Pēcoperācijas analgēzijas līmeņa izvērtēšana pēc n. ischiadicus bloka ar multimodālās analgēzijas protokolu apakšstilba lūzuma gadījumā

Neskatoties uz mūsdienu medicīnas attīstību, augsts pēcoperācijas sāpju līmenis vēl aizvien ir aktuāla problēma, īpaši traumatoloģijā. Apakšstilba lūzuma rekonstrukcijas operācijās kā pamata anestēzijas metode tiek lietota spinālā anestēzija (SA). Tiek meklētas papildus iespējas, kuras iekļaut multimodālās analgēzijas shēmā, lai efektīvi kontrolētu sāpes. N. ischiadicus bloks (NIB) varētu būt efektīvs perifēriskās nervu blokādes veids, kas samazinātu pēcoperācijas sāpju līmeni. Mērķis: samazināt pēcoperācijas sāpes pēc apakšstilba lūzumu rekonstrukcijas operācijām, izmantojot NIB. Materiāli un metodes: Prospektīvs, randomizēts pētījums. Pacienti, kuriem tika veikta apakšstilba lūzuma rekonstrukcijas operācija, tika sadalīti divās grupās: I grupa - SA grupa – SA, izmantojot Sol. Bupivacaini 2-4 ml 0,5%; II grupa - SA+NIB grupa – SA un NIB, izmantojot Sol. Ropivacaini 20–40 ml 0,375%. NIB veikšanai tika izmantota posterior poplitea pieeja USG kontrolē. Pacienti pēcoperācijas periodā saņēma pretsāpju medikamentus pēc standartizētas multimodālās analgēzijas shēmas. Kā glābejmedikaments tika lietots morfijs 10 mg p/o, ja sāpju līmenis > 6 balles pēc NRS. Pēcoperācijas periodā pacienti pildīja sāpju dienasgrāmatu, izmantojot NRS. Iegūtie dati tika apstrādāti izmantojot statistikas programmu IBM SPSS Statistics 23.0 versiju. Rezultāti: 52 pētījuma pacienti tika sadalīti 2 grupās: SA grupā 25 (44,46%) pacienti un SA+NIB grupā 27 (55,36%) pacienti. Sāpju līmenis uzreiz pēc operācijas SA grupā pret SA+NIB grupu: 0,4:0. Sāpju 2 h pēc operācijas SA grupā pret SA+NIB grupu: 1,24:0,22 (p<0,05). Sāpju līmenis 4 h pēc operācijas SA grupā pret SA+NIB grupu: 2,52:1 (p<0,05). Sāpju līmenis 6 h pēc operācijas SA grupā pret SA+NIB grupu: 5,24:2,74 (p<0,05). Sāpju līmenis 8 h pēc operācijas SA grupā pret SA+NIB grupu: 4,68:3,44. Sāpju līmenis 10 h pēc operācijas SA grupā pret SA+NIB grupu: 4,16:4,07. Statistiski ticamas atšķirības attiecībā uz morfija lietošanu netika atrastas. Secinājumi: 1. Gan SA, gan tās kombinācija ar NIB efektīvi samazina sāpes agrīnajā pēcoperācijas periodā. 2. SA papildināšana ar NIB pagarina pēcoperacijas analgēzijas periodu par sešām stundām. 3. NIB veikšana klīniski nepagarina laiku līdz morfija lietošanas uzsākšanai, kā arī neietekmē saņemtās morfija devas lielumu. Despite modern medical development, high levels of post-operative pain are still an ongoing problem, particularly in traumatology. Spinal anaesthesia (SA) is used as a basic anaesthesia method in the lower extremity fracture reconstruction surgeries. New additional opportunities for multimodal analgesia are being researched to effectively control the pain. N. ischiadicus block (NIB) could be a suitable peripheral nerve block form that would reduce the level of post-operative pain and improve the quality of patient life. Objective: reduce post-operative pain after lower extremity reconstruction surgeries by using NIB. Materials and methods: A prospective and randomised study. Patients who underwent a lower extremity fracture reconstruction surgery were divided in two groups: I group – SA group – SA via Sol. Bupivacaini 2-4 ml 0.5%; II group – SA+NIB group – SA plus NIB with Sol. Ropivacaini 20-40 ml 0.375%. An USG guided posterior popliteal approach was used for NIB. Patients received painkillers after a standardised multimodal analgesia scheme during post-operative period. Morphine 10 mg p/o was used, if the pain level was higher that 6 (NRS). In the post-operative period, patients completed a pain dairy using the NRS. The data was processed using IBM SPSS Statistics 23.0 for statistical analysis. Results: Overall of 52 patients, SA group - 25, SA+NIB group – 27. The subjective pain level right after the surgery in SA group vs SA+NIB group: 0.4 : 0. The subjective pain 2 hours the after surgery in SA group vs SA+NIB group: 1.24 : 0.22 (p<0.05). The subjective pain level 4 hours after the surgery in SA group vs SA+NIB group: 2.52 : 1 (p<0.05). The subjective pain level 6 hours after the surgery in SA group vs SA+NIB group: 5.24 : 2.74 (p<0,05). The subjective pain level 8 hours after surgery in SA group vs SA+NIB group: 4.68 : 3.44. The subjective pain level 10 hours after surgery in SA group vs SA+NIB group: 4.16 : 4.07. No statistically significant differences were found in the usage of morphine. Conclusions: 1. Both SA and its combination with NIB effectively reduce the pain in the early post-operative period. 2. The addition of SA with NIB extends the post-operative analgesia period by six hours compared with SA alone. 3. NIB does not clinically prolong the time until morphine is given, and has no effect on the dose of morphine received.
