Morfofunkcionālo kritēriju izstrāde hroniska tonsilīta diagnostikā un ārstēšanā

Abstract

Pētījuma mērķis bija novērtēt morfoloģiskās pārmaiņas aukslēju mandelēs akūta un hroniska tonsilīta pacientiem, kā arī mikrofloras nozīmi tonsilīta izcelsmē. Histoloģiskais materiāls tika ņemts no 36 operētiem pacientiem, kas bija 10 – 18 gadus veci un kam tika veikta mandeļu mikrobioloģiskā un virusoloģiskā izmeklēšana un tonsilektomija. Hroniska tonsilīta pacientiem novēro samazinātu T regulatoro limfocītu (Treg), IgA un IgG pozitīvo šūnu skaitu, kā arī palielinātu antigēnprezentētājšūnu skaitu (S-100 pozitīvās šūnas). Pētījums liecina, ka uz Treg un antigēnprezentētājšūnu sistēmas pārmaiņu fona rezidentmikrofloras antigēni izraisa hronisku iekaisumu mandelēs. T regulotoro limfocītu skaits varētu būt kritērijs tonsilīta un imunoloģisko traucējumu diagnostikā.

ABSTRACK The aim of research was to compare the inflammatory changes and the role of local microflora in palatine tonsils in patients with acute and recurrent tonsilltis. Histological material was taken from 36 patients with a history of tonsillitis. Patients were 10 – 18 years old. Patients with recurrent tonsillitis had less T regulatory cells in parafollicular regions compared to acute tonsillitis. Furthermore, IgA and IgG positive celss were decreased in tonsils tissue. Patients with recurrent tonsillitis had more S-100 cells. Recurrent tonsillitis probably results from a lack of mucosal tolerance to mixed commensal microflora-viruses, grampositive and gramnegative bacteria. T regulatory cells could be the morphofunctional biomarker of recurrent tonsillitis.
