Mērķis
Šī pētījuma mērķis ir analizēt pulsa viļņa īpašības un novērtēt centrālo asinsspiedienu kā kardiovaskulārā riska marķierus pacientiem ar koronāro sirds slimību, salīdzinot ar pacientiem bez koronārās sirds slimības, Latvijas sabiedrībā.
Metodes
Klīniskais pilotpētījums tika veikts ar 30 pacientiem (14 siev., 16. vīr.), kas bija nosūtīti veikt plānveida koronāro angiogrāfiju Latvijas Kardioloģijas centrā (LKC) P. Stradiņa Klīniskajā universitātes slimnīcā (PSKUS). Pacienti tika izvēlēti, pamatojoties uz vairākiem iekļaušanas/izslēgšanas kritērijiem. Katra pacienta izmeklēšana iekļāva sevī (1) KVS un KSS anamnēzes precizēšanu, un KVS konvencionālo riska faktoru izvērtēšanu. Tad sekoja (2) perifērā asinsspiediena mērīšana ar ierasto metodi brahiālajā artērijā, (3) centrālā asinsspiediena noteikšana un SphygmoCor pulsa viļņa analīze, izmantojot aplanācijas tonometriju radiālajā, karotīdajā un femorālajā artērijā, (4) slodzes tests, (5) ehokardiogrāfija, (6) laboratoriskie izmeklējumi un (7) koronārā angiogrāfija. Pacienti tika sadalīti grupās - ar hemodinamiski nozīmīgas stenozi (>50%) un bez hemodinamiski nozīmīgas stenozes (<50%) KSS, balstoties uz koronārās angiogrāfijas rezultātiem. Tika izveidota datu bāze rezultātu apkopošanai un to statistiskā analīze tika veikta, izmantojot SPSS 12.0.
Rezultāti
Pētījums apstiprināja, ka brahiālā asinsspiediena mērījumu rezultāti uzrāda lielākas vērtības,
t. i., lielaku spiedienu, nekā aortas asinsspiediena mērījumi, veikti ar aplanācijas tonometriju. KSS slimniekiem, kuriem ir hemodinamiski nozīmīgas stenozes (>50%), ir pierādīts, ka centrālais asinsspiediens ir paaugstināts, salīdzinot ar pacientiem, kuriem nav hemodinamiski nozīmīgas stenozes (<50%). Pētījumā tika konstatēts, ka centrālajam asinsspiedienam (p=0.001) ir spēcīgāka korelācija ar stenozes pakāpi nekā perifērā asinsspiediena mērījumiem (p=0.008) slimniekam ar KSS. Runājot par pulsa viļņa īpašībām, jāatzīmē, ka centrālais pulsa spiediens (p<0.001) un sekojošais aortas augmentācijas indekss (p<0.001) uzrādīja visspēcīgāko korelāciju ar stenozes pakāpi. Centrālais augmentācijas indekss (indekss I p=0.012 un indekss II p=0.014) uzrādīja vāju korelāciju ar koronāro artēriju stenozes pakāpi. Pulsa viļņa izplatīšanās ātrums (p=0.214) un Bukberga indekss (p=0.621) neuzrādīja korelāciju ar vainagartēriju stenozes pakāpi.
Seicinājumi
Konvencionālā brahiālā asinsspiediena mērījumu rezultāti uzrāda lielākas izmēklējuma rezultāta vērtības, nekā centrālā asinsspiediena mērīšana ar aplanācijas tonometriju.
Centrālā asinsspiediena mērījumiem ir lielāka korelācija ar koronāro artēriju stenozes pakāpi nekā brahiālā asinsspiediena mērījumiem.
Analīzejot pulsa viļņa īpašībās, tika konstātēts, ka centrālajam aortas augmentācijas indeksam un centrālajam pulsa spiedienam ir visspēcīgākā saistība ar koronāro artēriju stenozes pakāpi. Centrālais augmentācijas indekss uzrādīja vāju korelāciju ar koronāro artēriju bojājuma pakāpi. Pulsa viļņa izplatīšanās ātrums un Bukberga indekss neuzrāda korelāciju ar vainagartēriju bojājumu. Aim
This study aims to analyze pulse wave properties and to evaluate the central blood pressure as markers of cardiovascular risk in patients with coronary heart disease compared to patients without coronary disease in a Latvian population.
Methods
A clinical pilot study was carried out in 30 patients (14 females and 16 males) admitted for elective coronary angiography at the Latvian Center of Cardiology (LKC) in P. Stradins Clinical University Hospital (PSCUH). The patients were selected based on several inclusion and exclusion criteria. The examination of each patient included (1) taking of history of CHD and CVD and evaluation of CVD conventional risk factors. Then followed (2) peripheral blood pressure measurement with the conventional method on the brachial artery, (3) central blood pressure measurement and pulse wave analysis of SphygmoCor by applanation tonometry at the radial, carotid and femoral artery, (4) exercise test, (5) echocardiography, (6) laboratory analyses and (7) coronary angiography. The patients were subdivided into patients with (hemodynamically significant stenoses >50%) and without (hemodynamically significant stenoses <50%) CHD based on results from the coronary angiography. A database was created for collecting all results and statistical analysis was performed in SPSS 12.0.
Results
This study has confirmed that measurement of the brachial blood pressure by conventional method gave a higher value than the actual aortic blood pressure measured by applanation tonometry.
Patients with CHD who have hemodynamically significant stenoses (>50%) have been proved to have an increased central blood pressure compared to patients without hemodynamically significant stenoses (<50%). In this study it’s been found that the central blood pressure (p=0.001) has a stronger correlation to the degree of stenosis than has measurement of the peripheral blood pressure (p=0.008) in patients with CHD.
Of the pulse wave properties, the central pulse pressure (p<0.001) followed by the aortic augmentation index (p<0.001) showed the strongest correlation to the degree of stenosis in coronary arteries. Both central augmentation indexes (index I p=0.012 and index II p=0.014) showed a weak correlation to the degree of lesions in coronary arteries. The pulse wave velocity (p=0.214) and the Buckberg index (p=0.621) did not provide any correlation with the state of atherosclerotic lesions in the coronary arteries.
Conclusions
Measurement of the peripheral (brachial) blood pressure by conventional method gave a higher value than the central (aortic) blood pressure measured by applanation tonometry.
The central blood pressure has a stronger correlation to the degree of stenosis in coronary arteries than has measurement of the peripheral blood pressure in patients with CHD.
Analysis of the pulse wave properties revealed that the central aortic augmentation index and central pulse pressure has the strongest correlation to the degree of stenosis in coronary arteries. Both central augmentation indexes showed a weak correlation to the degree of lesions in coronary arteries. The pulse wave velocity and the Buckberg index did not provide any correlation with the state of lesions in the coronary arteries.
