Pirmā tipa cukura diabēta pacientu klīniskais novērojums endokrinologa praksē: metabolās kompensācijas novērtējums, akūto komplikāciju blakusslimību analīze

Pētījuma darba galvenais mērķis ir analizēt dažāda smaguma hipoglikēmiju biežumu pirmā tipa cukura diabēta pacientiem un salīdzināt to ar diabēta metabolo kompensāciju. Materiāli un metodes: pētījumā tika analizētas „Latvijas Universitātes Medicīniskās pēcdiploma izglītības institūta” Dr.med. Kristīnes Ducenas pacientu ambulatorās kartes. Tika analizēti pacientu dati ar apstiprinātu diagnozi – pirmā tipa cukura diabēts. Atsevišķi tika apstrādāti dati par pacienta dzimumu, vecumu, cukura diabēta ilgumu (gados), HbA1C līmeni un citām autoimūnām blakussaslimšanām. Atkarībā no HbA1c līmeņa, pētījuma dalībnieki tika sadalīti sekojošās grupās: ●Pirmā grpa- laba diabēta kompensācija (HbA1c <6,5 %), ●Otrā grupa- apmierinoša diabēta kompensācija (HbA1c= 6,5-7,5), ●Trešā grupa- slikta diabēta kompensācija (HbA1c >7,5%). Papildus informācija tika iegūta, izmantojot aptaujas anketu. Kopumā pētījumā piedalijās 29 pacienti, tika analizētas 29 aizpildītas anketas. Darba uzdevumi bija: 1)analizēt pirmā tipa cukura diabēta pacientu metabolo kompensāciju. Noskaidrot, kuriem pacientiem ir laba, apmierinoša vai slikta diabēta metabolā kompensācija. 2)analizēt HbA1c saistību ar hipoglikēmiju biežumu pētāmajās grupās 3)noskaidrot kā diabēta apmācība ietekmē metabolo kompensāciju pētāmajās grupās 4)analizēt pirmā tipa cukura diabēta un citu autoimūno slimību biežumu 5)analizēt korelāciju starp pacientu nosauktajiem HbA1c un glikozes līmeņiem pret objektīvajiem HbA1c un glikozes līmeņu datiem, kas ņemti no pacientu ambulatorajām kartēm. Rezultāti: Visbiežāk dažāda smaguma hipoglikēmijas tika konstatētas trešajā grupā, bet netika atrasta statistiski ticama korelācija starp grupām un vieglu (p=0,819), vidēji smagu (p=0,061) vai smagu (p=0,540) hipoglikēmiju epizožu biežumu. Netika atrasta arī statistiski ticamas atšķirības starp grupām un diabēta apmācību (p=0,552). No citām autoimūnām blakussaslimšanām visbiežāk tika diagnosticēts hronisks autoimūns tireoidīts jeb Hašimoto tireoidīts (20,7%) un Greivsa slimība (6,9 %). Abas vairogdziedzera saslimšanas vairāk tika konstatētas sievietēm. Šie dati atbilst literatūras avotiem. Pētījuma ietvaros tika konstatēts, ka starp HbA1c un vidējiem glikēmijas rādītājiem (nosauc pacienti) pastāv statistiski ticama, vāja pozitīva korelācija (R (s) = 0,425, p = 0,021). Tika pierādīta, ka pastāv statistiski ticama, mērena korelācija starp vidējo glikozes līmeni (nosauc pacienti) un HbA1c (nosauc pacienti) mērījumu (R (s) = 0,551, p = 0,002), kā arī konstatēta statistiski ticama, stipra, pozitīva korelācija starp HbA1c mērījumu (dati iegūti no ambulatorajām kartēm) un pacientu nosaukto HbA1c mērījumu (R (s) = 0,680, p< 0,001). The main objective of this study was to analyze the frequency of different severity of hypoglycemia of patients with type one diabetes and compare it with the frequency of metabolic compensation. Materials and methods: The study analyzed Latvian University Medical Postgraduate Education Institute`s ambulatory patient cards of Dr. Kristine Ducena. It focused onthe data of patients with a confirmed diagnosis - Type 1 diabetes. Separately data on the patient's gender, age, diabetes duration in years, HbA1c levels and other autoimmune concomitants were processed. Depending on the level of HbA1c, participants of this study were divided into the following three groups: • first grupa- good compensation of diabetes (HbA1c <6.5%), • second grupa- satisfactory compensation of diabetes (HbA1c = 6.5-7.5) • third grupa- poor diabetic compensation (HbA1c> 7.5%). Additional information was obtained through a questionnaire. Overall, the data was analyzed from 29 questionnaires. Terms of reference were: 1) to analyze the metabolic compensation of the type one diabetic patients. To find out which of diabetic patients have good, satisfactory or poor metabolic compensation. 2) to analyze correlation of study groups HbA1c with hypoglycemia 3) to find out how the diabetes training affects the study groups metabolic compensation 4) to analyze the type 1 diabetes and other autoimmune disease frequency 5) To analyze the correlation between the patient listed HbA1c and glucose levels against objective HbA1c and glucose levels taken from outpatient cards. Results: The most common varying severity of hypoglycaemia were found in Group three, but study did not finde a statistically valid correlation between groups and miled (p = 0.819), moderate (p = 0.061) or severe (p = 0,540) episodes of hypoglycemia. There statistically significant differences between groups and the diabetic training (p = 0.552) could not be found. From other autoimmune concomitant diseases chronic autoimmune thyroiditis or Hashimoto's thyroiditis (20.7%) and Graves' disease (6.9%) most often were diagnosed. Both thyroid diseases are mostly detected in females. The study found that between HbA1c and medium glycemic parameters (acquired from the patients) there was a statistically valid, weak positive correlation (R (s) = 0.425, p = 0.021). The study also found that there is a statistically valid moderate correlation between the average level of glucose (acquired from the patients) and HbA1c measurements (acquired from the patients) (R (s) = 0.551, p = 0.002), as well as that there was a statistically valid, strong positive correlation between HbA1c measurements (data acquired from the outpatient cards) and HbA1c eports (data aquired from pacients) (R (s) = 0,680, p <0.001).
