Deguna un plaušu ventilācijas funkciju salīdzinājums pacientiem pirms deguna ķirurģijas

Ir pierādīta augšējo un dziļo elpceļu vienotība alerģisku slimību kontekstā, taču apgrūtinātas deguna elpošanas ietekme uz elpceļiem ārpus tā joprojām nav pilnībā izpētīta, un ir nepieciešama dziļāka izpratne par elpceļu daļu mijiedarbības rezultātiem. Darba mērķis ir salīdzināt deguna un plaušu funkcionālos rādītājus pacientiem ar apgrūtinātu deguna elpošanu, kam plānota deguna ķirurģija, kā arī salīdzināt spirometrijas rādītājus apgrūtinātas un brīvas deguna elpošanas grupās. Materiāli un metodes: Tika veikts gadījuma-kontroles pētījums klīnikā „Headline”, izmantojot četru fāžu rinomanometriju (4PR) pētījuma grupai (24 pacienti ar deguna obstrukciju, kam plānota deguna ķirurģija), un spirometriju pētījuma un kontroles grupās (9 pacienti bez deguna obstrukcijas). Rādītāji tika statistiski analizēti un salīdzināti. Rezultāti: Vidējā kopējā logaritmiskā efektīvā rezistence (LogReff) pēc deguna gļotādas anemizācijas statistiski ticami samazinājās: labajai nāsij no 1,04 ± 0,38 uz 0,84 ± 0,35 (PPT = 3,24 x10-4), kreisajai nāsij no 1,06 ± 0,34 uz 0,8 ± 0,29 (PV = 4,42 x10-4) . Statistiski ticami pēc anemizācijas samazinājās arī vidējā rezistences klase: labajai nāsij no 2,78 ± 1,44 uz 2,04 ± 1,33 (PV = 2,51 x10-3), kreisajai nāsij no 2,78 ± 1,38 uz 1,87 ± 1,18 (PV = 3,96 x10-3). Visi vidējie spirometrijas rādītāji un to Z-score pētījuma un kontroles grupās bija norma: FEV1 (%) pētījuma grupā 107,5 ± 8,43 (Z-score 0,70 ± 0,74); FEV1 (%) kontroles grupā 103,67 ± 11,6 (Z-score 0,27 ± 0,85); FVC (%) pētījuma grupā 111,04 ± 8,87, (Z-score 1,02 ± 0,82), FVC (%) kontroles grupā 108,78 ± 10,71 (Z-score 0,67 ± 0,77); FEV1/FVC (%) pētījuma grupā 81,42 ± 5,55 (Z-score -0,02 ± 0,79), FEV1/FVC (%) kontroles grupā 80,11 ± 5,82, (Z-score -0,11 ± 0,68); PEF (%) pētījuma grupā 98,83 ± 10,66 (Z-score -0,1 ± 0,86), PEF (%) kontroles grupā 99,33 ± 13,47 (Z-score -0,03 ± 1,04). Spirometrijas rādītāji un to Z-score pētījuma un kontroles grupās nebija statistiski ticami atšķirīgi (PT > 0,05). Salīdzinot visus 4PR un spirometrijas rādītājus, tika iegūtas divas statistiski ticamas, vājas (ρ = |0,31|) korelācijas: starp LogReff pirms anemizācijas un FVC (%) (apgriezta korelācija); starp LogReff pirms anemizācijas un PEF (%) (tieša korelācija). Secinājumi: Pētījuma grupā deguna elpošanas rezistence pēc anemizācijas samazinājās statistiki ticami. Visu pētījuma dalībnieku plaušu funkcija bija normāla, netika atrasta statistiski ticama atšķirība starp spirometrijas rādītājiem grupās. Tika atrastas divas korelācijas starp 4PR un spirometrijas rādītājiem, taču tās ir vājas. Hipotēze, ka apgrūtināta deguna elpošana korelē ar plaušu funkciju pasliktināšanos, neapstiprinājās, un nepieciešami apjomīgāki pētījumi ticamu rezultātu iegūšanai. A relationship between upper and lower airways has been confirmed in the context of allergic diseases, but it has not yet been fully researched in non-allergic cases. A deeper understanding of how both airway parts affect each other in these cases is required. The aim of this study is to compare nose and lung function in patients with difficult nasal breathing before nasal surgery, and to compare spirometry values between patient groups with difficult and normal nasal breathing. Materials and methods: A case-control study was done in „Headline” clinic, using four-phase rhinomanometry (4PR) for case group (24 patients with nasal obstruction before nasal surgery), and spirometry for both case and control (9 patients without nasal obstruction) groups. Results were statistically compared and analyzed. Results: Average total logarithmic effective resistance (LogReff) after nasal decongestion decreased significantly: right nostril 1,04 ± 0,38 to 0,84 ± 0,35 (PPT = 3,24 x10-4), left nostril 1,06 ± 0,34 to 0,8 ± 0,29 (PV = 4,42 x10-4). Average resistance class also decreased with statistical significance: right nostril 2,78 ± 1,44 to 2,04 ± 1,33 (PV = 2,51 x10-3), left nostril 2,78 ± 1,38 to 1,87 ± 1,18 (PV = 3,96 x10-3). All average spirometry values and their Z-scores were within normal range in both groups: FEV1 (%) in case group 107,5 ± 8,43 (Z-score 0,70 ± 0,74); FEV1 (%) in control group 103,67 ± 11,6 (Z-score 0,27 ± 0,85); FVC (%) in case group 111,04 ± 8,87, (Z-score 1,02 ± 0,82), FVC (%) in control group 108,78 ± 10,71 (Z-score 0,67 ± 0,77); FEV1/FVC (%) in case group 81,42 ± 5,55 (Z-score -0,02 ± 0,79), FEV1/FVC (%) in control group 80,11 ± 5,82, (Z-score -0,11 ± 0,68); PEF (%) in case group 98,83 ± 10,66 (Z-score -0,1 ± 0,86), PEF (%) in control group 99,33 ± 13,47 (Z-score -0,03 ± 1,04). Spirometry value and Z-score differences between groups were not statistically significant (PT > 0,05). Two statistically significant, weak (ρ = |0,31|) correlations were found: LogReff before decongestion and FVC (%) (inverse correlation); LogReff before decongestion and PEF (%) (direct correlation). Conclusions: Nasal resistance in control group decreased with statistical significance after decongestion. Lung function of all participants was normal, no significant differences in spirometry values were found between groups. Two weak correlations were found between 4PR and spirometry values. The hypothesis that difficult nasal breathing relates to reduced lung function was not confirmed, and more extensive research is needed to achieve reliable results.
