Nosaukums: Uz RL-PĶR balstīta urīnpūšļa marķiera EpiCheck kā diagnostikas instrumenta izvērtējums augšējo urīnceļu audzēju gadījumā Autore: Margherita Palermo Darba vadītaja: Assoc. Prof. Dr. med. Egils Vjaters Ievads: Augšējo urīnceļu uroteliāla karcinoma (AUUK) veido aptuveni 5–10% no visiem urotēlija audzējiem. Pēdējo desmitgažu laikā tās biežums ir pieaudzis. Pašreizējie izmeklējumi AUUK diagnostikā ir citoloģija, datortomogrāfija (DT), urogrāfija un ureterenoskopija (URS). Šī pētījuma mērķis bija novērtēt Bladder Epicheck® testa, kā diagnostikas instrumenta, lietderību AUUK diagnostikā. Materiāli un metodes: Ar citoloģijas un Bladder Epicheck® testu tika analizēti 136 urīna paraugi, kas savākti no augšējiem urīnceļiem (AU) pirms URS, lai konstatētu AUUK. Sešpadsmit analīzes netika iekļautas pētījumā dēļ nediagnostiska Bladder Epicheck® vai citoloģijas. Abu marķieru jūtība, specifiskums, pozitīvā paredzamā vērtība (PPV) un negatīvā paredzamā vērtība (NPV) tika aprēķināta un salīdzināta, izmantojot URS un/vai histoloģiju kā atsa09876ci. Rezultāti: 40 gadījumos (33,3%) tika konstatēts audzējs, 30 bija zemas diferenciācijas pakāpes (ZP) un 10 augstas diferenciācijas pakāpes (AP). Kopējā jūtība bija 65% Bladder Epicheck® un 42,5% citoloģijai, palielinoties līdz 100% Bladder Epicheck® un 90% citoloģijai, ja ņemam vērā tikai AP audzējus. Bladder Epicheck® specifiskums bija 81,2%, bet citoloģijas - 93,7%. PPV un NPV bija 63,4% un 82,2% Bladder Epicheck® un 77,2% un 76,5% citoloģijai. Ņemot vērā Episcore robežvērtību >75, nevis 60, Bladder Epicheck® specificitāte uzlabojas līdz 89%, bet PPV līdz 74,2%. Ja tiek vērtēti abi testi kopā, pavisam nedaudz uzlabojas jūtība un NPV. Secinājumi: Tā kā AP audzēju diagnostikā ir augsta jūtība, Bladder Epicheck® testu var izmantot, lai novērotu pacientus ar AUUK pēc konservatīvas ārstēšanas un samazinātu nevajadzīgas procedūras, ar ļoti zemu risku palaist garām AP recidīvu vai progresēšanu. Egils Vjaters Introduction: Upper urinary tract urothelial carcinoma (UTUC) represents about 5-10% of all urothelial neoplasm with increasing incidence in the last few decades. The current standard tools in the diagnosis of UTUC include Cytology, Computed Tomography (CT) Urography and Ureterorenoscopy (URS). The aim of this study was to evaluate the usefulness of Bladder Epicheck® test as diagnostic tool in the diagnosis of UTUC. Material and Methods: 136 urine samples collected from upper urinary tract (UUT) before URS for suspicion of UTUC were analysed with Cytology and Bladder Epicheck® Test. Sixteen analyses were excluded due to a non-diagnostic Bladder Epicheck® or Cytology. Sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of both markers were calculated and compared using URS and/or histology as reference. Results: In 40 cases (33.3%) a tumour was found, 30 were low-grade (LG) and 10 high-grade (HG). Overall sensitivity was 65% for Bladder Epicheck® and 42.5% for Cytology, increasing to 100% for Bladder Epicheck® and 90% for cytology if we consider only HG tumours. Overall specificity of Bladder Epicheck® was 81.2% and of Cytology 93.7%. The PPV and the NPV were 63.4% and 82.2% for Bladder Epicheck® and 77.2% and 76.5% for cytology. Considering an Episcore cut-off >75, instead of 60, the specificity of Bladder Epicheck® improves to 89% and the PPV to 74.2%. If we evaluate the 2 tests together, we assist only to a slight improvement of sensitivity and NPV. Limitations of this study include the use of a marker validated only for bladder cancer and the relatively small number of cases. Conclusions: Due to the high sensitivity for HG tumours, the Bladder Epicheck® Test can be used in diagnosis and treatment decision-making of UTUC. Furthermore, it can be very useful in follow-up of the patients with UTUC after endoscopic treatment to reduce the unnecessary procedure and invasive follow-up regimens with a very low risk to miss a HG recurrence or progression.
