Meteoroloģisko apstākļu ietekmes izpēte uz migrēnu

Ievads. Daudzi migrēnas pacienti apgalvo, ka dažas laika apstākļu izmaiņas provocē viņiem galvassāpes. Tomēr pētījumu dati ir pretrunīgi. Darba mērķis. Novērtēt potenciālo korelāciju starp meteoroloģiskiem parametriem un galvassāpju intensitāti, biežumu. Novērtēt dalībnieku dzīves kvalitāti (DzK). Materiāli un metodes. Tika veikts prospektīvs pētījums, kas ietvēra 100 pacientus ar migrēnas diagnozi (pētāmā grupa, PG) un 100 pacientus bez migrēnas (kontrolgrupa, KG), statistiskā analīze attiecīgi tika veikta 82 un 80 dalībniekiem. Par svarīgu statistisko nozīmību tika uzskatīts, ja statistiskā ticamība bija p < 0,05. Statistiskie dati tika apstrādāti ar Microsoft Excel Office 365 un IBM SPSS Statistics Base Grad Pack 22.0 programmu. Pētījums tika veikts SIA “Veselības centrs 4” galvassāpju kabinetā. Analīzei tika izmantotas pacientu ambulatorās kartiņas, anketas un dienasgrāmatas. Tika noteikta sāpju intensitātes (VAS) un biežuma korelācija ar gaisa temperatūru, mitrumu, atmosfēras spiedienu un saules spīdēšanas ilgumu 12 stundu intervālā 2019. novembrī. Rezultāti. Vidējais PG vecums (M = 42,3; SD = 13,28) un KG (M = 44,8; SD = 13,32) statistiski ticami neatšķīrās (p > 0,05). 53,6% PG pacientu galvassāpes bija biežāk nekā 4 reizes mēnesī (biežas migrēnas lēkmes), 36% (n = 16) no tiem lietoja profilaktisku terapiju, visbiežāk tricikliskus antidepresantus (n = 8; 50%). Galvassāpju biežumam un stiprumam bija vāja vai vidēja korelācija ar dažādiem laika apstākļiem. PG dalībniekiem biežākas galvassāpes korelēja ar ievērojamu atmosfēras spiediena pazemināšanos rīta stundās (r = 0,35; p < 0,05) un ar lielāku gaisa mitrumu vakaros (r = 0,32; p < 0,05; φ = 90 - 97%). Tika novērota tendence galvassāpju stiprumam palielināties saulainās dienās (Latvijā 2019. gada novembrī bija 5 saulainas dienas, apmēram 4 saulainas stundas dienā). Galvassāpju stiprums vienādā mērā korelēja arī ar citām laika apstākļu izmaiņām: gan gaisa mitruma, gan atmosfēras spiediena, gan gaisa temperatūras pieaugumu un samazinājumu. KG citas lokalizācijas sāpes bija biežākas dienās ar lielāku gaisa mitrumu (r = 0,49; p < 0,05) un siltākās dienās (r = 0,42; p < 0,05; t = +11º), un tās bija stiprākas pie zemāka atmosfēras spiediena. KG dalībniekiem statistiski biežāk bija hroniskas sāpes balstu-kustību aparātā nekā PG dalībniekiem (p < 0,05). Nelielu PG (8,5%; n = 7) un KG (5%; n = 4) dalībnieku procentu varēja uzskatīt par meteojutīgiem. PG meteojutīgie dalībnieki biežāk atzīmēja stipras galvassāpes pie gaisa mitruma un temperatūras izmaiņām, savukārt, KG meteojutīgie dalībnieki bija jutīgi uz gaisa temperatūru. Mēneša laikā PG daudz biežāk atzīmēja savu DzK kā ļoti sliktu salīdzinājumā ar KG (p < 0,05). Secinājumi. Nav acīmredzamas saistības starp galvassāpju biežumu, intensitāti un meteoroloģiskiem apstākļiem. Mazs meteojutīgo dalībnieku procents tika noteikts abās grupās. Migrēna negatīvi ietekmē DzK. Introduction. Many migraine patients claim that some weather changes trigger their headaches. However, research data is contradictory. Methods. A prospective study was carried out, involving 100 patients with migraine (study group, SG) and 100 people without migraine (control group, CG), but statistical analysis was performed for 82 and 80 respectively. The level of statistical significance was expressed as p-value<0,05. The study was conducted in the Health centre 4 headache consulting office. For analysis outpatient record data, headache diary and questionnaire were used. The intensity (VAS) and frequency of pain were correlated with temperature, humidity, atmospheric pressure, the number of sun-hours within an interval of 12 hours over November. Aim. Investigate possible correlation between parameters of weather conditions and frequency as well as intensity of headaches. Evaluate the quality of life (QoL) between SG and CG. Results. The mean age of SG (M = 42,3; SD = 13,28) and CG (M = 44,8; SD = 13,32) did not differ statistically significantly (p> 0,05). 53,6% of SG patients had headache more than 4 times a month (frequent migraine attacks), 36% (n = 16) of whom received prophylactic therapy, most commonly tricyclic antidepressants (n = 8; 50%). The frequency and intensity of headaches showed a weak or moderate correlation with various weather conditions. In SG there were more headache in the morning with lower atmospheric pressure (r=0,35; p<0,05) and in the evening with higher humidity (r=0,32; p<0,05). There was a tendency for headache to be worse on sunny days (in November 2019 there were 5 sunny days in Latvia; 4 sunny hours a day). The severity of headaches was equally correlated with other changes in weather conditions: increases and decreases in humidity, atmospheric pressure and air temperature. In CG pain of other localization occurred more often on humid (r=0,49; p<0,05; φ=90-97%) or warmer days (r=0,42; p<0,05; t=+11º), and it was more intense with lower atmospheric pressure. CG participants were statistically more likely to have chronic musculoskeletal pain than SG participants (p <0,05). Small percentage of patients in SG (8,5%; n=7) and in CG (5%; n=4) with an average correlation to the weather conditions was revealed, which can be considered as meteosensitive. SG meteosensitive participants were more likely to report severe headaches at changes in humidity and temperature, while CG meteosensitive participants were mostly sensitive to air temperature. During a month, SG more often marked their QoL as very poor compared to CG (p = 0,04). Conclusion. There was no obvious relationship between any weather condition and intensity or frequency of headache in SG. There was small percentage of meteosensitive patients in both groups. Migraine attacks had negative impact on the QoL.
