Aneiploīdija, dzimumhromosomu izmaiņas un mejotisko gēnu ekspresija vīriešu ļaundabīgajos audzējos: Iespējamā mejotiskā izcelsme

Novērots, ka vairākos vēža tipos DNS sadalījums para-triploīdajā diapazonā (ap 69 hromosomām) ir bieži saistīts ar lielāku ļaundabīgumu, paaugstinātu rezistenci pret ķīmijterapiju un nelabvēlīgu prognozi pacientam. Šobrīd vadošā teorija uzskata, ka triploīdās vēža šūnas veidojas no tetraploīdajām nejaušo anomālo mitožu sērijas rezultātā. Tomēr jaunākie novērojumi liecina, ka triploīdijas rašanās var nebūt pilnīgi nejauša un ka tā varētu būt iesaistīta vēža attīstības svarīgākajās stadijās. Sakarā ar jaunākajiem novērojumiem par to, kādu lomu spēlē vēža cilmes šūnas audzēju pašatjaunošanās procesā, zinātnieki sāka atkārtoti pievērsties 19. gadsimta teorijai, kura uzskata, ka vēža šūnu evolūcijā ir iesaistītas embrionālas īpašības. No tās attīstījās partenoģenētiskā vēža teorija, saskaņā ar kuru vēža attīstībā ir iesaistīts partenoģenēzei līdzīgs process. Tika izvērsta hipotēze, ka triploīdijas izplatība vēža šūnās varētu norādīt uz to, ka noteiktā vēža šūnas dzīves cikla stadijā notika pilna genoma izmaiņas – mejozei līdzīgā procesā tika izolēts haploīdais hromosomu komplekts. Šis haploīdais komplekts pēc tam saplūda ar diploīdo komplektu, kas noveda pie triploīdijas. Lai mēģinātu pierādīt mejozei un partenoģenēzei līdzīgo mehānismu iesaistīšanos vēža attīstības procesā, nepieciešams mērīt mejotisko gēnu ekspresijas līmeņus vēža paraugos, kā arī pētīt vīriešu vēža šūnām raksturīgās dzimumhromosomu skaita izmaiņas. Visbiežāk sastopamās dzimumhromosomu skaita anomālijas ir Y hromosomas zudums un/vai X hromosomas disomija – hromosomu līdzsvara novirze uz X hromosomas pusi. Šī pētījuma hipotēze pieņem, ka šīs aberācijas ir saistītas ar triploīdiju, kas iesaista pilnus vecāku hromosomu komplektus un rodas mejozei un partenoģenēzei līdzīgo procesu rezultātā. Par šo procesu iesaistīšanos vēža attīstībā varētu liecināt ar mejozi un agrīno embrioģenēzi saistīto gēnu paaugstinātā ekspresija vēža šūnās. Pētījuma gaitā 18 sīkšūnu plaušu vēža paraugos, kas tika iegūti no Paula Stradiņa klīniskās universitātes slimnīcas, tika izmērīta triju mejotisko un ar agrīno embrioģenēzi saistīto gēnu (Oct4 (precīzāk, šī gēna Oct4A transkripta), Stella un Mos) ekspresija, pielietojot qPCR. Kā pozitīvā kontrole tika izmantots olnīcas audu paraugs, un kā negatīvā kontrole – veselo plaušu audu paraugs. Vēža paraugos tika novērota ievērojāmi paaugstinātā mērķa gēnu ekspresija, salīdzinājumā ar normālo plaušu audu paraugu. Tomēr šos rezultātus var tikai pieņemt par priekšpētījumu, jo to ticamību ievērojāmi ietekmē pacientu kohortas mazais izmērs, kā arī Mos, Oct4 un Stella praimeru trūkumi, ko detektēja pielietojot Sangera sekvenēšanas metodi. Pētījuma otrajā daļā ietilpa 1352 vīriešu ļaundabīgo audzēju, kas tika savākti no Mitelmaņa citoģenētiskās datubāzes, analīze. Kopumā tika savākti 87 seminomu kariotipi, 154 teratomu kariotipi, 42 sīkšūnu plaušu vēža kariotipi, 171 resnās zarnas adenokarcinomas, 110 kuņģa adenokarcinomas, 128 pārejas šūnu karcinomas n It has been revealed for some cancers that a DNA distribution in the para-triploid or trisomic range (around 69 chromosomes) is often associated with higher cancer malignancy, increased chemotherapy resistance and poor prognosis for the patient. The currently dominant theory on the origin of triploidy suggests that triploid cancer cells are formed from tetraploid ones in a series of random aberrant mitoses. However, some evidence indicates that triploidy may not originate randomly and may represent a crucial stage of cancer development. In light of recent findings on the role of cancer stem cells in self-renewal of tumors, the 19th century theory which suggests that the evolution of cancer cells involves embryonal traits has been revisited. From it stemmed the parthenogenetic theory of cancer, according to which a parthenogenesis-like process is involved in cancer development. It has been hypothesized that the prevalence of triploidy in cancer cells could possibly indicate that at some point in the cancer cell’s life cycle a haploid chromosome set had been isolated by a process akin to meiosis. The haploid set had then proceeded to fuse with a diploid set, resulting in triploidy. The evidence of meiosis and parthenogenesis-like processes being involved in carcinogenesis could potentially be found by testing cancer samples for potential expression of meiotic genes, as well as studying the characteristic numerical abnormalities of the sex chromosomes detected in male cancer cell genomes. The most frequently reported numerical sex chromosome aberrations in the cancer cells of male patients include the loss of the Y chromosome and/or X chromosome disomy (losing balance in favor of the X chromosome). The hypothesis of this study was that these aberrations are associated with triploidy involving the whole parental chromosome sets, which had originated as a result of a programmed event (meiosis & parthenogenesis), and that evidence of such an event would be manifested as heightened expression of meiotic genes in cancer samples. For the purpose of this study qPCR was performed on 18 small cell lung cancer samples acquired from the Pauls Stradins University Hospital in order to test them for the expression of three meiosis and early embryogenesis-related genes - Oct4 (in particular, its Oct4A transcript), Stella and Mos, with normal lung and ovarian samples used as respectively negative and positive controls. The results have indeed shown a prominent increase in meiotic gene expression in the cancer samples as compared to normal tissue, however, these results should only be considered preliminary due to the small cohort of patients from whom the samples were taken and peculiarities of the structure of Oct4, Stella and Mos primers revealed by Sanger sequencing. The second part of the study consisted of analyzing 1352 male cancer karyotypes that have been collected from the Mitelman Database, of them 87 seminomas, 110 stomach adenocarcinom
