K.pneumoniae ir viena no biežākajām nozokomiālo infekciju izraisītājām visā pasaulē. Visbiežāk tā ierosina infekcijas intensīvās terapijas pacientiem, pacientiem pēc apdegumiem, transplantācijām un jaundzimušajiem. K.pneumoniae ir cilvēka normālās mikrofloras sastāvā, tā bieži ierosina infekcijas pacientiem ar imūnsupresiju. Arvien pieaug arī K.pneumoniae rezistence pret antibakteriālajiem līdzekļiem, kas būtiski palielina hospitalizācijas laiku un finansiālos izdevumus, kā arī pacientu mirstību visā pasaulē. Diplomdarba tēma ir: Klebsiella ģints baktēriju rezistence pret antibakteriālajiem līdzekļiem. Darba mērķis ir izdalīt, identificēt un noteikt Klebsiella pneumoniae rezistenci pret antibakteriālajiem līdzekļiem. Tika apkopoti un analizēti K.pneumoniae paraugi laika posmā no 2016.gada janvārim līdz 2016.gada decembrim. K.pneumoniae jutības noteikšanai pret antibakteriālajiem līdzekļiem tika izmantoti divi testi: Kirby–Bauer disku difūzijas tests un E-tests. ESBL producējošās K.pneumoniae tika noteiktas ar dubultdisku metodi. K.pneumoniae ātrai identifikācijai no hemokultūras tika izmantota GenoType gramnegative BC metode. No pacientu elpceļiem tika izdalīti 155 K.pneumoniae izolāti. Tika noteikta K.pneumoniae rezistence pret sešiem antibakteriālajiem līdzekļiem. Visbiežāk rezistence tika noteikta pret ceftazidīmu – 21,94%, kas ir 34 baktēriju izolāti. Pret ciprofloksacīnu rezistenti bija 27 baktēriju izolāti (17,2%), pret piperacilīnu/tazobaktāmu – 7 izolāti (4,52%), savukārt pret karbapenēmiem tika noteikta 100% jutība. Kopējais ESBL gadījumu skaits ir 21,9% (n- 34) baktēriju izolāti. Most frequently it causes infections in patients of intensive therapy, patients after burns, transplantations and newborn children. K.pneumoniae is in the content of normal microflora ofa human body; it often causes infections in patients with immunosuppression. In addition, the resistance of K.pneumoniae to antibacterial agents is augmenting, what significantly increases the time of hospitalization and financial expenses as well as mortality of patients all over the world. Theme of the diploma paper is: resistance of bacteria of genus Klebsiella to antibacterial agents. Goal of the paper is to distinguish,identify and determine the resistance of Klebsiella pneumoniae to antibacterial agents. The specimens of K.pneumoniae were summarized and analysed in the period from January 2016 until December 2016. To determine the susceptibility of K.pneumoniae to antibacterial agents, the author used two tests: Kirby–Bauer disc diffusion test and E-test. The ESBL-producing K.pneumoniae were detected using double disc method. For a fast identification of K.pneumoniae from hemoculture the author used the method of GenoType BC gramnegative. There were 155 K.pneumoniae isolates distinguished from the patients’ respiratory tracts. The author determined the resistance of K.pneumoniae to six antibacterial agents. The most frequently the resistance was to ceftazidime – 21.94%, which is 34 bacterial isolates. 27 bacterial isolates were resistant to ciprofloxacin(17.2%), 7isolates (4.52%)– to piperacillin/tazobactam, and there was 100% susceptibility to carbapenems determined. The total number of ESBL cases is 21.9% (n- 34) bacterial isolates. Key words: K.pneumoniae, antimicrobial resistance, ESBL
