Koronāro artēriju oklūziju prevalence un tālākās ārstēšanas taktikas izvēle, veicot plānveida koronarogrāfiju

Pētījuma aktualitāte. Koronārā sirds slimība ir plaši izplatīta visā pasaulē, radot augstu mirstību. Neārstējot slimība progresē, kas veicina hronisku totālu oklūziju attīstību. Hroniskai totālai oklūzijai ir sliktākas prognozes aterotrombotiskiem notikumiem. Pētot riska faktorus var saprast, kas visvairāk ietekmē hronisku totālu oklūziju attīstību, kuras artērijas visbiežāk tiek skartas. Kā arī salīdzinot savā starpā metodes iespējams noteikt efektīvāku metodi dažādām pacientu grupām. Darba mērķis. Izpētīt hroniskas totālas oklūzijas sastopamību un ārstēšanas taktikas izvēli pacientiem, kam veikta plānveida koronārā angiogrāfija. Pētījuma metode. Pētījumā tika analizēti dati no Paula Stradiņa Klīniskās universitātes slimnīcas (PSKUS) Latvijas Kardioloģijas centra (LKC) dienas stacionāra datu bāzes. (Ētikas komitejas atzinums Nr. 120123-3L). Tika analizēti koronāro angiogrāfiju rezultāti, kuras veiktas laika posmā no 01.02.2019 līdz 01.05.2022. Kopējais respondentu skaits pētījumā 10911. Pētījumā tika izvērtēti hroniskas totālas oklūzijas riska faktori, skartās koronārās artērijas, izvērtēta taktikas izvēle. Rezultāti. Hroniska totāla oklūzija sastopama 13.5% (N=927) pacientiem ar KSS. 9.5% (N=653) pacientiem sastopama trīs artēriju slimība. HTO reģistrā iekļauti 9.0% jeb 615 pacienti. No visiem reģistra pacientiem trīs artēriju slimība sastopama 68.5% (N=421). HTO reģistra pacientu vidējais vecums 67.9 +/- 9.2 SD. Galvenie riska faktori Latvijā: hipertensija 90.1% (N=554), dislipidēmija 100% (N=615), sirds mazspēja 64.5% (N=395). Izvēlētā taktika: medikamentoza terapija 36.4% (N=224), PCI 38.2% (N=235), KAŠ 25.4% (N=256). Secinājumi. No 10911 analizētām pacientu koronārajām angiogrāfijām koronārā sirds slimība sastopama 62.9% pacientu (n=6868). No visiem KSS gadījumiem HTO sastopama 13.5% (n= 927) pacientiem. Latvijā visbiežāk sastopamie riska faktori ir dislipidēmija, arteriālā hipertensija, sirds mazspēja. Hroniskas totālas oklūzijas gadījumā biežāk skartā koronārā artērija ir RCA, kam seko LCX un LAD. Pētījumā tika iekļautas, tādas ārstēšanas metodes kā medikamentozā terapija, koronāro artēriju šuntēšana un perkutāna koronāra intervence. Visbiežāk izmantotā taktika hroniskas totālas oklūzijas gadījumā ir perkutāna koronāra intervence. Pētījumā izvirzītā hipotēze apstiprinājās. Introduction. Coronary heart disease is widespread worldwide, causing high mortality. Without treatment, the disease progresses, which contributes to the development of chronic total occlusions. Chronic total occlusion has a worse prognosis for atherothrombotic events. By studying the risk factor, one can understand what has the greatest influence on the development of chronic total occlusions, which arteries are affected. Also, it is possible to determine the method of different patients differently. Aim. To investigate the incidence and risk factors of development of chronic total occlusion and choice of tactics in patient groups. Materials and methods. The study analyzed data from the day-patient database of the Latvian Cardiology Center (LKC) of Paul Stradins Clinical University Hospital. (Ethics Committee No. 120123-3L). The results of coronary angiography performed in the period from 01.02.2019 to 01.05.2022. The total number of respondents in the study was 10911. The study evaluated the risk factors of chronic total occlusion, the affected coronary arteries, and evaluated the choice of tactics. Results. Chronic total occlusion occurs in 13.5% (N=927) of patients with CHD. Three-artery disease occurs in 9.5% (N=653) patients. 9.0% or 615 patients are included in the CTO register. Of all patients in the registry, three-artery disease occurs in 68.5% (N=421). The mean age of CTO registry patients is 67.9 +/- 9.2 SD. The main risk factors in Latvia: hypertension 90.1% (N=554), dyslipidemia 100% (N=615), heart failure 64.5% (N=395). Selected tactics: drug therapy 36.4% (N=224), PCI 38.2% (N=235), CABG 25.4% (N=256). Conclusions. Out of 10911 analyzed coronary angiograms of patients, coronary heart disease occurs in 62.9% of patients (n=6868). Of all CHD cases, CTO occurs in 13.5% (n= 927) of patients. Risk factors found in Latvia are dyslipidemia, arterial hypertension, heart failure. The coronary artery most commonly affected by chronic total occlusion is the RCA, followed by the LCX and the LAD. Treatment methods such as drug therapy, coronary artery bypass grafting and percutaneous coronary intervention were included in the study. The most commonly used tactic in chronic total occlusion is percutaneous coronary intervention. The hypothesis put forward in the study was confirmed.
