Krūts vēža molekulāro surogātapakštipu klīniski morfoloģiskais raksturojums sievietēm Latvijā un to saistība ar ķirurģiskās ārstēšanas veida izvēli un rezultātiem

Šī uz Latvijas Onkoloģijas centra klīniskās bāzes veiktā retrospektīvā pētījuma mērķis bija analizēt krūts vēža molekulāro surogātapakštipu klīniski patoloģisko raksturojumu un to korelāciju ar slimības iznākumu atkarībā no ķirurģiskās ārstēšanas veida. Pētījumā iekļauti dati par 588 operētām krūts vēža pacientēm,
kuras tika grupētas atbilstoši krūts vēža molekulārajiem surogātapakštipiem
saskaņā ar 2011. un 2013. gada SentGallenas Agrīna krūts vēža konsensusa konferences
(St.Gallen Consensus Conference on Early Breast Cancer) kritērijiem.
Tika analizēti un savstarpēji salīdzināti visu krūts vēža molekulāro surogātapakštipu
klīniski patoloģiskie parametri, kā arī ārstēšanas rezultāti (kopējā
dzīvildze un no slimības brīvā dzīvildze) katra apakštipa gadījumā atkarībā no
operācijas veida.
Pētījuma rezultāti apstiprināja izvirzīto hipotēzi, ka ārstēšanas rezultāti ir atkarīgi
no krūts vēža molekulārā surogātapakštipa, bet nav atkarīgi no operācijas
apjoma. Tādēļ ir izdarīts secinājums, ka piederība noteiktam molekulāram surogātapakštipam
nevar būt vienīgais noteicošais faktors operācijas apjoma izvēlē. The aim of this retrospective study is to analyze the clinical pathological
characteristics of breast cancer surrogate molecular subtypes and their
correlation with the clinical outcome depending on type of surgical treatment.
The study was carried out at the Latvian Oncology Centre and enrolled
588 surgically treated breast cancer patients. Patients were grouped into
surrogate molecular subtypes in accordance with the 2011 and 2013 St. Gallen
Consensus Conference on Early Breast Cancer criteria.
The clinicopathological parameters within each surrogate molecular subtype
as well as the treatment outcomes (overall survival and disease-free survival)
for each surrogate molecular subtype depending on the type of surgery were
analyzed and evaluated in comparison with each other.
The results of the study confirmed the hypothesis that treatment outcomes
depend on breast cancer surrogate molecular subtype but are not contingent on
the type of surgery. It is therefore concluded that tumor belonging to particular
surrogate molecular subtype itself should not affect the choice of the type of surgery.
