Taisnās zarnas vēža ārstēšanas taktika atkarībā no klīniskās stadijas un tās korelācija ar patoloģisko stadiju

Pētījuma aktualitāte. Taisnās zarnas vēža izplatība Latvijā ir piektā vietā no visiem vēža paveidiem starp vīriešiem un sievietēm. Visbiežāk pacienti ar taisnās zarnas vēzi vēršas pēc palīdzības jau ar izteiktu simptomātiku un retāk to atklāj agrīni. Noskaidrojot pacienta klīnisko stadiju, ir iespējams nozīmēt pacientam adekvātu ārstēšanas terapiju un noteikt korelāciju ar patoloģisko atradni ārstēšanas gaitā. Darba mērķis. Izpētīt ārstēšanas taktiku atkarībā no taisnās zarnas vēža klīniskās stadijas un noteikt korelāciju ar patoloģisko stadiju. Pētījuma metode. Kvantitatīvā (pielietots hī-kvadrāta tests). Pētījuma ietvaros tika analizēti taisnās zarnas vēža pacientu izveidota datu bāze, kura tika izveidota RAKUS “Latvijas Onkoloģijas Centrs” retrospektīva pētījuma ietvaros (Ētikas komitejas atzinumu Nr. 19-25/15). Kopējais respondentu skaits ir 112 pacienti. Datu analīzē tika iekļauta informācija par taisnās zarnas vēža pacientiem, kuriem tika nozīmēta dažāda veida ārstēšana atkarībā no klīniskās stadijas un veikta patoloģiskā stadijas atradnes uzskaite. Rezultāti. Kopumā pacientiem šajā izlasē tika biežāk nozīmēta un veikta neoadjuvanta ķīmijterapija 51% (N=57) gadījumu, neoadjuvanta staru terapija 49% (N=55) gadījumu un radikāla operācija tika veikta 48% (N=54) gadījumu. Stoma tika izveidota 17% (N=19) gadījumu un 1% (N=1) gadījumu tika pielietota paliatīva ķīmijterapija. Pastāv statistiski nozīmīga saistība starp taisnās zarnas vēža klīnisko stadiju un, tādiem primārās terapijas veidiem kā, neoadjuvantu staru terapiju, neoadjuvantu ķīmijterapiju, radikālu operāciju, un paliatīvu ķīmijterapiju (visos gadījumos p<0,001), bet saistība ar kolostomu netika konstatēta (p=0,113). Pēc patoloģiskās atradnes rezultātiem izlasē vislielākais procentuālais pacientu daudzums 46% (N=52) bija ar I stadiju, tālāk seko pacienti ar IIA stadiju 16% (N=18), 14% (N=16) ar 0 stadiju, 13% (N=14) pacientu ar IIIB stadiju, 6% (N=7) pacientu ar IV stadiju un 1% (N=1) ar IIIC stadiju. Stadijas IIB un IIC netika konstatētas nevienam pacientam. Pētījums uzrādīja, ka veicot patoloģisko stadijas noteikšanu, tā tika noteikta statistiski nozīmīga (p=0,001), bet ar salīdzinoši vāju saistību (ρ=0,299**) starp patoloģisko un klīnisko taisnās zarnas vēža stadiju. Secinājumi. RAKUS “Latvijas Onkoloģijas Centrs” visbiežāk pielietotā ārstēšanas terapija ir neoadjuvanta ķīmijterapija, kurai seko neoadjuvanta staru terapija, radikāla operācija, kolostomijas izveide un paliatīva ķīmijterapija. Ārstēšana tika nozīmēta kā primārā terapija, uz kuras pamata tika ņemta vērā pacienta klīniskā stadija pirms terapijas. Pamatojoties uz iegūtajiem datiem, var secināt, ka pastāv statistiski nozīmīga saistībā ar visām uzskaitītām terapijas izvēlēm, izņemot kolostomiju. Pozitīvā korelācija starp patoloģisko un klīnisko stadiju norāda uz to, ka pacientiem ar augstāko vēža stadiju pirms operācijas un/vai terapijas pielietošanas, bija arī salīdzinoši augstākā vēža stadija pēc terapijas pielietošanas. Introduction. The incidence of rectal cancer in Latvia ranks fifth among all types of cancer for both men and women. Patients with rectal cancer most commonly seek medical help when they already have developed symptoms, and less frequently it is detected in earlier stages. Determining the clinical stage of the patient makes it possible to prescribe appropriate treatment therapy and establish a correlation with pathological findings during the course of treatment. Aim. Research the treatment tactics depending on the clinical stage of rectal cancer and establish a correlation with the pathological stage. Materials and methods. Quantitative (applied chi-square test). Within the framework of the study, a database created with patients with rectal cancer was analyzed, which was established within the framework of the prospective study of the RAKUS "Latvian Oncology Center" (Ethics Committee opinion No. 19-25/15). The total number of respondents is 112 patients. Data analysis included information on rectal cancer patients who were assigned different types of treatment depending on the clinical stage and pathological stage findings. Results. Overall, in this cohort, neoadjuvant chemotherapy was more frequently prescribed and performed in 51% (N=57) of cases, neoadjuvant radiotherapy in 49% (N=55) of cases, and radical surgery was performed in 48% (N=54) of cases. A colostoma was created in 17% (N=19) of cases, and palliative chemotherapy was applied in 1% (N=1) of cases. There was a statistically significant association between the clinical stage of rectal cancer and primary therapy modalities such as neoadjuvant radiotherapy, neoadjuvant chemotherapy, radical surgery, and palliative chemotherapy (in all cases p<0,001), but no assocation with colostomy was found (p=0,113). Based on the pathological findings, the largest percentage of patients, 46% (N=52), were in stage I, followed by patients in stage IIA at 16% (N=18), 14% (N=16) in stage 0, 13% (N=14) in stage IIIB, 6% (N=7) in stage IV, and 1% (N=1) in stage IIIC. Stages IIB and IIC were not detected in any patient. The study showed that when determining the pathological stage, a statistically significant (p=0,001) but relatively weak correlation (ρ=0,299**) was found between the pathological and clinical stages of rectal cancer. Conclusions. In RAKUS “Latvian Oncology Centre”, the most commonly used treatment therapy is neoadjuvant chemotherapy followed by neoadjuvant radiotherapy, radical surgery, colostomy creation, and palliative chemotherapy. Treatment was prescribed as primary therapy, based on the patient's clinical stage that was staged before therapy. Based on the obtained data, it can be concluded that there is a statistically significant association with all listed treatment choices except for colostomy. A positive correlation between pathological and clinical stages indicates that patients with higher cancer stages before surgery and/or therapy also had relatively higher cancer stages.
