Korelācija starp aknu steatozi un lipīdu frakcijām asins analīzēs

Ievads. Tauku hepatoze ir atgriezenisks process un relatīvi bieža atradne ar riska faktoriem kā aptaukošanās, dislipidēmija, cukura diabēts, glikozes tolerances traucējumi. Viena no visnozīmīgākajām analīzēm riska faktoru izvērtēšanai ir lipīdu frakcijas serumā. Aknu steatoze var izzust bez sekām vai progresēt par aknu fibrozi, aknu cirozi un hepatocelulāro karcinomu. Darba mērķis. Izpētīt aknu steatozes un lipīdu frakciju iespējamu korelāciju. Materiāli un metodes. Tika veikts retrospektīvs pētījums, kura ietvaros atlasīja pacientus ar radioloģiski apstiprinātu aknu steatozes diagnozi stacionēšanas laikā PSKUS laika periodā no 01.01.2014.-31.12.2015. Par aknu blīvumu pie tauku hepatozes tika spriests, vadoties pēc Haunsfilda vienībām datortomogrāfijā, savukārt par lipīdu frakciju līmeņiem – pēc asins analīzēm slimības vēsturēs. Rezultāti. Pētījumā iekļauti 136 aknu steatozes pacienti, kuriem diagnoze uzstādīta ar datortomogrāfiju (N = 65; 48%) vai ultrasonogrāfiju (N = 71; 52%). Pētījumu grupa sastāvēja no 43 (31,6%) sievietēm un 93 (68,4%) vīriešiem, vecumā no 29 līdz 88 gadi. Atlasīto pacientu anamnēzēs dokumentētie riska faktori: adipozitāte (N = 17), dislipidēmija (N = 29), glikozes tolerances traucējumi (N = 9), cukura diabēts (N = 24). Nebija dati par aknu fibrozi un hepatocelulāro karcinomu anamnēzes, toties fiksēta aknu ciroze (N = 5; 3,7%). Iegūtie korelācijas dati: kopējais holesterīns (r = 0,061; p = 0,727; rS = 0,058; p =0,742), TG (r = -0,120; p =0,504; rS = -0,099; p =0,585), ABL (r = 0,093; p =0,612; rS = 0,059; p =0,750), ZBL (r = 0,130; p =0,471; rS = 0,062; p =0,732). Secinājumi. Pētījumā vīriešu grupai aknu steatozes diagnoze tika konstatēta divas reizes biežāk nekā sieviešu grupai. Atlasītājās pacientu anamnēzes datos dokumentētie riska faktori atbilst literatūras aprakstā minētajiem. Korelācija pie visām četrām lipīdu frakcijām vērtējama kā ļoti vāja bez statistiskas ticamības. Introduction. Fatty hepatosis is a reversible process and a relatively frequent finding with risk factors such as obesity, dyslipidemia, diabetes, glucose tolerance disorder. One of the most significant tests for analyzing risk factors are serum lipid fractions. Hepatic steatosis may disappear without consequences or progress of liver fibrosis, cirrhosis and hepatocellular carcinoma. Aim of the study. Explore the possible correlation between liver steatosis and lipid fractions. Materials and methods. A retrospective study was carried out and consisted of selected patients with radiologically confirmed diagnosis of hepatic steatosis during PSCUH hospitalization in the time period from 01.01.2014.-31.12.2015. Liver density of fatty hepatosis was determined according to Hounsfield units in computed tomography while lipid fraction levels – after blood tests in patient historys. Results. The study included 136 patients with liver steatosis whose diagnosis was made with computed tomography (N = 65; 48%) or ultrasonography (N = 71; 52%). The study group consisted of 43 (31,6%) females and 93 (68,4%) men in the age group from 29 to 88 years old. From patient historys the documented risk factors were: obesity (N = 17), dyslipidemia (N = 29), glucose tolerance disorder (N = 9), diabetes (N = 24). There was no data about liver fibrosis and hepatocellular carcinoma but liver cirrhosis was noted (N = 5; 3,7%). The obtained correlation data: total cholesterol (r = 0,061; p = 0,727; rS = 0,058; p = 0,742), TG (r = -0,120; p = 0,504; rS = -0,099; p = 0,585), HDL (r = 0,093; p = 0,612; rS = 0,059; p = 0,750), LDL (r = 0,130; p = 0,471; rS = 0,062; p = 0,732). Conclusions. The study showed that in the men group liver steatosis was diagnosed two times more often than in the female group. Selected patient history data corresponds to the documented risk factors mentioned in literature. Correlation of all four lipid fractions were assessed as very weak without statistic credibility.
