Pēdējo gadu laikā pieaug saslimstība ar žultsakmeņu slimību. Tā viens no iemesliem ir pusmūža un vecu cilvēku skaita pieaugums. Šī tendence veicina lielāku nepieciešamību pēc ķirurģiskas iejaukšanās. Pieaug arī žultsakmeņu komplikāciju: obstruktīva dzelte, ascendējošs holangīts un akūts biliārs pankreatīts biežums. Iejaukšanās žultsvados operatīvā ceļā pusmūžā rada augstu letalitātes risku. Endoskopiska retrogrādā holangiogrāfija ar papillotomiju nodrošina šo smago slimnieku ārstēšanu, neveicot konvencionālo ķirurģisko iejaukšanos. Darba mērķis: Izvērtēt holedoholitiāzes ārstēšanas taktiku, ERHP terapeitisko efektu un tās komplikācijas Rīgas 1. slimnīcā.
Darba materiāls un metodes: analizētas 242 pacientu slimības vēstures Rīgas 1. slimnīcas arhīvā, kuriem tika veikta ERHP. Retrospektīvi tika atlasīti 103 pacienti, ar simptomātisku holedoholitiāzi, bioķīmiskiem un ultrasonogrāfiskām aizdomām par holedoholitiāzi. Endoskopiska ārstēšana tika veiksmīgi pielietota 88 no 103 pacientiem. Holedoholitiāzes terapijas taktika atbilst pasaules atzītām tendencēm, kur pamatā pielieto ERHP terapeitisko metodi. Iegūtais ERHP komplikāciju skaits sastāda 31%, kas sakrīt ar vidējo Eiropā iegūto komplikāciju skaitu. In the last years, morbidity with gallstone disease has grown. One of the reason is incresing number of middle-aged and old people. This tendency promotes greater necessity for surgical intervention. Increses also an incidence of gallstone complications - obstructive jaundice, ascending cholangitis and acute biliary pancreatitis. Interference with bile duct through operation in middle-aged patient creates higher risk of lethality. Endoscopic retrograde cholangiography with sphincterotomy provides treatment for these severe patients, without using conventional surgical intervention. The objective of the work: to evaluate treatment technique of choledocholithiasis, treatment effect and complications with ERCP in Rigas 1. Hospital.
Work material and methods: analysed 242 pantients histories in Rigas 1. Hospital’s archive, which have had ERCP. Retrospectively were selected 103 patients with symptomatic cholelithiasis and with biochemical and ultrasonographic suspicion for choledocholithiasis. Endoscopic treatment was successfully used on 88 patients, from selected group (103 patients). Choledocholithiasis treatment technique correspond to world recognized tendencies, where basically is used ERCP as a treatment method. Obtained ERCP complications number constitutes 31%, which mach with average obtained complication number in Europe.
