Dzīves kvalitātes novērtējums pacientiem ar primāru un sekundāru traheoezofageālās punkcijas protēzi pēc laringektomijas

Ievads: Totāla laringektomija tiek plaši uzskatīta par zelta standartu T3, T4 balsenes vēža ārstēšanā. Operācija rezultējas ar pilnīgu balss zudumu, kas negatīvi ietekmē pacientu un rada psihosociālu traumu. Ir vairākas balss rehabilitācijas metodes, taču par mūsdienīgāko un progresīvāko šobrīd tiek uzskatīta traheozeofageālās punkcijas protēžu implantācija. Galvenais mērķis, lietojot balss protēzi ir veiksmīgas balss rehabilitācijas sasniegšana, dzīves kvalitātes uzlabošana un komunikācijas atjaunošana. Darba mērķis: Šī pētījuma mērķis ir salīdzīnāt un izvērtēt dzīves kvalitāti pacientiem, kam veikta totāla laringektomija un ir ievietota primāra vai sekundāra traheoezofageālās punkcijas protēze. Materiāli un metodes: Prospektīvajā pētijumā tikai iekļauti pacienti no Rīgas Austrumu klīniskās universitātes slimnīcas Latvijas Onkoloģijas centra, kuriem ir ievietota primāra vai sekundāra traheoezofageāla punkcijas protēze pēc vai totālas laringektomijas laikā. Latvijas Onkoloģijas centra Galvas un kakla ķirurģijas nodaļā tika veikta anonīma, brīvprātīga pacientu anketēšana. Anketēšanā piedalījās 21 cilvēks, kas aizpildīja anketu ar 21 jautājumu. Anketas tika sastādītas, izmantojot Microsoft Word 2021 datorprogrammu. Visi iegūtie dati tika apstrādāti ar Microsoft Office Excel 2021 un RStudio. Rezultāti: Kopā pētījumā tika iekļauts un analizēts 21 pacients ar ievietotu primāru vai sekundāru traheoezofageālo punkcijas protēzi Latvijas Onkoloģijas centra, Galvas un kakla ķirurģijas nodaļā. Pētījumā piedalījās 19 vīrieši (90,5%) un divas sievietes (9,5%). Vidējais pacientu vecums bija 62,3 gadi, vecuma diapazonā no 47 līdz 76 gadiem. Secinājumi: Pacientiem ar primāro traheoezofageālās punkcijas protēzi un pacientiem ar sekundāro traheoezofageālo punkcijas protēzi dzīves kvalitāte būtiski neatšķiras. Introduction: Total laryngectomy is widely considered the gold standard for the treatment of T3, T4 laryngeal cancer. The surgery results in a complete loss of voice, which negatively affects the patient and creates psychosocial trauma. There are several methods of voice rehabilitation, but implantation of tracheoesophageal puncture prostheses is currently considered the most modern and advanced. The main goal when using a voice prosthesis is to achieve successful voice rehabilitation, improve the quality of life and restore communication. Aim: The aim of this study is to compare and evaluate the quality of life by patients who have undergone total laryngectomy and received either primary or secondary tracheoesophageal puncture prosthesis. Methods and materials: The prospective study included patients from the Latvian Oncology Center of the Riga Eastern Clinical University Hospital who had a primary or secondary tracheoesophageal puncture prosthesis inserted after or during total laryngectomy. An anonymous, voluntary patient questionnaire was conducted at the Head and Neck Surgery Department of the Latvian Oncology Center. In the questionnaire, 21 people filled out the questionnaire with 21 questions. Questionnaires were compiled using the Microsoft Word 2021 computer program. All obtained data were processed with Microsoft Office Excel 2021 and RStudio. Results: A total of 21 patients with primary or secondary tracheoesophageal puncture prosthesis inserted in the Head and Neck Surgery Department of the Latvian Oncology Center were included and analyzed in the study. 19 men (90.5%) and two women (9.5%) participated in the study. The average age of the patients was 62.3 years, with an age range of 47 to 76 years. Conclusions: Quality of life does not differ between patients with primary tracheoesophageal puncture prosthesis and secondary tracheoesophageal puncture prosthesis.
