E.coli, K.pneumoniae un P. aeruginosa antimikrobiālā rezistence urīnceļu infekcijās

Fons: 2015. gadā PVO ieviesa Globālo mikrobu rezistences uzraudzības sistēmu (GLASS). 2017. gadā to pirmoreiz ieviesa Latvijā. Mērķi: Raksturot pašreizējo situāciju attiecībā uz antimikrobiālo rezistenci, multirezistenci kā arī ESBL (plaša spektra beta laktamāzes) producētājus urīnceļu infekcijām, ko izraisa E. coli, K. pneumoniae un P. aeruginosa un analizēt saistību ar pacienta vecumu, dzimumu un infekcijas izcelsmi. Materiāli un metodes: Kopskaitā tika analizēti 924 urīna paraugi no Paula Stradiņa Klīniskās universitātes slimnīcas, 2016. gadā (731 — E. coli, 147 — K. pneumoniae, 46 — P. aeruginosa). Rezultāti: E. coli rezistences procentuālās daļas: amikacīns: 0,3%, ceftazidīms: 13,8%, ci¬pro¬flok¬sacīns: 29,5%, cefotaksīms: 16,6%, cefepīms: 12,6%, fosfomicīns: 0,5%, gentamicīns: 9,2%, norfloksacīns: 38,6%, carbapenēms 0%. K. pneumoniae rezistences procentuālās daļas:amikacīns: 4,3%, ceftazidīms: 47,2%, ciprofloksacīns: 50,4%, cefotaksīms: 46,6%, ertapenēms: 5,5%, cefepīms: 36,4%, gentamicīns: 40,7%, meropenēms: 0,7%, norfloksacīns: 72,7%. P. aeruginosa rezistences procentuālās daļas: amikacīns: 13,0%, ceftazidīms: 32,6%, ciprofloksacīns: 42,2%, cefepīms: 40,0%, gentamicīns: 30,4%, imipenēms: 26,7%, meropenēms: 35,9%, tobramicīns: 11,1%, piperacilīns-tazobaktāms: 37,0%. Multirezistence (MDR): 30,37% no 731 E. coli infekcijām atbilda MDR kritērijiem. 57,14% no 147 K. pneumoniae infekcijām atbilda MDR kritērijiem. 36,95% no 46 P. aeruginosa infekcijām atbilda MDR kritērijiem, un viens paraugs atbilda ekstensīvas rezistences (XDR) kritērijiem. ESBL producētāji: Pozitīvas bija 19,55% no 624 urīnceļu infekcijām, ko izraisīja E. coli un ko testēja ESBL noteikšanai. Pastāv ievērojama statistiskā saistība starp ESBL klātbūtni un infekcijas izcelsmi (p < 0,008), pacienta dzimumu (p < 0,001) un atšķirību starp vidējo ESBL un ar ESBL nesaistītas infekcijas vecumu (p < 0,001). Pozitīvas bija 50,74% no 134 urīnceļu infekcijām, ko izraisīja K. pneumoniae un ko testēja ESBL noteikšanai. K. pneumoniae gadījumā saistība starp ESBL statusu un infekcijas izcelsmi nebija būtiska (p < 0.052), tāpat arī starp ESBL statusu un dzimumu (P < 0,726) un starp vidējo vecumu atšķirībām (p < 0.060). Secinājums: Ir novērotas augstas procentuālās daļas fluorokvinolona rezistencei E. coli, K. pneumoniae un P. aeruginosa. Spriežot pēc šiem konstatējumiem, fluo¬ro¬kvi¬no¬loni nav piemēroti empīriskai urīnceļu infekciju ārstēšanai, kas savukārt atbilst spēkā esošajām slimnīcas vadlīnijām. Ir novērotas augstas procentuālās daļas ESBL producējošo Enterobacteriaceae kopā ar saistītu augstu trešās paaudzes cefalosporīna rezistenci. Ir konstatēta augsta procentuālā daļa aminoglikozīdu rezistencei K. pneumoniae un P. aeruginosa, bet ne E. coli. Ir konstatēta arī augsta procentuālā daļa P. aeruginosa rezistencei pret piperacilīnu-tazobaktāmu, kas ir daļa no tūlītējās nozokomiālo urīnceļu infekciju ārstēšanas Paula Stradiņa Klīniskajā universitātes slimnīcā. Background: In 2015, the Global Antimicrobial Resistance Surveillance System (GLASS) was introduced by the WHO. In 2017, it was implemented for the first time in Latvia. Objectives: To characterise the current state of antimicrobial resistance, multidrug-resistance and extended-spectrum β-lactamase (ESBL)-pro¬du¬cers in urinary tract infections (UTIs) caused by E. coli, K. pneumoniae and P. aeruginosa and to analyse the association with patient’s age, sex and origin of infection. Material and methods: In total 924 urine samples from the Paul Stradiņš University Hospital from 2016 have been analysed (731 for E. coli, 147 for K. pneumoniae, 46 for P. ae¬ru¬gi¬no¬sa). Results: E. coli proportions of resistance: Amikacin: 0.3%, Ceftazidime: 13.8%, Ci¬pro¬flo¬xa¬cin: 29.5%, Ceftotaxime: 16.7%, Cefepime: 12.6%, Gentamicin: 9.2%, Norfloxacin: 38.6%, carbapenems: 0%. K. pneumoniae proportions of resistance: Amikacin: 4.3%, Ceftazidime: 47.2%, Ci¬pro¬floxacin: 50.4%, Ceftotaxime: 46.6%, Er¬ta¬pe¬nem: 5.5%, Cefepime: 36.4%, Gen¬ta¬mi¬cin: 40.7%, Meropenem: 0.7%, Norfloxacin: 72.7%. P. aeruginosa proportions of resistance: Amikacin: 13.0%, Ceftazidime: 32.6%, Ci¬pro¬flo¬xa¬cin: 42.2%, Cefepime: 40.0%, Gentamicin: 30.4%, Imipenem: 26.7%, Mero¬pe¬nem: 35.9%, To¬bra-mycin: 11.1%, Piperacillin-Tazobactam: 37.0%. Multidrug resistance: 30.37% of E. coli, 57.14% of K. pneumoniae and 36.95% of P. aeruginosa met the criteria for MDR. ESBL-producers: 19.55% of E. coli have been positive. There was a statistical significant association between ESBL-presence and origin of infection (p < 0.008), patient’s sex (p < 0.001) and difference of mean ages of ESBL and non-ESBL-infection (p < 0.001). 50.74% of K. pneumoniae have been positive. For K. pneumoniae, the association between ESBL status and origin of infection was not significant (p < 0.052), neither was the as¬so¬ci¬a¬tion between ESBL status and sex (p < 0.726), nor the association between the difference of mean ages (p < 0.060). Conclusion: High proportions of fluoroquinolone resistance in all three bacteria have been observed. According to these findings, they are not suitable as an empirical treatment of UTIs. High proportions of ESBL-producing Enterobacteriaceae with an associated high third generation cephalosporin resistance have been observed. High proportion of aminoglycoside resistance of K. pneumoniae and P. aeruginosa have been found. A high proportion of P. aeruginosa resistant to pi¬pe¬ra¬cil¬lin-tazobactam, the first line treatment for nosocomial UTIs in the Stradiņš Hospital, was found. Previously reported low carbapenem resistance can be confirmed for Enterobacteriaceae, but not for P. aeruginosa.
