Ortokeratoloģijas efektivitāte tuvredzības stabilizācijā

Diplomdarbs ir uzrakstīts latviešu valodā uz 57 lapām. Tajā ir 18 attēli, 12 tabulas, un 76 atsauces uz literatūras avotiem. Tuvredzība ir biežākais refrakcijas traucējumu veids pasaulē. Viena no efektīvākajām tuvredzības optiskās kontroles metodēm ir ortokeratoloģija (Orto-K)– radzenes refraktīvās terapijas metode, kad, lietojot kontaktlēcas (KL) nakts laikā, tiek likvidēta korekcijas nepieciešamības dienā. Pētījuma mērķis bija izvērtēt Orto-K efektivitāti tuvredzības stabilizācijā, nosakot acs ābola aksiālā garuma (AL) izmaiņas 18 mēnešu laika periodā. Pētījumam tika atlasītas 102 pacientu ambulatoras kartītes. Tika izveidotas divas pacientu grupas, pirmā– 50 bērni (100 acis), kuri tuvredzības korekcijai izmanto Orto-K KL, otrā– 52 bērni (104 acis), kuri izmanto monofokālās briļļu lēcas (MBL). Bērniem, kuri tuvredzības korekcijai izmanto Orto-K KL, tika noteiktas acs ābola AL, refrakcijas, keratometrijas un biomikroskopijas izmaiņas pirms terapijas uzsākšanas, kā arī sešus, 12 un 18 mēnešus pēc tās. Bērniem, kuri izmanto MBL, tika noteiktas acs ābola AL un refrakcijas izmaiņas pirms terapijas uzsākšanas, kā arī sešus un 18 mēnešus pēc tās. Pēc iegūtajiem rezultātiem, Orto-K KL 18 mēnešu ilgā lietošanas periodā acs ābola AL pagarinās vidēji par 0,12 milimetriem (mm), bet MBL lietošanas laikā par 0,65 mm. Tika konstatēts, ka atšķirība starp abām metodēm ir vairāk nekā piecas reizes, un iegūtās atšķirības ir statistiski nozīmīgas (P < 0,05). Darba autore var secināt, ka Orto-K ir efektīva tuvredzības kontroles metode. Šo KL lietošanas laikā acs ābola AL pagarinās piecas reizas lēnāk, nekā lietotojot MBL. Arī refrakcijas rādītāju izmaiņas apstiprināja ievērojami lēnāku tuvredzības progresēšanu Orto-K KL lietotājiem. Orto-K KL lietošanas laikā netiek inducēta astigmātisma veidošanās un keratopātijas attīstība tika konstatēta tikai 9,0 % pacientu. The work is written in latvian language, on 57 pages. It contains 18 images, 12 tables, and 76 references to literature sources. Myopia is one of the most common refractive disorders in the world. Orthokeratology (Ortho-K) is known as one of the most effective optical control methods for myopia. It is a refractive therapy method of the cornea, where contact lenses are worn by night, thus eliminating the need for a correction during the day. Aim of the study was to evaluate the effectiveness of Ortho-K in the stabilization of myopia, by identifying changes of the axial length (AL) of the eyeball over a period of 18 months. Two groups of patients were formed, with the first one comprising 50 children (100 eyes) who use Ortho-K contact lenses (CL) for the correction of myopia, whereas the second consisting of 52 children (104 eyes) who use monofocal eyeglasses lenses (MEL). For those children using Ortho-K CL for the correction of myopia the changes of the AL of the eyeball, refraction, keratomery and also changes of biomicroscopy were recorded before starting the therapy, as well as 6, 12 and 18 months after the therapy. For children using MEL changes of the AL of the eyeball and refraction were recorded before the therapy, as well as 6 and 18 months after using the correction. While using the Ortho-K CL for a period of 18 months, AL of the eyeball has extended by 0,12 mm, whereas while using MEL by 0,65 mm. The study showed that the difference between both of the methods is more than five times, and statistically significant (P < 0,05). The author can conclude that the use of Ortho-K is an effective control method for patients with myopia. While using the Ortho-K CL, the AL of the eyeball extends five times slower than when using MEL. Changes of the refraction indicators confirm a substantially slower progression of myopia for Ortho-K CL users. Astigmatisms did not develop while using Ortho-K CL and keratopathy has been found only among 9,0 % of the patients.
