Autoantivielu profils asins serumā pacientiem ar Sistēmisko sarkano vilkēdi, kas ārstējušīes Paula Stradiņa klīniskās universitātes slimnīcas Reimatoloģijas centrā

Abstract

Sistemiskā sarkanā vilkēde ir hroniska autoimūna slimība, kas pieder pie retām reimatiskām slimībām. Tā raksturojas ar autoantivielu un to veidoto imūnkompleksu izraisītu audu bojājumu. Sistēmiskai sarkanai vilkēdei ir daudzpusīgas izpausmes ar daudzu orgānu iesaisti un tieši tādēļ, tā bieži paliek nediagnosticēta, kas kavē pacienta pareizās ārstēšanas terapijas uzsākšanu. Līdz ar to īpaša uzmanība ir jāpievērš imunoloģisko rādītāju izmaiņām, lai savlaicīgi varētu diagnosticēt šo slimību. Pētījuma mērķis. Retrospektīvi izpētīt un noskaidrot imunoloģisko rādītāju profīlu izmaiņas pacientiem ar Sistēmisko sarkano vilkēdi. Materiāli un metodes. Šis retrospektīvais pētījums tika veikts Paula Stradiņa Klīniskās universitātes slimnīcā, kur analizējamie dati (imunoloģiskie rādītāji, asins aina, asins bioķīmija, ārstēšanas terapija) iegūti no pacienta slimības vēsturēm, kas atradās Reimatoloģijas centra arhīvā. Kopumā tika analizētas 55 pacientu vēstures. Iegūto datu apstrādei tika izmantota IBM SPSS 21.0 (statistikas programma). Rezultāti. Pētījumā tika iekļauti 55 pacienti, no kuriem 48 bija sievietes (87%) un 7 vīrieši (13%). Pacientu vidējais vecums bija 49.1 gadi. Vairumam pacientu (30) ANA skrīnings HEp-2 šūnās uzrādīja - ANA homogēnu (55%). ANA pozitīvs titrs bija 82% pacientu (45). Anti-dsDNS bija pozitīvas 64% pacientu (35) un vidējā vērtība bija 166.15 IU/ml. C3 vidējā vērtība bija 0.81 g/L, kas bija samazināta 65% pacientu (36). Anti-Sm bija pozitīvas 35% pacientu (15). Lielāko daļu no asins ainas 22% (12) veido anēmija ar limfopēniju. Ārstēšana uzrādīja, ka lielāko daļu 29% (16) sastāda kombinētā terapija. Statistiski nozīmīgas atšķirības (p<0.05) starp vecuma grupām un laboratoriskiem rādītājiem tika novērotas - ANA (0.046), anti-dsDNS (0.024), C3 (0.025), C4 (0.002) un kreatinīns (0.041). Secinājumi. Imunoloģisko rādītāju profils Latvijā un citos pasaules reģionos būtiski neatšķiras. Jaunākiem pacientiem (līdz 45 gadiem) imunoloģisko rādītāju atšķirības ir izteiktākas salīdzinot ar pacientiem virs 45 gadu vecuma. Tika apstiprināta hipotēze, ka dažādos ģeogrāfiskos reģionos (Skandināvijas valstis, Polija, Indija, Saūda Arābija, Alžīrija, Eiropa) atbilstoši literatūras datiem imunoloģiskie rādītāji pie SSV tomēr ir dažādi, bet atbilst diagnostisko kritēriju ietvariem. Systemic lupus erythematosus is a chronical autoimune disease that belongs to the rare rheumatic diseases. It is characterized by autoantibody and its formed immune complex caused material damage. Systemic lupus erythematosus has multiple manifestations with the involvement of many organs and that is a reason why it often remains undiagnosed, that prevents initiating appropriate treatment for the patient. Therefore, special attention should be paid to changes in immunological parameters for timely diagnosis of this disease. The aim of the research. To research retrospectively and to clarify changes in the profile of immunological parameters among patients with systemic lupus erythematosus. Materials and methods. This retrospective research was performed at Pauls Stradins Clinical University Hospital, where the analyzed data (immunological parameters, blood scene, blood biochemistry, treatment therapy) were obtained from the patient's medical histories in the archives of the Rheumatology Center. Overall 55 patient histories were analyzed. IBM SPSS 21.0 (statistical program) was used to process the obtained data. Results. 55 patients were involved in the study 48 of whom were women (87%) and 7 men (13%). The average age of the patients was 49.1 years. For most patients (30) ANA screening in HEp-2 cells showed - ANA homogeneous (55%). The ANA-positive titre had 82% of patients (45). Anti-dsDNA was positive in 64% of patients (35) with an average value of 166.15 IU / ml. The average value of C3 was 0.81 g / L, which was reduced in 65% of patients (36). Anti-Sm was positive in 35% of patients (15). Most of the blood scene 22% (12) formed anemia with lymphopenia. The treatment showed that the majority 29% (16) consisted of combination therapy. Statistically significant differences (p <0.05) between age groups and laboratory parameters were observed - ANA (0.046), anti-dsDNS (0.024), C3 (0.025), C4 (0.002) un creatinine (0.041). Conclusions. The profile of immunological indicators in Latvia and other regions of the world do not differ significantly. For younger patients (up to 45 years) differences in immunological parameters are more pronounced compared to patients over 45 years of age. The hypothesis was confirmed that in different geographical regions (Scandinavia, Poland, India, Saudi Arabia, Algeria, Europe), according to the literature, the immunological parameters at SLE are still different, but within the framework of diagnostic criteria.
