Ievads. Ģimenes hiperholesterinēmija (ĢH) ir viena no biežāk ģenētiski pārmantotām slimībām visā pasaulē. Tā raksturojas ar paaugstinātu zema blīvuma lipoproteīna holesterīna (ZBL-H) līmeni asinīs. Darba mērķis. Izpētīt koronārās sirds slimības biežumu pacientiem, kuri ir Latvijas Ģimenes hiperholesterinēmijas reģistra uzskaitē ar klīnisku ĢH. Darba metodes. Prospektīvā kohorta pētījumā tika iekļauti pacienti no LĢHR laika posmā no 02.2015 līdz 11.2022. Iekļaušanas kritēriji: iepriekš parakstīta piekrišana intervēšanai, probandi ar klīniski drošu vai ticamu ĢH, vērtējot pēc Nīderlandes Lipīdu klīniku tīkla kritērijiem vai pirmās pakāpes radinieki ar ZBL-H >95% percentīles. Rezultāti. Latvijas Ģimenes Hiperholesterinēmijas reģistrā līdz 2022.gada novembrim bija iekļauti 513 pacienti ar klīnisku ĢH, pētījuma kritērijiem atbilda 302 pacienti. Pētījuma ietvaros bija iespējams sazināties ar 252 pacientiem, 18 pacienti intervijai nepiekrita, rezultātā pētījumā iekļauti 234 pacienti. Koronārā sirds slimība līdz iekļaušanas brīdīm bija diagnosticēta 31,2% pacientu, pēc iekļaušanas reģistrā jaunatklāto KSS skaits palielinājās par 21,8%p, kopā 53,0% (n=124). No visiem KSS pacientiem, 18,0% bija simptomātiski, bet 35,0% asimptomātiski. Līdz iekļaušanas brīdim, miokarda infarkts (MI) anamnēzē bija 14 pacientiem. Pēcpārbaudes periodā spontāns MI tika diagnosticēts vienam pacientam, perkutāna koronāra intervence (PCI) veikta 11,9% pacientu un koronārā artēriju šuntēšana 1,7% pacientu. Stājoties LĢHR uzskaitē, hipolipidemizējošu terapiju lietoja 47,9% pacientu (n=112), bet pēc iekļaušanas lietoja papildus 35,9% (p<0,001), kopā 83,8% (n=196). Zema blīvuma holesterīna mediāna iekļaušanas brīdī bija 5,02+2,21 mmol/l, pēc pēdējās vizītes 4,0+2,27 mmol/l, sasniedzot 21,4% redukciju (p<0,001). Secinājumi. Latvijas Ģimenes Hiperholesterinēmijas reģistra speciālistu efektīvu pasākumu rezultātā pēc iekļaušanas reģistrā bija būtiski pieaudzis jaunatklāto koronārās sirds slimības gadījumu skaits un koronārā revaskularizācija tika veikta vairāk nekā ceturtdaļai pacientu. Spontāni lielie kardiovaskulārie notikumi attīstījās ļoti nelielai pacientu daļai, kas norāda uz ļoti zemu kardiovaskulāro notikumu attīstības risku īstermiņa periodā pēc iekļaušanas LĢHR. Introduction. Familial hypercholesterolemia (FH) is one of the most common genetically inherited diseases. It is characterized by elevated low-density lipoprotein levels in blood. Aim. Investigate coronary heart disease incidence in clinical FH patients who are listed in the Latvian Registry of Familial Hypercholesterolemia. Methods. In this retrospective cohort study patients were included from LRFH between 02.2015 and 11.2022. Inclusion criteria: previously signed consent for interview, probands with probable or definitive FH based on Dutch Lipid clinic network criteria, or first-degree relatives with LCL-C >95% percentile. Results. By November 2022, a total of 513 patients were diagnosed with clinical FH, and 302 patients were eligible for this study. Only 252 patients were reachable by phone, 18 refused the interview, 234 patients were enrolled in this study. Coronary heart disease (CHD) had been diagnosed in 31,2% of patients by the time of inclusion, with a 21,8% increase in newly discovered CHD after enrollment, giving a total of 53.0% (n=124). Of all CHD patients, 18.0% were symptomatic and 35.0% asymptomatic. Prior to inclusion, 14 patients had a history of myocardial infarction (MI). Spontaneous MI was diagnosed in one patient during follow-up, percutaneous coronary intervention (PCI) was performed in 11.9% of patients and coronary artery bypass grafting in 1.7% patients. Before inclusion in the LRFH 47.9% of patients (n=112) received lipid lowering therapy (LLT), followed by additional 35.9%p, after the last follow-up visit (p<0.001), with a total of 83.8% (n=196). Median low-density cholesterol at the inclusion was 5.02+2.21 mmol/l, after the last visit 4.0+2.27 mmol/l, achieving a reduction of 21.4%p (p<0.001) Conclusion. As a result of effective measures taken by specialists of the LRFH, the incidence of newly diagnosed CAD significantly increased after inclusion in the registry. Considering that coronary revascularization was performed for more than a quarter of patients, major cardiovascular events developed for a small proportion of patients, these data imply a very low short-term cardiovascular event risk when managed in Latvia.
