Klopidogrela rezistences biežums, tās riska faktori un ārstēšanas taktika pacientiem, kam veikta perkutāna koronāra intervence ar DES implantāciju

Klopidogrels ir selektīvs, neatgriezenisks P2Y12 receptoru antagonists. Tam ir pierādīta efektivitāte un drošums pie akūta koronāra sindroma, kā arī pēc perkutānas koronāras intervences (PCI) ar un bez stenta implantācijas. Vislielākā nozīme klopidogrelam ir pēc ar zālēm pārklātā stenta (DES) implantācijas, jo pastāv vislielākais stenta trombožu risks. Pasaulē ir dati par interindividuālu, samazinātu reakciju uz klopidogrelu un ar to saistītu paaugstinātu kardiovaskulāro notikumu risku. Mērķis: noskaidrot klopidogrela rezistences izplatību pacientiem pēc DES implantācijas Latvijā. Metode: Pacienti prospektīvi iekļauti pēc PCI ar DES implantāciju, anketēti, noteikts trombocītu reaktivitātes indekss (PRI), izmantojot VASP metodi. Rezultāti: rezistences izplatība ir augsta – 73%. Leikocitoze (p < 0,001), hipertrigliceridēmija (p = 0,001), hiperglikēmija (p = 0,002), zema ABLH koncentrācija (p = 0,017), ilgstoša hipertensija (p = 0,009), ilgstoša smēķēšana (p = 0,012), kā arī samazināta kreisā kambara izsviedes frakcija (p = 0,012) ir ticami klopidogrela rezistences riska faktori. Klopidogrela rezistenci iespējams efektīvi pārvarēt ar papildus piesātinošām devām. Clopidogrel is a selective, irreversible P2Y12 receptor antagonist. It has been proven efficacious in acute coronary syndrome as well as in percutaneous coronary intervention (PCI) with and without stenting. Most importantly clopidogrel reduces stent thromboses after drug-eluting stent (DES) placement. There is data on interindividual response to clopidogrel and low response has been associated with worse cardiovascular outcomes. Objective: the aim of this study was to evaluate the prevalence of clopidogrel hyporesponders in patients after PCI with DES placement in Latvia. Methods: Patients were included prospectively after DES placement, questioned and the platelet reactivity index (PRI) was detemined using VASP-P. Results: the prevalence of hyporesponders was high - 73%. Hypertriglyceridaemia (p = 0,001), hyperglycaemia (p = 0,002), high WBC count (p < 0,001), low HDL (p = 0,017), long term smoking (p = 0,012), long term hypertension (p = 0,009) and low ejection fraction (p = 0,012) are risk factors for clopidogrel resistance. Clopidogrel resistance can be effectively overcome by additional loading doses.
