Background: Tonsillectomies are one of the most frequently performed surgical operations in human history. Tonsillectomy technologies and procedures have evolved throughout the years with the intention of improving patient care in relation to intraoperative and postoperative hazards. ent. Aim: The aim of this study is to compare the BiZact tonsillectomy technique with the conventional Cold steel tonsillectomy regarding surgical outcomes. Objective: The "classic" cold steel tonsillectomy technique is one of the most often used technique and has relied upon tonsillectomy instruments because of its ease of handling, lack of electricity (and hence simplicity), low cost, and reusability via sterilizing. However, primary and secondary hemorrhage is the most dreaded and serious consequences for individuals undergoing tonsillectomy. Additionally, pain management, patient perceptions, resumption of regular nutrition, and antibiotic use must all be addressed when selecting a tonsillectomy approach. Materials and methods: Data were exclusively acquired from medical records provided by the "Headline" clinic, thus, there was no direct contact with patients. Patients had to be at least 18 years old and residents of Latvia to be included in the study. The following parameters were collected from the patients’ medical records: surgery technique (BiZact or Cold steel), gender, age, indications for surgery, surgical and post-operative complications, use of pain medication, postoperative complaints, comorbidities, and antibiotic usage. Results: Statistical analyses revealed no significant differences between the two groups in any of the following variables: intraoperative bleeding (p = .118), postoperative bleeding (p = .087), sore throat (p = .266), pain (p = .583), pain medication (p = .859), postoperative patient complaints (p = .689), and comorbidities (p = .327). Only the antibiotic usage parameter demonstrated a significant difference in favor of the BiZact TE (p = .038). More specifically, no BiZact patient required antibiotics after surgery, whereas three patients of the Cold steel TE group did. Conclusions: The outcome variables investigated here did not differ between the BiZact and Cold steel tonsillectomy devices, thus, it cannot be concluded that the "hot" procedure (BiZact) is preferable to the "cold
