Mans darbs ir veltīts stafilokoku virulences faktoru pētīšanai ar īpaši pievērstu uzmanību diviem izplatītākiem un ar cilvēku infekcijām saistītiem stafilokokiem: S. aureus un S. epidermidis sugām. Tas ir laboratorisks pētījums ar retrospektīvu un prospektīvu paraugu vākšanu. Diplomdarba praktiskā daļa ir izpildīta Traumatoloģijas un ortopēdijas slimnīcas bakterioloģijas laboratorijā.
Mana diplomdarba mērķis ir noteikt fenotipiskās atšķirības starp komensāliem un nozokomiāliem stafilokoku celmiem, galveno uzmanību pievēršot mikroorganismu hemolītiskās aktivitātes izvērtēšanai. Kopumā darba izpildīšanai tika savākta 141 mikroorganismu kultūra, no kuriem nozokomiālā grupā bija 88 baktēriju kultūras no klīniskā materiāla un komensālā grupā 53 baktēriju kultūras no veseliem cilvēkiem. Visi mikroorganismi tika audzēti uz 5 % cilvēka un 5 % aitas asins agara, lai noteikt α un δ hemolizīnus.
Nozokomiālās S. epidermidis kultūras cilvēka asins agārā ir ar zemāku hemolītisko aktivitāti ( 55,6%) kā komensālās ( 90%). Nozokomiālā grupā gan S. aureus, gan S. epidermidis hemolītiskā aktivitāte uz cilvēka un uz aitas asins agara bija stipri līdzīga, tātad būtiski neatšķīrās. Komensālā grupā S. epidermidis un S. aureus hemolītiskā aktivitāte būtiski atšķiras uz cilvēka asins agara un uz aitas asins agara.
No variablām bioķīmiskām īpašībām tika noteikta ureāze un sārmainā fosfatāze. Nozokomiāliem S. epidermidis 93.8 % ir pozitīva ureāzes un fosfatāzes reakcija.Ureāzes aktivitāte komensāliem celmiem neatšķiras, bet sārmainās fosfatāzes aktivitāte ir zemāka. Konstantās bioķīmiskās īpašības (katalāze, koagulāze, lecitināze) komensālos un nozokomiālos celmos neatšķiras.
Tika meklēta saistība starp meticilīnrezistenci un hemolītisko aktivitāti. Meticilīnsensitīviem S. epidermidis hemolītiskā aktivitāte bija ļoti augsta (84.4 % uz cilvēka asins agara). Savukārt meticilīnrezistento S. epidermidis grupā hemolītiskā aktivitāte nebija tik izteikta (57.5 %). No mūsu iegūtiem datiem var secināt, ka saistības starp meticilīnrezistenci un hemolīzi nav.
Darba gaitā tika novērots, ka hemolītiskā aktivitāte labāk izpaužas uz cilvēka asins agara, savukārt pigments bija labāk atšķirams uz aitas asins agara. My work is dedicated to the research of staphylococcal virulence factors. The two most common and human infection related staphylococci S. epidermidis and S. aureus were studied. This is laboratory research with retrospective and prospective sample collection. Practical part of the diploma work was performed in the bacteriology laboratory of the Traumatology and orthopedics hospital.
The goal of my diploma work is to identify phenotypical differences of nosocomial and commensal staphylococcal strains. During the work 141 microorganism cultures were collected, among which nosocomial group of 88 cultures was collected from clinical materials and the commensal one (53 cultures) - from healthy individuals. All of the microorganisms were incubated on 5% human and 5% sheep blood agar. This was done for specifying α and δ haemolysins.
Nosocomial S. epidermidis cultures, incubated on human blood agar, showed lesser haemolytic activity (55.6%) than the commensal strains. Haemolytic activity of both nosocomial S. aureus and nosocomial S. epidermidis cultures was similar on human and sheep blood agars. Commensal S. epidermidis and S. aureus showed essential difference of haemolytic activity on human and sheep blood agars.
The urease and the alkaline phosphatase were identified in studied strains. 93.8% of nosocomial S. epidermidis had positive urease and phosphatase reactions. Urease activity of commensal strains did not differ, but alkaline phosphatase activity was lower. Constant biochemical characteristics (catalase, coagulase, lecitinase) didn’t differ within commensal and nosocomial strains.
Relation of resistance to methicillin and haemolytic activity was studied. Haemolytic activity of methicillin-sensitive S. epidermidis was high (84.4% on human blood agar). However methicillin-resistant S. epidermidis haemolytic activity wasn’t so expressed (57.5%). Judging by the acquired data the following can be summarized, that there is no relation between methicillin resistance and haemolytic activity.
During the work it was also noticed that haemolytic activity was showed more frequently on human blood agar than on sheep one, in turn the pigment was better distinguishable on sheep blood agar.
