Izvēlētā bakalaura darba tēma: “Attēla kvalitāti ietekmējoši faktori mugurkaula datortomogrāfijas izmeklējumos pacientiem ar mugurkaulā ievietotiem implantiem”. Tēmas aktualitāti raksturo plaši pielietotie mugurkaula pēcoperācijas datortomogrāfijas izmeklējumi, ievietoto metālisko mugurkaula implantu stāvokļa un iespējamo komplikāciju izvērtēšanai. Veiktajos pēcoperācijas datortomogrāfijas izmeklējumos mugurkaulam bieži vien vizualizējami ne tikai metāla implantu radītie artefakti, bet arī ar pacienta pozicionēšanu un ar neatbilstošu izmeklējuma parametru izvēli saistīti artefakti. Tādēļ jāuzsver pacienta pozicionēšanas precizitāte izmeklējuma laikā un spēja izmeklējuma protokolā esošos ekspozīcijas parametrus pielāgot pacienta ķermeņa īpatnībām, izprotot datortomogrāfijas attēlu artefaktu veidošanos, maksimāli nodrošinot kvalitatīvus, diagnostiski derīgus izmeklējumus pēcoperācijas pacientiem. Pētījuma mērķis: noskaidrot attēla kvalitāti ietekmējošus faktorus mugurkaula datortomogrāfijas izmeklējumos ar ievietotiem metāliskiem mugurkaula implantiem. Pētījuma metodoloģija: jaukta metode. Retrospektīvs pētījums. Pētījuma metodes: Pētījumā izmantotie instrumenti: autora sastādīta attēla kvalitāti ietekmējošu faktoru tabula, pēc kuras tika analizēti un izvērtēti 50 datortomogrāfijas izmeklējumi, kas veikti pacientiem ar ievietotiem metāliskiem mugurkaula implantiem. Pētījuma norise: Analizējot zinātnisko literatūru par mugurkaula krūšu un jostas daļas datortomogrāfiju, tika izveidota attēla kvalitāti ietekmējošu faktoru tabula. Pēc izveidotās tabulas, izvērtējot mugurkaula datortomogrāfijas izmeklējumus pacientiem ar ievietotiem mugurkaula implantiem, tika apkopoti biežākie attēla kvalitāti ietekmējošie faktori, tajā skaitā vizualizējamie attēla artefakti, pacienta pozicionēšanas neprecizitātes, un atspoguļoti diagrammās. Izvirzītā hipotēze: veicot precīzu pacienta pozicionēšanu atbilstoši pacienta ķermeņa īpatnībām un, izprotot DT attēlu artefaktus veidojošos faktorus, iespējams veikt kvalitatīvu, diagnostiski derīgu datortomogrāfijas izmeklējumu pacientam ar ievietotiem mugurkaula implantiem, tādējādi samazinot artefaktu veidošanos iegūtajā attēlā. Pētījuma rezultāti un secinājumi: 20% gadījumu tika pieļautas pacienta pozicionēšanas neprecizitātes, kas noveda pie DT attēlu artefaktu veidošanās vai izmeklējamās zonas anatomisko struktūru nepilnīgas vizualizācijas. Visbiežāk attēlos redzamie artefakti – metāla artefakti. The topic of the selected bachelor thesis is “Image quality influencing factors in computed tomography examinations of the spine in patients with spinal implants”. The relevance of the topic is characterised by the widespread use of post-operative computed tomography scans of the spine to assess the condition and possible complications of metallic spinal implants. Post-operative CT scans of the spine often visualise not only artefacts caused by metal implants, but also artefacts related to patient positioning and inappropriate choice of imaging parameters. Therefore, the accuracy of patient positioning during the examination and the ability to adapt the exposure parameters in the examination protocol to the patient's body characteristics with an understanding of CT image artefact formation should be emphasised, maximising the quality of diagnostically useful examinations in post-operative patients. Objective: to investigate factors influencing image quality in spinal CT scans with metallic spinal implants. Design: mixed method. Retrospective study. Methods: Outcome measures: a table of image quality factors was compiled by the author, after which 50 CT scans of patients with metallic spinal implants were analysed and evaluated. Process: a table of factors affecting image quality was created by analysing the scientific literature on CT of the thoracolumbar spine. The table was used to evaluate CT scans of the spine in patients with spinal implants, and the most common factors affecting image quality, including visualisation artefacts, patient positioning inaccuracies, were summarised and presented in charts. Main results and conclusions: in 20% of cases, patient positioning inaccuracies led to the formation of CT image artefacts or incomplete visualisation of the anatomical structures of the area to be imaged. Metal artefacts were the most frequent artefacts seen in the images.
