Plaušu audzēja histopatoloģisko kritēriju izstrāde potenciālai imūnterapijas efektivitātes izvērtēšanai

Temas aktualitāte: Plaušu audzējs ir viens no vadošajiem nāves iemesliem pasaulē. Un ar katru gadu pieaug jaunatklāto gadījumu skaits. Bieži vien plaušu vēzi diagnosticē novēloti, kad slimība ir jau 3 vai 4 stadijā, pielietojot imūnterapiju ārstēšanas procesā iespējams uzlabot izārstēšanas iespējas pacientiem. Mērķis: izmantojot mākslīgā intelekta (neironu tīkla) analīzes un statistikas metodes novērtēt saistības starp plaušu audzēja klīniskajiem, histopatoloģiskajiem rādītājiem un imūnterapijas efektivitāti. Metodes: Retrospektīvs pētījums veikts P. Stradiņa klīniskās Universitātes slimnīcas (PSKUS) Patoloģijas Institūtā un LU Medicīnas Fakultātes Patoloģijas katedrā. Pētījumā iekļauti 108 pacienti, kuriem pirmreizēji tika diagnosticēts plaušu audzējs laika periodā no 2021.gada 1.janvāra līdz 2023.gada 30. decembrim. Dati ievākti no Paula Stradiņa klīniskas universitātes slimnīcas informācijas sistēmas “Ārstu birojs”, PSKUS Patoloģijas Institūta datu bāzes “Dialab”, kā arī no “Datamed” sistēmas. Neironu tīkla analīzei tika izmantota “Microsoft Cognitive Toolkit” programma. Dati apkopoti un uzglabāti “Microsoft Excel” programmā, bet apstrādāti “GraphPad Prism” programmas 10.versijā. Rezultati: Pētījumā tika iekļauti 108 pacienti, kuru minimālais vecums bija 45 gadi, maksimālais vecums bija 85 gadi un vidējais vecums bija 69,3 gadi. No 108 pacientiem 76 (70,4%) bija vīrieši, bet 32 (29,6%) bija sievietes. Vidējais vīriešu vecums, kas tika iekļauti pētījumā bija 68,2 gadi, savukārt vidējais sieviešu vecums bija 72,1 gadi. Salīdzinot pacientu vecumu un dzimumu tika konstatēta statistiski ticama sakarība, ka sievietēm plaušu vēzis tiek atklāts lielākā vecumā, nekā vīriešiem (P=0,0234). 2 (1,85%) pacientiem bija pirmā diferenciācijas pakāpe (Grade 1). Otrā diferenciācijas pakāpe (Grade 2) tika noteikta 70 (64,8%) slimniekiem. Plaušu vēzis ar trešo diferenciācijas pakāpi (Grade 3) esot bijis 36 (33,3%) pacientiem. 36 (76,6%) pacientiem ar plakanšūnu vēzi tika noteikta audzēja 2.diferenciācijas pakāpe, 10 (21,3%) audzējiem bija 3.diferenciācijas pakāpe. 1.diferenciācijas pakāpe bija viena pacientam gan ar plākanšūnu vēzi (2,1%), gan ar adenokarcinomu (1,6%). 34 (55,7%) adenokarcinomam bija 2.diferenciācijas pakāpe, bet 26 (42,6%) bija 3.diferenciācijas pakāpe. Tika konstatēta statistiski nozīmīga saistība starp audzēja histoloģisku tipu un diferenciācijas stadiju (P= 0,0252). PD-L1 ekspresija netika atrasta 25 (23%) pacientiem, savukārt pārējiem tā bija vai nu pozitīva – 48 (44%) pacientiem, vai nu augsti pozitīva – 35 (33%) pacientiem. Pacientiem ar adenokarcinomu negatīva PD-L1 ekspresija bija novērojama 2 reizēs biežāk (29,5%), nekā slimniekiem ar plakanšūnu plaušu vēzi (16%). Pozitīva ekspresija abiem plaušu vēža tipiem bija procentuāli vienāda – 44%. Augsti pozitīva ekspresija ir novērojama 16 (26,5%) adenokarcinomas pacientiem un 19 (40%) slimniekiem ar plakanšūnu plaušu vēzi. Ir novērota statistiski nozīmīga saistība starp PD-L1 ekspresiju un Introduction: lung tumour is one of the leading causes of death in the world. And with each year, the number of newly discovered cases is increasing. Lung cancer is often diagnosed late when the disease is already at 3rd or 4th stages, with immunotherapy the treatment process can improve the chances of cure in patients. Aim: to assess the relationship between clinical, histopathological parameters of the lung tumour and the efficacy of immunotherapy using artificial intelligence (network of neurons) analysis methods. Methods: a retrospective study has been conducted at the Institute of pathology at P. Stradins clinical University hospital (PSKUS) and at the department of pathology at LU Faculty of medicine. The study includes 108 patients who were first diagnosed with lung cancer between January 1, 2021 and December 30, 2023. The data was collected from the information system “Ārstu birojs” of Pauls Stradins clinical University hospital, database “DIALAB” of PSKUS pathology Institute, as well as from the “Datamed” system. A “Microsoft cognitive Toolkit” program was used to analyze the neural network. Data collected and stored in “Microsoft Excel”, but processed in version 10 of “GraphPad Prism”. Results: 108 patients with a minimum age of 45, a maximum age of 85 and an average age of 69.3 years were included in the study. Of the 108 patients, 76 (70.4%) were male, while 32 (29.6%) were female. The average age of men enrolled in the study is 68.2 years, while the average age of women was 72.1 years. A statistically reliable relationship was found between patients' age and gender that lung cancer is detected in women at a higher age than in men (P = 0.0234). 2 (1.85%) patients had the first grade of differentiation (Grade 1). The second grade of differentiation (Grade 2) was determined in 70 (64.8%) patients. Lung cancer with a third grade of differentiation (Grade 3) was reported in 36 (33.3%) patients. In 36 (76.6%) patients with squamous cell cancer, the 2nd differentiation grade of tumour was determined, 10 (21.3%) tumours had a 3rd degree of differentiation. The first grade of differentiation was one for both squamous cell cancer (2.1%) and adenocarcinoma (1.6%). 34 (55.7%) adenocarcinoma patients had a 2nd grade of differentiation, while 26 (42.6%) had a 3rd grade. A statistically significant relationship was found between tumour histological type and differentiation stage (P = 0.0252). PD-L1 expression was not found in 25 (23%) patients, while for others it was either positive in 48 (44%) patients or highly positive in 35 (33%) patients. In patients with adenocarcinoma, negative PD-L1 expression was seen 2 times more frequently (29.5%) than in patients with squamous cell lung cancer (16%). Positive expression was the same percentage for both types of lung cancer at 44%. Highly positive expression is observed in 16 (26.5%) adenocarcinoma patients and 19 (40%) squamous cell lung cancer patients. A statistically significant relationship has be
