Aprūpe pacientiem ar mākslīgo plaušu ventilāciju intensīvās terapijas nodaļā

Abstract

Bakalaura darba tēma “Aprūpe pacientiem ar mākslīgo plaušu ventilāciju intensīvās terapijas nodaļā”. Tēmas aktualitāti nosaka intensīvās terapijas liels intrahospitālās pneimonijas saslimšanas skaits pacientiem pie mākslīgās plaušu ventilācijas un to, ka no māsas aprūpes kvalitātes ir atkarīga pacienta atveseļošanās prognoze un iegūto komplikāciju risks. Darba mērķis ir noskaidrot galvenos aprūpes pamatprincipus pacientiem ar mākslīgo plaušu ventilāciju intensīvās terapijas nodaļā. Pētniecības uzdevumi: 1) Apkopot un analizēt literatūras avotus, vadlīnijas par mākslīgo plaušu ventilācijas pacientu aprūpi intensīvās terapijas nodaļā; 2) Analizēt Dorotejas Elizabetes Oremas (Dorothea Elisabeth Orem) māszinību teoriju saistībā ar bakalaura darba tēmu; 3) Izstrādāt pētījuma instrumentu - intervijas jautājumus; 4) Veikt pilotpētījumu un pētījumu; 5) Apkopot un analizēt iegūtos rezultātus; 6) Izstrādāt secinājumus. Pētniecības jautājums ir - kādi ir aprūpes pamatprincipi pacientiem ar mākslīgo plaušu ventilāciju intensīvās terapijas nodaļā? Pētījumā tika izmantota kvalitatīvā pētniecības metode un instruments - nestrukturēta intervija. Respondenti ir “X” stacionāra intensīvas terapijas nodaļas māsas, kas tika atlasīti pēc nevarbūtīgā ērtuma izlases metodes. Intervijā piedalījās 10 respondenti, vecumā no 25 līdz 43 gadiem. Pētniecības rezultātā tika secināts, ka par aprūpes pamatprincipiem intensīvās terapijas māsas uzskata: pacienta oksigenāciju pie mākslīgās plaušu ventilācijas, elpceļu caurlaidības nodrošināšanu un infekcijas risku novēršanu. Māsa nodrošina pacientam pie mākslīgās plaušu ventilācijas visas dzīvībai nepieciešamās funkcijas, tādas kā: barošana, uzņemtā un izvadītā šķidruma nodrošināšana, pacientu pozicionēšana u.c. Tika arī noskaidrots, ka biežākās aprūpes problēmas pacientiem pie mākslīgās plaušu ventilācijas ir izgulējumi un infekcijas.

The topicality of the topic is determined by the high number of cases of intrahospital pneumonia in intensive care patients on artificial lung ventilation and the fact that the prognosis of the patient's recovery and the risk of acquired complications depend on the quality of nursing care. The aim of the work is to find out the main basic principles of care for patients with artificial lung ventilation in the intensive care unit. Research tasks are: 1) Collect and analyze literature sources, guidelines on the care of artificial lung ventilation patients in the intensive care unit; 2) Analyze Dorothea Elisabeth Orem's theory of nursing in connection with the topic of the bachelor thesis; 3) Develop a research tool - interview questions; 4) Conduct a pilot study and research; 5) Summarize and analyze the obtained results; 6) Develop conclusions. The research question is - what are the basic principles of care for patients with artificial lung ventilation in the intensive care unit? The study used the qualitative research method and the tool unstructured interview. The respondents are nurses from the inpatient intensive care unit of “X” who were selected by non-probability sampling method. 10 respondents aged between 25 and 43 took part in the interview. As a result of the research, it was concluded that intensive care nurses consider oxygenation of the patient during artificial lung ventilation, ensuring the patency of the patient's airways and prevention of infection risks as the basic principles of care. The nurse provides the patient with artificial palpitation ventilation with all the functions necessary for life, such as: feeding, ensuring intake and output of liquid, patient positioning, etc. It was also found out that more frequent care problems for patients on artificial lung ventilation are bed rest and infections.
