem, kas dzīvo novados. Iedzīvotāju atsaucību piedalītites skrīningā ietekmē tādi faktori kā - saņemtā testa veids, saņemtā skrīninga uzaicinājuma vēstule, rūpes par savu veselību, kā arī iespēja veikt testu mājas apstākļos, neapmeklējot ārstu. Introduction Colorectal cancer (CRC) is the second most prevalent type of cancer in Latvia. There are 1000 new cases or CRC every year and most of them are diagnosed in late stages, leading to mortality in aproximately 700 cases per year. CRC screening has been implemented in Latvia since 2009, however, it does not cover the whole population. In 2014 screening uptake in Latvia was disappointinly low - 10.59%. Aim of this study is to define and characterize the factors that encouraged to take part in the organised colorectal cancer screening in Latvia. Materials and methods. Materials for this study was taken from the data of the pilotstudy "Organised colorectal cancer screening pilot study in Latvia". Fifteen thousand randomly selected residents of Latvia aged 50–74 years were invited directly to perform a screening test. The target population received one of three different test systems: either a guaiac faecal occult blood test (gFOBT) or one of two laboratory-based immunochemical tests (FIT) – FOB Gold or OC-Sensor. The participants were asked to fill-in a self administred multianswer questionnaire and specific questions were used to find the main reasons why the individual did want to participate in the study. Author will describe and analyse factors that apply to those individuals who took part in the screening. Results Altogether, 14656 tests were mailed and 6023 tests were returned. Uptake for the entire study population was 41.1%. Uptake for was significantly lower among men than women. There were 2086 (34.1%; TI 32.9-35.3) male participiants and 3937 (46.1%; TI 45.1-47.2) female participiants. The lowest uptake was from citizens in age group 50-54 years old - 35.3% (n=1323; TI 33.8-36.8). The highest uptake was from those citizens who were aged 65-69 years old - 45.4% (n=1089; TI 43.5-47.4), 70 and older - 44.8% (n=1098; TI 42.9-46.8). Uptake for OC-Sensor test was - 47.4% (n=recieved/ 2307/sent 4871; TI 45.9-48.8) and for FOB GOLD test - 44.7% (n=recieved 2191/sent 4899; TI 43.3-46.1) The lowest uptake was from those who recieved gFOBT tests - 31.2% (n=recieved 1525/sent 4886; TI 29.9-32.5). As the main contributing factors for participating in screening the citizens marked - screening invinvitation letter - 56.2% (n=2809), concern about their health - 41.2% (n=2061), the opportunity to take the test at home - 39.7% (n=1986). Conclusions. Women and citizens in the age group 65-69 years old, had the highest uptake for the screening. Screening uptake was also significantly affected by the recieved form of the test. The main contributing factors for taking part in screening were - recieving screening participation letter, concerns about health and the opportunity to take the test at home.
