Pamatojums. Cukura diabēta aprūpe ir kompleksa un tajā iekļauta ne tikai glikēmijas kontrole. Vadlīnijām ir ļoti nozīmīga loma, izvēloties piemērotu terapiju 2.tipa cukura diabēta pacientiem no vairākām esošām medikamentu grupām. Paliek neskaidrības, kādā mērā vadlīnijas tiek ievērotas.
Darba mērķis. Izvērtēt primāra 2. tipa cukura diabēta pacientu pirmreizējās medikamentozās terapijas atbilstību Latvijas 2007.gadā publicētajām 2.tipa cukura diabēta profilakses, diagnostikas un ārstēšanas vadlīnijām. Analizēt šo pacientu kardiovaskulāro slimību profilakses terapijas atbilstību šīm vadlīnijām.
Metodes. Pētījums tika veikts P. Stradiņa Klīniskās Universitātes slimnīcā, analizējot 53 pirmreizēji diagnosticētu 2. tipa cukura diabēta pacientu anketas un laboratorisko analīžu datus, kas tika ievākti laika periodā no 2010. gada augusta līdz 2011. gada janvārim. Medikamentu izvēle tika konstatēta pēc ārstējošā ārsta ieraksta anketā.
Rezultāti. Pētījumā analizēto pacientu kopā (n=53) orālo antidiabētisko ārstēšanu monoterapijā saņēma 69,8% (n=37). Monoterapiju ar metformīnu saņēma 52,8% (n=28), sulfanilurīnvielas grupas preperātiem 13,2% (n=7), ārstēšanu ar DPP-4i un TZD monoterapijā uzsāka ar katru 1,9% (n=1). Biežākā kombinētā ārstēšana bija metformīna un sulfanilurīnvielas preperāti – 17% (n=9) un metformīns ar TZD vai DPP-4i, abi 5,7% (n=3). Paaugstināts asinsspiediens (TA ≥130/80 mmHg) bija 79,3% (n=42). Antihipertensīvo terapiju saņēma 45,3% (n=24). Tādu pacientu, kuriem asinsspiediens bija ≥130/80 mmHg un viņi nesņēma nekādu antihipertensīvo ārstēšanu, bija 49,1% (n=26). Palielināts ZBLH (≥2 mmol/l) bija 94,3% (n=50). Terapiju ar statīniem saņēma 26,4% (n=14), pārējie 73,6% (n=39) nesaņēma nekādu lipīdus mazinošu terapiju.
Secinājumi. Pirmreizēja 2. tipa cukura diabēta orālā antidiabētiskā ārstēšana tiek veikta atbilstoši vadlīnijām. Tomēr kardiovaskulāro komplikāciju profilaksei jāpievērš lielāka vērība. Title. Background. Diabetes care is complex and invloves not only glycemic control but also many other factors, there is no simple solution for diabetes care. In treatment of type 2 diabetes patients guidelines play an extremely important role in making the appropiate choice from a number of existing drug groups. Uncertainty remains as to what extent the guidelines are being met.
Aim of the study. To evaluate initial oral antidiabetic treatment for newly diagnosed type 2 diabetic patients according to the 2007 national guidelines. To compare cardiovascular disease prevention therapy with the therapy detailed within the national guidelines.
Methods. The study was conducted at the P. Stradins Clinical University Hospital. The study analyzed questionaires and laboratory data of 53 newly diagnosed type 2 diabetes mellitus patients. The data was collected from August 2010 to January 2011 and the medication preference was recorded by the physician within the questionnaire.
Results. Treatment was initiated with one oral antidiabetic drug in 69,8% (n=37). Monotheraphy with metformin was started in 52,8% (n=28), with sulfanilurea in 13,2% (n=7), with DPP-4i and TZD 1,9% (n=1) each. Most frequent combination was metformin with sulfanilurea - 17% (n=9) and metformin with TZD or DPP-4i, each 5,7% (n=3).Increased blood preassure (TA ≥130/80 mmHg) was found in 79,3% (n=42). 45,3% (n=24) were treated for hypertension. 49,1% (n=26) of patients with high blood preassure (TA ≥130/80 mmHg) recieved no treatment for hypertension.
Increased LDL of ≥2 mmol/l was found in 94,3% (n=50). 26,4% (n=14) received theraphy with statins, the rest 73,6% (n=39) did not get any treatment for dislipidemia.
Conclusions. Oral antidiabetic therapy in newly diagnosed type 2 diabetes patients were prescribed in line with the national guidelines. The possibility of cardiovascular complications in patients with type 2 diabetes must be recognised and treatment more widely implemented.
