Psihomotorās attīstības izvērtējums bērniem ar hipoksiski - išēmisko encefalopātiju pēc terapeitiskās hipotermijas

Jaundzimušajiem, kuriem dzemdībās diagnosticēta vidēji smaga vai smaga hipoksiski-išēmiska encefalopātija, ir augstāks risks, ka tiem attīstīsies psihomotorās attīstības traucējumi vai iestāsies pacienta nāve. Agrīna pacienta adekvāta novērtēšana un savlaicīga ārstēšanas uzsākšana ar terapeitiskās hipotermijas metodi mazina riskus, savukārt ārstēšanas laikā un pēc tās veiktie izmeklējumi palīdz noteikt iespējamo pacienta klīnisko stāvokli, kas ietekmē pacienta psihomotorās attīstības traucējumu attīstības prognozi. Nav iespējams precīzi noteikt, kādi psihomotorās attīstības traucējumi pacientam attīstīsies, bet ir iespējams noteikt augstāka riska pacientus smagam neiroloģiskam stāvoklim, izmantojot dažādus pacienta novērtēšanas kritērijus. Lai palīdzētu pacientiem mazināt traucējumu attīstību, Latvijā pacienti tiek iekļauti BKUS neonatologu 2 gadu dinamiskās novērošanas (follow-up) programmā, kurā tiek regulāri izvērtēti, konsultēti un nodrošināti ar attiecīgo speciālistu, izmeklējumu un rehabilitācijas programmu atbalstu. Secinājumi: Pacientiem, kuri ārstēti ar terapeitisko hipotermiju, vecumam atbilstoša psihomotorā attīstība novērota 57% gadījumu, tāpat šiem bērniem konstatēta normāla fiziskā attīstība. Biežāk konstatētie psihomotorās attīstības traucējumi 2 gadu vecumā šiem bērniem ir valodas attīstības traucējumi (23%,) un bērnu cerebrālā trieka (BCT) (6%). Noskaidrota psihomotorās attīstības traucējumu asociācija ar jaundzimušā klīniskajiem raksturlielumiem un agrīniem attīstības novērtēšanas parametriem. Konstatēta statistiski nozīmīga sakarība starp magnētiskajā rezonansē konstatētām hipoksiskām izmaiņām 5.-7. dzīves dienā un BCT attīstību (p<0,001). Pacientiem, kam diagnosticēja BCT, biežāk bija vērojams suboptimāls neiroloģiskais izvērtējums atbilstoši HINE 12 mēnešu vecumā (p=0,002). Bērniem, kuriem konstatēta HIE, psihomotorās attīstības traucējumu risks 2 gadu vecumā ir lielāks, ja Apgares novērtējums 5.minūtes beigās ir zems (0-3 balles) (p=0,011) vai ir konstatēta smaga HIE (p=0,018). Neonates with moderate to severe hypoxic-ischemic encephalopathy at birth are at a higher risk of developing psychomotor impairment or death. Adequate early assessment of the patient with risk of therapeutic hypothermia and timely initiation of treatment affects the prognosis of the patient’s development of psychomotor development disorders, namely lessening the risks of them developing. It is not possible to determine exactly what psychomotor impairment patient will develop but it is possible to identify a patient at higher risk for a severe neurological condition using various patient evaluation criteria. In order to help patients reduce the development of disorders patients in Latvia are included in a 2-year dynamic neonatology follow-up program of the Children’s Clinical University Hospital (CCUH) in which they are regularly evaluated, consulted and provided with the support of relevant specialists, examinations and rehabilitation programs. Conclusions: In patients treated with therapeutic hypothermia, age-appropriate psychomotor development was observed in 57% of cases, and normal physical development was also found in these children. The most frequently detected disorders of psychomotor development at the age of 2 in these children are speech development disorders (23%) and cerebral palsy (CP) (6%). The association of psychomotor development disorders with clinical characteristics of the newborn and early developmental assessment parameters was determined. A statistically significant relationship was found between hypoxic changes detected in the magnetic resonance 5.-7. day of life and development of CP. (p<0.001). Patients diagnosed with CP had a higher frequency of suboptimal neurological assessment according to HINE at 12 months of age (p=0.002). In children diagnosed with HIE, the risk of psychomotor development disorders at the age of 2 years is higher if the Apgar score at the end of the 5th minute is low (0-3 points) (p=0.011) or severe HIE is found (p=0.018).
