Abstract
Ievads: hipospādija ir iedzimts vīrišķo ārējo dzimumorgānu defekts, kas ir visbiežāk sastopamā urīnizvadkanāla anomālija. Savlaicīga diagnostika un agrīna ārstēšana sekmīgi novērš dzimumlocekļa defektu.
Diplomdarba mērķis: gūt priekšstatu par hipospādijas biežumu Latvijā, izzināt tās cēloņus, ārstēšanas efektivitāti un komplikācijas. Darbā izmantoti BKUS Bērnu ķirurģijas klīnikas dati par laika periodu no 2005. līdz 2009. gadam. Pētījumā izmantoti 104 slimnieku klīnisko novērojumu materiāli.
Rezultāti: hipospādijas formu biežums: Hypospadia distalis 42% (n=42), Hypospadia medialis 30% (n=31), Hypospadia proximalis 28% (n=29). Operāciju veidi: vienetapa rekonstruktīvas operācijas 92% (n=96), divetapu 8% (n=8). Komplikācijas biežāk sastapa pēc divetapu operācijām.
Secinājumi: tikai 52% hipospādijas pacientu tiek operēti optimālā vecumā. Lai uzlabotu pacientu ārstēšanas rezultātus, nepieciešams izstrādāt hipospādijas diagnostikai un ārstēšanai veltītas rekomendācijas.
Background: hypospadia is a common congenital urogenital abnormality resulting in incomplete development of the anterior urethra. Early diagnostics and early treatment of the pathology provides successful elimination of penile defects. The aim of the research work: to obtain a percept of hypospadias frequency and their forms in Latvia, to find out the possible causes of hypospadia, to clarify the surgical repair methods of hypospadias used in Latvia, their effectivness, and frequency of complications. Results: frequency of forms: Hypospadia distalis 42% (n=42), Hypospadia medialis 30% (n=31), Hypospadia proximalis 28% (n=29); Operation techniques: one – stage repairs 92% (n=96), two – stage repairs 8% (n=8); Complications after two – stage urethral repairs were more often than after one – stage repairs. Conclusions: only 52 % of patients with hypospadia are operated at appropriate age. It is necessary to work out recomendations of hypospadias treatment for neonatalogists, pediatrists, family doctors and patients` parents.
