Divu Helicobacter Pylori asins plazmas testu un fēču antigēnu testa precizitātes izvērtējums pacientiem Latvijas populācijā

Ievads. Populācijās ar augstu Helicobacter pylori (H.pylori) izplatību rekomendē „pārbaudi-un-ārstē” stratēģiju, kur priekšroka ir dodama neinvazīviem testiem, no kuriem ekonomiski izdevīgāki ir antivielu un fēču antigēnu testi. Darba mērķis. Noteikt H. pylori neinvazīvo diagnostisko testu (lateksa-aglutinācijas, ELISA (ar enzīmu saistīta imūnfermentatīvā metode), fēču antigēnu testa) precizitāti, salīdzinot tos ar histoloģiju un noteikt piemērotāko skrīninga metodi H. pylori infekcijas diagnostikai Latvijas populācijā. Materiāli un metodes. Tika analizēti pacientu dati vecumā no 40 līdz 64 gadiem (GISTAR pilotpētījuma ietvaros), kuriem tika piedāvāts veikt ezofagogastroduodenoskopiju. Pacienti, kas saņēma H.pylori eradikācijas terapiju 12 mēnešus pirms iesaistīšanās pētījumā, tika izslēgti. Pacienti vienu mēnesi pirms fēču analīžu nodošanas tika aicināti nelietot protonu sūkņu inhibitorus, antibakteriālos līdzekļus vai bismuta preparātus. Antivielas asins plazmā tika noteiktas pēc divu veidu metodēm lateksa-aglutinācijas un ELISA. H.pylori antigēns fēcēs tika noteikts ar monoklonālo fēču antigēnu testu no iepriekš sasaldētiem paraugiem (-80oC). Baktērijas verifikācijai no biopsijā iegūtā materiāla tika izmantots modificēts Gimzas krāsojums. Tika noteikta neinvazīvo testu precizitāte, konstruētas pētāmo parametru darbības raksturlīknes (ROC līkne), aprēķināts Jūdena indekss un testu optimālā robežvērtības, noteikta pētīto testu precizitāte pacientiem ar samazinātu pepsinogēna līmeni. Rezultāti. Kopējais iekļauto pacientu skaits bija 1717 (47,35% vīrieši, vidējais vecums 51,60 gadi ± 6,73). Lateksa aglutinācijās testa (n=779) sensitivitāte, specifitāte, pozitīvā prognotiskā vērtība, negatīvā prognotiskā vērtība un precizitāte, attiecīgi, bija 85,96; 81,11; 86,53; 80,37; 83,95%, ELISA testam (n=1002) – 96,75; 71,53; 82,60; 94,03; 86,23%, savukārt, monoklonālo fēču antigēnu testam (n=672) – 87,13; 80,60; 87,13; 80,60; 84,52%. Testu precizitāte pacientu grupā ar samazinātu pepsingēna līmeni – lateksa-aglutinācijas testam bija 81,74%, ELISA – 77,68% un fēču antigēnu testam – 75,31%. Secinājumi. Kopumā Latvijas populācijā visu analizēto testu precizitāte bija zema. Turpretim ELISA testa rekomendējamā robežvērtība būtu jāpaaugstina. Kā H.pylori skrīninga metodi Latvijas populācijā varētu izmantot ELISA testu, papildinot izmeklējumu ar elptestu. ELISA un fēču antigēna testa precizitātes samazinājums pacientiem ar zemu pepsinogēna līmeni varētu būt saistīts ar kuņģa gļotādas atrofijas procesu un baktērijas izzušanu. Atslēgvārdi. In Helicobacter pylori (H.pylori) high prevalence populations is recommended “test-and-treat” strategy, where preferable are non-invasive tests, of which antibody and stool antigen tests are most attractive from the cost perspective. Objective. To assess the accuracy of non-invasive diagnostic tests (latex-agglutination, ELISA (enzyme – linked immunosorbent assay), stool antigen test) compared to histology and to find out most suitable screening method for diagnosing H.pylori infection in patient population in Latvia. Materials and methods. 40-64 years old generally healthy individuals from a population-based pilot study GISTAR that were referred for upper endoscopy. Individuals having received H.pylori eradication 12 months prior to the enrolment were not enrolled. Patients were asked not to use proton pump inhibitors, antibacterial medications or bismuth compounds one month before investigations. Antibodies to H.pylori were assessed in blood/plasma by two systems (latex-agglutination and ELISA). Antigen in faecal samples was detected by stool antigen test from frozen samples (-80oC). For bacteria verification from biopsy obtained material was used modified Giemsa staining method. Accuracy, Youden index and tests’ optimal cut-off values, constructing receiver-operating characteristic curves (ROC), test accuracy in patient group with low pepsinogen levels, were assessed. Results. A total of 1717 patients were recruited (males 47.35%, the mean age 51.60 years ±6.73). Sensitivity, specificity, positive predictive value, negative predictive value and accuracy for latex-agglutination test (n=779) were 85.96; 81.11; 86.53; 80.37; 83.95%, for ELISA test (n=1002) – 96.75; 71.53; 82.60; 94.03; 86.23%, whereas, for monoclonal stool antigen test (n=672) – 87.13; 80.60; 87.13; 80.60; 84.52%, respectively. Accuracy in patient group with low pepsingen level for latex-agglutination test was 81.74%, for ELISA – 77.68% and for stool antigen test – 75.31%. Conclusions. Overall accuracy analyzed non-invasive tests was relatively low in Latvia population. Whereas, ELISA test recommended cut-off value should be raised. As screening method in Latvia population can be used ELISA test, complementing with urea breath test. Reduction of accuracy for ELISA and stool antigen test in patient group with low pepsinogen level could be associated with gastric mucosal atrophy and due to clearance of infection. Keywords.
