Tiopurīna metiltransferāzes genotipa un fenotipa saistība ar tiopurīnu blakusparādībām pacientiem ar iekaisīgām zarnu slimībām

Ievads. Tiopurīna metiltransferāze (TPMT) ir enzīms, kurš piedalās tiopurīnu metabolismā. Skaitās, ka TPMT genotipa un fenotipa noteikšana ļauj atklāt pacientus, kuri ir pakļauti mielosupresijas riskam tiopurīnu terapijas gadījumā. Latvijā TPMT testēšana ir jauna metode. Neskatoties uz vairākiem pētījumiem par TPMT testēšanas efektivitāti, joprojām pastāv bažas par TPMT testēšanas nepieciešamību. Darba mērķis. Noteikt TPMT genotipa un fenotipa saistību ar tiopurīnu blakusparādībām pacientiem ar iekaisīgām zarnu slimībām (IZS). Materiāli un metodes. Prospektīvā pētījumā iekļauti 59 iekaisīgo zarnu slimību pacienti vecumā no 18 līdz 77 gadiem, kuri ārstējās Paula Stradiņa Klīniskās universitātes slimnīcas 10. nodaļā laika posmā no 2018. gada februāra līdz 2019. gada martam. Pacientu DNS paraugi tika pārbaudīti uz TPMT*2, *3A, *3B un *3C polimorfismu klātbūtni ar reāla laika PĶR metodi. Papildus, pacientiem tika noskaidrots TPMT fenotipa testēšanas rezultāts. Papildus datu savākšanai pielietota anketa. Datu apkopošanai un statistiskajai apstrādei tika izmantotas programmas Microsoft Office Excel 2010 un IBM SPSS Statistics 22. Rezultāti. 94,9% (n=56) pacientu ir savvaļas tipa genotips, 5,1% (n=3) heterozigotisks TPMT genotips: 3,4% (n=2) tika atrasts TPMT*3A polimorfisms un 1,7% (n=1) TPMT*2 polimorfisms. TPMT aktivitāte bija zināma 14 pacientiem: 86% (n=12) ir normāla TPMT aktivitāte, 7% (n=1) ir zema TPMT aktivitāte un 7% (n=1) ir paaugstināta TPMT aktivitāte. Saistība starp TPMT fenotipu un genotipu nav statistiski ticama (p>0,05). Kopumā 46% pacientu (n=27) bija ārstēšanas pieredze ar tiopurīniem, no tiem 30% (n=8) ir saskarušies ar tiopurīnu blakusparādībām: leikopēniju (11%, n=3), zilumu veidošanos (4%, n=1), akūtu pankreatītu (4%, n=1), mialģijām (4%, n=1) un vemšanu (4%, n=1). Starp TPMT genotipu un azatioprīna izraisīto mielosupresiju tika konstatēta statistiski ticama saistība (p=0,018). Saistība starp TPMT fenotipu un azatioprīna izraisīto mielosupresiju nav statistiski ticama (p>0,05). Saistību starp TPMT fenotipu un pārējām blakusparādībām nav iespējams izrēķināt mazā pacientu skaita dēļ. Secinājumi. Tiopurīni tiek plaši pielietoti IZS ārstēšanā, tomēr 30% pacientu saskaras ar tiopurīnu blakusparādībām, visbiežākā no tām ir leikopēnija (30%). 5% pacientu ir atrasti TPMT polimorfismi, pastāv statistiski ticama saistība starp TPMT gēna polimorfismiem un tiopurīnu izraisītu mielosupresiju. 7% ir zema TPMT aktivitāte, šai pacientu grupai nav statistiski ticama saistība ar tiopurīnu blakusparādībām. Saistība starp TPMT genotipu un fenotipu nav statistiski ticama. Introduction. Thiopurine methyltransferase (TPMT) is an enzyme, which takes part in thiopurine drug metabolism. TPMT phenotype and genotype testing can evaluate patients, who are at risk of myelosupression if they receive thiopurines. TPMT testing is a new method in Latvia. Despite several studies on TPMT testing efficiency, there are still concerns about the need of TPMT testing. The aim. To determine the association between TPMT genotype and phenotype with thiopurine side effects in patients with inflammatory bowel disease (IBD). Materials and methods. The prospective study included 59 patients with inflammatory bowel disease aged 18-77 who were treated in Pauls Stradins Clinical University Hospital, from February 2018 to March 2019. Patient DNA samples were tested for TPMT *2, *3A, *3B, and *3C polymorphisms using a qPCR method. In addition, patients were asked for the TPMT phenotype test result. A questionnaire was used for the extra data collection. Microsoft Office Excel 2010 and IBM SPSS Statistics 22 were used for data collection and analysis. Results. 94.9% (n=56) of patients had wild-type genotype, 5.1% (n = 3) were heterozygous for TPMT polymorphisms: 3.4% (n=2) had TPMT *3A polymorphism and 1.7% (n=1) had TPMT *2 polymorphism. TPMT activity was known in 14 patients: 86% (n=12) had normal, 7% (n=1) low and 7% (n=1) high TPMT activity. The relationship between the TPMT phenotype and genotype is not statistically significant (p>0.05). In total, 46% of patients (n=27) had ever been treated with thiopurines, of which 30% (n=8) were exposed to thiopurine adverse events: leukopenia (11%, n=3), bruising (4%, n=1), acute pancreatitis (4%, n=1), myalgia (4%, n=1) and vomiting (4%, n=1). A statistically significant relationship was found between TPMT genotype and azathioprine-induced myelosuppression (p=0.018). The relationship between TPMT phenotype and azathioprine-induced myelosuppression is not statistically significant (p>0.05). The relationship between the TPMT phenotype and the other side effects cannot be calculated. Conclusion. Thiopurines are widely used in the treatment of IBD, but 30% of patients are faced with thiopurines side effects, the most common being leukopenia (30%). TPMT polymorphisms have been found in 5% of patients. There is a statistically significant relationship between TPMT gene polymorphisms and thiopurine-induced myelosuppression. 7% have low TPMT activity, and there is no statistically significant association with thiopurine side effects in this patient group. The relationship between the TPMT genotype and the phenotype is not statistically significant.
