Dzīves kvalitāte pacientiem ar hemodialīzes nieru aizstājējterapiju

Abstract

Maģistra darba tēma - Dzīves kvalitāte pacientiem ar hemodialīzes nieru aizstājējterapiju. Pasaulē palielinās to pacientu skaits, kam konstatē hronisku nieru slimību (HNS) terminālā stadijā. Nieru aizstājējterapija ļauj viņiem saglabāt iespējami labāku dzīves kvalitāti. Dzīves kvalitāte ir sensitīvs koncepts, kurš dod iespēju uz pacienta dzīvi paraudzīties no dažādiem skatupunktiem. Tomēr termināla HNS, kas tiek ārstēta ar hemodialīzes palīdzību, noved pie izmaiņām fizioloģiskajā, psiholoģiskajā un sociālajā sfērā. Darba mērķis ir noskaidrot dzīves kvalitātes rādītājus pacientiem ar hemodialīzes nieru aizstājējterapiju. Pētniecības uzdevumi: analizēt literatūru par dzīves kvalitāti, hemodialīzes nieru aizstājējterapiju un pastāvigiem asinsvadu pieejas veidiem pacientiem ar HNS terminālā stadijā; analīzēt B. Ņūmenas māszinības sistēmas modeļa teoriju un K. Kolkabas labsajūtas teoriju; izstrādāt pētījuma instrumentu – anketu, veikt aptauju pacientu vidū; veikt pētījumā iegūto datu apstrādi un analīzi; izveidot secinājumus un ieteikumus. Izvirzīta hipotēze: dzīves kvalitātes rādītāji hemodialīzes pacientiem ar arteriovenozo fistulu ir augstāki nekā pacientiem, kam ir centrālais venozais katetrs. Pētniecības darba instrumenti: Feransas un Pauersas dzīves kvalitātes aprēķināšanas indeksa dialīzes versijas anketa un autores izstrādāta strukturēta anketa. Pētniecības darba metode: kvantitatīvā, neeksperimentālā, salīdzinošā metode. Pētījuma rezultāti liecina, ka izvirzītā hipotēze ir apgāzta. Dzīves kvalitātes rādītāji atkarībā no hemodialīzes asinsvadu pieejas veida statistiski nozīmīgi neatšķiras.

The subject of the Master’s Paper is Quality of Life of Patients in Hemodialysis Renal Replacement Therapy. The number of patients who have chronic kidney disease (CKD) in the terminal stage is increasing in the whole world.. Renal replacement therapy allows them to maintain the best possible quality of life. The Quality of life is a sensitive concept, which enables the patient to look at life from different points of view. However, terminal CKD which is treated with the help of hemodialysis, leads to changes in physiological, psychological aspects, as well as in a social sphere. The objective of the Paper is to determine the Quality of life of Renal Replacement Therapy patients undergoing hemodialysis. The tasks of the research are to summarise and analyse literature dealing with the Quality of Life, of Hemodialysis Renal Replacement Therapy and Vascular Access; to describe application of Betty Neuman’s Systems Model nursing theory and Katharine Kolcaba’s Comfort Theory in practice; to work out the research instrument, i.e., the questionnaire, and to carry out a poll among patients; to process and analyse the data acquired in research; to make conclusions and suggest recommendations. The worked out hypothesis: the Quality of Life scores of Hemodialysis Patients with an Arteriovenous fistula is higher than Patients with a Central venous catheter. The instruments of the research are the Ferrans’s and Powers’s Quality of Life Index Dialysis Version III questionnaire and the designed questionnaire for the present research. The method of the research work: quantitative, non experimental, comparative method. The results of the research prove that the worked out hypothesis has not been proved.The Quality of life scorers are not depending of the type of hemodialysis vascular access and they are not statistically significantly different.
