dc.description.abstract | Hroniskai obstruktīvai plaušu slimībai (HOPS) ir raksturīga progresējoša un neatgriezeniska plaušu funkciju pasliktināšanās, ko izraisa hroniska elpceļu obstrukcija. Elpceļu obstrukciju novērtē, izmantojot spirometriju. HOPS pašlaik ir ceturtais no biežākajiem mirstības iemesliem Eiropā un ASV, taču tiek prognozēts, ka, smēķētāju skaitam pieaugot, 2020. gadā šī slimība būs jau trešais biežākais nāves cēlonis. Šī darba mērķis bija novērtēt, vai pastāv atšķirības starp iekaisuma procesa mehānismiem centrālajos un perifērajos elpceļos nesmēķētājiem, asimptomātiskiem smēķētājiem un vidēji smagas pakāpes HOPS pacientiem. Darba uzdevumi. Primārais: kvantitatīvi novērtēt iekaisuma šūnu infiltrāciju, nukleārā faktora-κBp65 (NF-κBp65) un 2. tipa histonu deacetilāzes (HDAC2) ekspresiju, kā arī T regulatoro limfocītu (Treg) iesaistīšanos centrālajos un perifērajos elpceļos nesmēķētājiem, smēķētājiem ar normālu plaušu ventilācijas funkciju un HOPS pacientiem. Sekundārais: novērtēt iespējamu korelāciju starp T regulatoro limfocītu, CD8+ T limfocītu, makrofāgu un neitrofīlo leikocītu skaitu, kā arī NF-κBp65 un HDAC2 ekspresiju starp elpošanas funkcionālajiem rādītājiem un izsmēķētajiem pakgadiem. Metodes. Pētījumā tika izmantots histoloģiskais materiāls, ko ieguva no 72 pacientiem, kuri tika operēti, sakarā ar nesīkšūnu plaušu vēzi. Izmeklējamās personas tika iedalītas trijās grupās: nesmēķētāji ar normālu plaušu ventilācijas funkciju (n=19), smēķētāji ar normālu plaušu ventilācijas funkciju (n=28) un vidēji smagas pakāpes HOPS pacienti (n=25). Lai novērtētu centrālos elpceļus, plaušu audu operācijas materiālā izmeklējām segmentāros bronhus, bet perifēro elpceļu analīzei - subpleirālu plaušu audu fragmentu. No tiem tika pagatavoti histoloģiskie preparāti, kurus krāsoja imūnhistoķīmiski, lai parādītu NF-κBp65 un HDAC2 ekspresiju, kā arī noteiktu CD8 T limfocītus, neitrofīlos leikocītus, makrofāgus un T regulatoros limfocītus. Rezultāti. Pētījumā noskaidrots, ka gan asimptomātiskajiem smēķētājiem, gan HOPS pacientiem ir raksturīgas iekaisīgas pārmaiņas elpceļos. HOPS pacientiem ir izteiktāka centrālo un perifēro elpceļu infiltrācija ar nukleārā faktora-κBp65 (NF- κBp65) pozitīvām šūnām, kā arī CD8 limfocītiem, neitrofīlajiem leikocītiem un makrofāgiem, salīdzinājumā ar asimptomātiskajiem smēķētājiem un nesmēķētājiem. Savukārt, 2. tipa histonu deacetilāzes (HDAC2) ekspresija HOPS pacientiem ir izteikti nomākta perifērajos elpceļos, salīdzinājumā ar asimptomātiskajiem smēķētājiem. Pētījums ir parādījis nevienmērīgu iekaisuma šūnu infiltrāciju centrālajos un perifērajos elpceļos. Gan HOPS pacientiem, gan asimptomātiskajiem smēķētājiem neitrofīlo leikocītu un CD8 limfocītu infiltrācija ir izteiktāka perifērajos elpceļos, salīdzinājumā ar centrālajiem elpceļiem, bet, savukārt, makrofāgu infiltrācija HOPS pacientiem ir izteiktāka centrālajos nekā perifērajos elpceļos. Pētījumā konstatējām, ka nukleārā faktora-κB, 2. tipa histonu deacetilāzes ekspresija un iekaisuma šūnu (CD8 T limfocītu, neitrofīlo leikocītu un makrofāgu) skaits korelē ar elpošanas funkcionālajiem rādītājiem un izsmēķētajiem pakgadiem centrālajos un perifērajos elpceļos. Mūsu pētījumā ir parādīts, ka HOPS pacientiem un asimptomātiskajiem smēķētājiem T regulatoro limfocītu skaits ir palielināts centrālajos elpceļos, kas korelē ar izsmēķētajiem pakgadiem, savukārt, perifērajos elpceļos HOPS pacientiem novērojām to skaita samazināšanos, kas korelē ar elpceļu obstrukciju. | en_US |
dc.description.abstract | Chronic obstructive pulmonary disease (COPD) is characterized by
a slowly progressive and irreversible airway obstruction. Cigarette smoking is
implicated as a major risk factor for development of COPD because
approximately 90% of COPD patients are smokers. However, only a minority of
smokers develop COPD. The basic reason for this phenomenon is still poorly
understood. COPD is a leading cause of morbidity and mortality worldwide.
The aim of our study was to analyse the differences between inflammatory
responses in large and small airways of nonsmokers, asymptomatic smokers and
moderate COPD patients.
Objectives. The primary objective was to compare nuclear factor-κBp65 (NF-
κBp65) and histone deacetylase 2. type (HDAC2) expression, inflammatory cells
(CD8+ T lymphocytes, macrophages and neutrophils) infiltration and T regulatory
cells special arrangement and distribution in large ans small airways of
nonsmokers, asymptomatic smokers and COPD patients.
The secondary objective was to analyze the relationship between airways
obstruction and smoked pack-years and inflammatory cells and T regulatory
lymphocytes count and NF-κBp65 and HDAC2 expression.
Methods. 72 subjects undergoing lung resection for a solitary peripheral nonsmall
cell carcinoma were enrolled in the study. They were subdivided into three
groups: 19 subjects were nonsmokers with normal lung ventilation function, 28
subjects were current smokers with normal lung function and 25 current smoking
subjects had moderate COPD. The diagnosis of COPD was established according
to the definition of GOLD guidelines. COPD patients had FEV1/FVC<70% and 50
%< FEV1<80% of predicted. The lung tissue specimens for small airways
evaluation were taken from the sub-pleural parenchyma, but for large airways
bronchial rings were taken from the segmental bronchus of the lobe obtained at
surgery as far away as possible from the tumour site. Samples were fixed without
inflation in 10% neutral buffered formalin, processed and embedded routinely.
The histological specimens were stained with NF-κBp65, HDAC2, FOXP3
(marker for T regulatory cells), CD8, CD68 and neutrophils leukocytes elastase
antibody.
Results. Obtained results showed that both asymptomatic smokers and COPD
patients had increased inflammatory responses in airways compared to
nonsmokers. However, COPD patients had increased CD8 lymphocytes,
macrophages and neutrophils infiltration as well as NF-κBp65 expression in large
and small airways compared to asymptomatic smokers. At contrast, COPD
patients had decreased HDAC2 expression in the small airways compared to
nonsmokers and asymptomatic smokers.
Our study showed the non-uniform distribution through the bronchial tree of
inflammatory cells. Asymptomatic smokers and COPD had more CD8+ T
lymphocytes and neutrophils in small compared to large airways, but COPD
patients had more macrophages in large compared to small airways.
There was a negative correlation between NF-κBp65 positive cells in large and
small airways and FEV1%, FEV1/FVC% and a positive correlation with smoked
pack-years. Asymptomatic smokers had more CD8 lymphocytes, macrophages
and neutrophils in large compared to small airways, but COPD patients in small
compared to large airways. The negative correlation between inflammatory cells
infiltration in airways and FEV1%, FEV1/FVC% was observed.Smokers with
normal lung function and COPD patients had increased numbers of T regulatory
cells in large airways compared to nonsmokers. A positive correlation was
observed between T regulatory cells in large airways and smoked pack years.
In small airways, COPD patients had decreased numbers of T regulatory cells
compared to asymptomatic smokers and nonsmokers that negatively correlated
with airflow obstruction. | en_US |
