Uztura alerģijas riska faktori un saistība ar Helicobacter pylori infekciju bērniem

Priekšvārds: Uztura alerģija ir alerģiska slimība, kas sākas agrīnā bērnībā. Uztura alerģijas simptomi var skart ādu, gastrointestinālo sistēmu, respiratorisko sistēmu un variē no viegla ādas apsārtuma līdz smagam anafilaktiskām reakcijām. H. pylori ir baktērija, kas apdzīvo kuņģa gļotādu. H.pylori domājams ir inficējusi divas trešdaļas pasaules iedzīvotāju. Pastāv uzskats, ka bērniem, kuri ir inficējušies ar šo baktēriju ir mazāks risks saslimt ar alerģiskām slimībām.
Pētījuma mērķi
Darba mērķis ir gūt un sniegt ieskatu bērnu alerģiju attīstības gaitā. Izpētīt atopiju ietekmējošos faktorus, kā arī pierādīt, ka bērni, kuriem kuņģa zarnu traktā ir sastopama H. pylori, retāk cieš no pārtikas alerģijām.
Materiāli un metodes: Pētījumā tika iekļauti bērni ar apstiprinātu alerģijas diagnozi un kontroles grupā – bērni bez alerģijas diagnozes. Dalībnieku vecākiem lūdzam atnest vēdera izejas paraugus un aizpildīt anketas par ģimenes alerģiskām saslimšanām, kā arī bērna alerģiskām saslimšanām, par ģimenes anamnēzi, par medikamentu lietošanu, lai izvērtētu alerģijas izpausmi saistībā ar H. pylori infekciju un riska faktoriem. Statistiskā analīzei izmantota χ2 tests un Fišera tests.
Rezultāti: : Pētījumā tika izmeklēti 130 bērni vecumā no 2-11 gadiem. Pacientu grupa - 42 bērni (32%). H. pylori konstatēja 11/130 (8%) bērniem. H. pylori pozitivitāte bija augstāka bērniem, kuru vecāku izglītības līmenis ir zemāks un ir zemāks sociālekonomiskais stāvoklis. H. pylori infekcija biežāk tika atrasta bērniem, kuriem nav alerģijas diagnoze : 11/77 (12.50%) vs 0/42 (0%).
Secinājumi: Pētījumā tika noskaidrots, ka H.pylori pozitivitāte, bija statistiski ticami augstāka bērniem bez alerģijas nekā bērniem ar alerģiju. Bērniem, kuriem nebija datu par alerģiju ģimenē, bija retāk alerģijas diagnoze kā bērniem, kuriem atzīmēja alerģiju ģimenes anamnēzē. Lai apstiprinātu H. pylori lomu hipotēzes kontekstā, būtu jāpēta, infekcijas mijiedarbība ar citiem riska faktoriem. Preface: Food allergy is an allergic disease that begins early in childhood. Food allergy symptoms can affect the skin, gastrointestinal system, respiratory system and ranges from light skin redness to severe anaphylactic reactions. Helicobacter Pylori is a bacterium found in the gastric mucosa. H.pylori is believed to infect approximately two-thirds of the world’s population. However H.pylori prevalence is decreasing in developed countries. Opinion stands that children who are infected with this bacterium have smaller risk to become ill with allergic diseases.
Aims of the research: The aim of this work is to obtain and give insight in the development of children’s food allergies. To investigate factors that influences of allergies and possible treatment and to prove, that children who has Helicobacter pylori infection suffer less from food allergies.
Materials and methods: the study included children, aged 2-10 yrs with and without the diagnosis of allergy . The parents of participants were asked to bring stool samples and to fill out the questionnaire about allergic illnesses in the family, allergic reactions, drugs. Presence of H.pylori infection was detected by rapid stool antigen test. Data were processed by χ2 and Fisher exact test.
Results: 130 children aged 2 till 11 years. The group of patients was 42 (32%). Children with allergy diagnosis had significantly more often allergy in the family. Presence of H.pylori was found in 11/130 (8%) of children. H.pylori positivity was associated with a lower parental level of education and a lower social-economical status. None of children with the diagnosis of allergy had H.pylori: 11/77 (12%) vs 0/42 (0%).
Conclusions:. Our study showed higher H.pylori positivity in non-allergic children compared to children with allergy. To confirm the role of H.pylori in the context of hypothesis the interaction of the bacteria with other risk factors should be investigated further.
