Lipīdus pazeminošās terapijas efektivitāte Latvijas Ģimenes hiperholesterinēmijas reģistra pacientiem: pēcpārbaudes dati

Ģimenes hiperholesterinēmija (ĢH) ir bieži sastopama, pārmantojama slimība, kas raksturojas ar izteikti palielinātu zema-blīvuma lipoproteīnu holesterīna (ZBLH) līmeni kopš bērnības. Tā izraisa agrīnu koronāru sirds slimību, kuras pirmā izpausme bieži ir miokarda infarkts vai pēkšņa koronāra nāve. Hipolipidemizējoša terapija ir ārstēšanas stūrakmens, bet ņemot vērā izteikti palielinātu ZBLH koncentrāciju, tā mērķa sasniegšana ir apgrūtinoša un nereti neiespējama. Mērķis: Raksturot hipolipidemizējošas terapijas efektivitāti ĢH pacientiem pirms un pēc iekļaušanas Latvijas Ģimenes hiperholesterinēmijas reģistrā (LĢHR). Metodes: Tika analizēta informācija par LĢHR pacientiem, kuri vērsās vai bija nosūtīti reģistrā sakarā ar aizdomām par ĢH. Pacienti ar klīnisku ĢH tika atlasīti ar Nīderlandes Lipīdu Klīniku diagnostisko kritēriju palīdzību, ja tie bija pirmie no savas ģimenes (probandi), vai ar specifiskas ZBLH procentiles palīdzību (ZBLH> 95. procentiles attiecīgajam pacienta vecumam un dzimumam), ja tie bija jau diagnosticēto pacientu radinieki. Rezultāti: Līdz 2019. gada februārim, LĢHR tika iekļauti 583 pacienti, no kuriem 42,7% (n=249) tika uzstādīta klīniska ĢH diagnoze. Pacientu augstākās dokumentētās kopējā holesterīna un ZBLH koncentrācijas bija attiecīgi 9,7±2.7 mmol/l un 7,2±1,9 mmol/l. Iekļaušanas brīdī hipolipidemizējošu terapiju lietoja 55,0% (n=137) pacientu, no kuriem tikai 2,8% (n=7) lietoja maksimālu hipolipidemizējošu terapiju. Pacientiem, kuriem bija vismaz viena atkārtota vizīte pie LĢHR speciālista (n=105), lipīdus pazeminošas terapijas lietošana uzlabojās no 44,8% (n=47) uz 83,8% (n=88) pacientiem, bet maksimālās kombinētās terapijas lietošana uzlabojās no 1,9% (n=2) uz 22,9% (n=24). Terapijas rezultātā, ZBLH koncentrācija pēdējo analīžu brīdī vidēji samazinājās par 37,9% (p<0,001), un ZBLH mērķa sasniegšana uzlabojās no 5,7% (n=6) iekļaušanas brīdī līdz 15,2% (n=16) pēdējo analīžu laikā. Vadlīnijās rekomendētajām Proproteīnu konvertāzes subtilizīns/kehsīns 9 tipa inhibitoru (PCSK9i) lietošanas indikācijām atbilda 11 (10,5%), bet teorētiski aprēķinot maksimālās kombinētās terapijas hipolipidemizējošu efektu pacientiem, kuri tādu vēl nesaņem, vismaz 23 (21,9%) pacientiem būtu prognozējamas indikācijas PCSK9i lietošanai. Familial hypercholesterolemia (FH) is common hereditary disorder, which causes extremely elevated low-density lipoprotein cholesterol (LDL-C) levels throughout the patient life. As the result, coronary heart disease (CHD) develops early in life, and frequently the first manifestations are myocardial infarction or sudden cardiac death. Effective lipid-lowering therapy (LLT) is a cornerstone of management of this patient group, however, due to baseline high LDL-C levels, LDL-C goal attainment is challenging or even impossible. Aim of the study: To analyse the effectiveness of LLT in FH patients before and after inclusion into Latvian Registry of Familial hypercholesterolemia (LRFH). Methods: The data of FH patients included into LRFH was analysed. Clinical FH cases were identified with Dutch Lipid Clinic Network criteria if they were first from their family (probands) or by diagnostic LDL-C percentile (LDL-C above 95th percentile of corresponding patient’s age and gender group), if patients were first-degree relatives of their probands. Results: Five hundred eighty-three patients were included in LRFH by February 2019. Clinical FH diagnosis was made in 42.7% (n=249) cases. Highest documented total cholesterol and LDL-C levels were 9.7±2.7 mmol/l and 7.2±1.9 mmol/l, respectively. At the inclusion, 55.0% (n=137) patients were on any LLT, 2.8% (n=7) were on maximal LLT. In FH patients, who had attended LRFH at least twice (n=105), use of LLT had improved from 44.8% (n=47) at inclusion to 83.8% (n=88) at follow-up, while maximal LLT usage improved from 1.9% (n=2) to 22.9% (n=24). As the result, the mean LDL-C decrease by 37,9% was observed (p<0,001). LDL-C goal attainment improved from 5,7% (n=6) at inclusion to 15,2% (n=16) at follow-up. Eleven (10,5%) have met indications for Proprotein Convertase Subtilisin/Kexin Type 9 (PCSK9) inhibition. After calculating a theoretical effect of switching all patients to maximal LLT, 23 (21,9%) patients were predicted to meet indications for PCSK9 inhibition.
