Ziemeļvalstu diētai raksturīgu produktu ietekme uz kuņģa gļotādas atrofiju

Ievads. Kuņģa gļotādas atrofija ir pirmsvēža stāvoklis, kas ir saistīts ar paaugstinātu kuņģa vēža attīstības risku. Šobrīd ir jau apzināti daži kuņģa vēža novēršanas pasākumi, piemēram, smēķēšanas atmešana, sāls patēriņa samazināšana un H. Pylori izskaušanas terapija, taču citi modificējamie riska faktori kā specifiski uztura paradumi un diētas, piemēram, Ziemeļvalstu diēta, ir nepietiekami izpētīti, un tāpēc varētu sniegt labāku izpratni par šīs diētas un tās atsevišķo produktu ietekmi uz kuņģa gļotādas atrofiju. Pētījuma mērķis bija identificēt Ziemeļvalstu diētas un tās dažādo produktu saistību ar pepsinogēna līmeņa izmaiņām serumā, kas liecina par kuņģa atrofiju. Materiāli un metodes. Pētījums tika veikts GISTAR pētījuma – daudzcentru randomizēta populācijas pētījuma ietvaros. Dalībnieki vecumā no 40 līdz 64 gadiem Latvijā līdz 2019. – 2021. veica Ziemeļvalstu diētas uztura aptauju. Katrs pārtikas produkts aptaujā tika definēts kā produkta patēriņa biežums no “nekad”, “reizi dienā” un līdz pat “vairākas reizes dienā”. Pamatojoties uz aptaujā sniegtajām atbildēm, dalībnieki tika sadalīti 3 diētas ievērošanas pakāpēs. Papildus tika ņemti asins paraugi, lai iegūtu datus par pepsinogēna līmeni serumā (Eiken Pg I, II un Eiken Pg I / II attiecība), lai novērtētu kuņģa gļotādas seroloģisko stāvokli. Savākto datu statistiskā analīze tika veikta, izmantojot IBM SPSS Statistics, izmantojot Spīrmena korelāciju, hī kvadrāta testu un T testu. Rezultāti. No 907 pieaugušajiem (vidējais vecums 53,72 (SD 6,66), 69% sievietes), kas veica uztura aptauju un sniedza datus par seroloģiskiem pepsinogēna mērījumiem, 8,5% (N=60) bija seroloģiski noteikta kuņģa atrofija. Statistiski nozīmīga pozitīva korelācija ar PgI (p=0,005) un PgII (p=0,001) tika novērota ar griķu patēriņu. Arī rupjmaizes patēriņš pozitīvi korelēja ar PgI/PgII attiecību (p=0,024). Piena patēriņš pozitīvi korelēja ar PgI (p=0,043). Un tika konstatēta statistiski nozīmīga pozitīva korelācija starp pākšaugu patēriņu un PgI/PgII attiecību (p=0,044). Netika konstatēta statistiski nozīmīga saistība starp Ziemeļvalstu diētas ievērošanu un pepsinogēna līmeni (p=0,62). Secinājumi. Šajā pētījumā netika konstatēta saistība starp pašu Ziemeļvalstu diētu un pepsinogēna līmeni, tomēr vairāki specifiski Ziemeļvalstu diētas produkti bija pozitīvi saistīti ar zemāku seroloģiskās kuņģa atrofijas līmeni, tāpēc šiem produktiem potenciāli varētu būt aizsargājoša loma kuņģa pirmsvēža bojājumu attīstībā. Objectives. Gastric mucosal atrophy is a pre-cancerous condition that carries an increased risk of developing gastric cancer. There are some already known gastric cancer preventing lifestyle changes such as quitting smoking, reducing salt consumption, and H.Pylori eradication therapy, but other modifiable factors such as specific dietary factors and diets, such as Nordic diet have been understudied and therefore could provide a better understanding of the impact this diet and its products have on gastric atrophy. The aim of the study was to identify the Nordic diet and its different dietary product association with changes in serum pepsinogen levels, that are indicative of gastric atrophy. Materials and methods. The study was performed within a framework of GISTAR study- a multicentric randomized population study. Participants aged 40 to 64 in Latvia through 2019. – 2021. conducted a Nordic diet dietary survey based on memory recall. Each food item in the survey was defined as the frequency of consumption from “never”, “once a day” and all the way to “multiple times a day”. Based on the answers given in the survey, participants were divided into 3 adherence groups. Additionally, blood samples were taken to obtain data on serum pepsinogen (Eiken Pg I, II and Eiken Pg I/II ratio) to assess serological gastric mucosa status. Statistical analysis of the collected data was performed by using IBM SPSS Statistics, using Spearman correlation, chi-squared test and T-test. Results. Out of 907 adults (mean age 53.72 (SD 6.66), 69% Female) that conducted a diet survey and provided data on serological pepsinogen measurements 8.5% (N=60) had serologically detected gastric atrophy. A statistically significant positive correlation with PgI (p=0.005) and PgII (p=0.001) was observed for buckwheat consumption. Also, rye bread consumption was positively correlated with the PgI/PgII ratio (p=0.024). Consumption of milk was positively correlated with PgI (p=0.043). And a statistically significant positive correlation was found between legumes consumption and PgI/PgII ratio (p=0.044). There was found no statistically significant association between Nordic diet adherence and pepsinogen levels (p=0.62). Conclusions. In this study there was no association found between the Nordic diet itself and pepsinogen levels, however, several specific Nordic diet products were positively associated with lower serological gastric atrophy rates, therefore these products could potentially play a protective role in the development of gastric precancerous lesions.
