Sekretorā IgA un zarnu mikrobioma mijiedarbība un dinamika antidiabētiskās terapijas laikā

Metformīns ir viens no visplašāk izmantotajiem medikamentiem 2.tipa cukura diabēta (T2D) ārstēšanā. Papildus glikozes izdales samazināšanai aknās un glikozes uzņemšanas palielināšanai muskuļos, arvien vairāk pierādījumu liecina, ka zarnu mikrobioms ir metformīna galvenā darbības vieta. Jaunākie pētījumi liecina, ka metformīns būtiski maina zarnu mikrobiomu T2D pacientiem. Sekretorajam IgA ir svarīga loma zarnu, elpceļu un uroģenitālās gļotādas epitēlija aizsardzībā un homeostatiskā regulēšanā, kā arī mijiedarbībā ar mikrobiomu. Metformīna terapijas laikā tika ievākti fēču paraugi no 19 jaundiagnosticētiem T2D pacientiem (trīs laika punktos) un astoņām veselām kontrolēm (divos laika punktos), kurus raksturoja, izmantojot visa metagenoma sekvenēšanu. Iegūtie rezultāti liecina, ka metformīna lietošana pēc trīs mēneša metformīna terapijas T2D pacientiem novērojams Roseburia, Alistipes un Rikenellaceae lielāks baktēriju īpatsvars (Presort paraugos) un Proteobacteria sIgA+ frakcijās. Veseliem indivīdiem ir novērojams Enterobacterales, Proteobacteria un Gammaproteobacteria lielāks baktēriju īpatsvars IgA+ frakcijās. Metformin is one of the most commonly used drugs for the treatment of type 2 diabetes. In addition to reducing hepatic glucose output and increasing muscle glucose uptake, there is increasing evidence that the intestinal microbiome is the major site of action of metformin. Recent studies suggest that metformin significantly alters intestinal microbiome in patients with T2D and scientists have identified the gut as a potential target of metformin. The secretory immunoglobulin A (sIgA) plays an important role in the protection and homeostatic regulation of the intestinal, respiratory, and urogenital mucosal epithelium. During metformin therapy, fecal samples were collected from 19 newly diagnosed T2D patients (three time points) and eight healthy controls (two time points), which were characterized using whole metagenoma sequencing. Metformin administration in the T2D cohort induced an increase in abundance of Roseburia, Alistipes and Rikenellaceae (Presort samples) and increase of Proteobacteria in sIgA+ fraction. In healthy controls, we observed an increase in abundance of Enterobacterales, Proteobacteria, Gammaproteobacteria in IgA+ fraction.
