Augšējās endoskopijas vizuālās un morfoloģiskās atrades analīze ambulatorajā medicīnas iestādē

Abstract

Problēma: Kuņģa vēzis joprojām ir bieži sastopama patoloģija. Kuņģa vēža veiksmīgai ārstēšanai ir svarīga agrīna diagnostika. Tāpat hronisks gastrīts un citas mikroskopiskas izmaiņas kuņģa gļotādā ir ļoti svarīgas agrīnā kuņģa diagnostikā. Augšējā gremošanas trakta endoskopija ir zelta standarts kuņģa vēža diagnostikā, savukārt hronisku izmaiņu atklāšanā, kas var novērst kuņģa vēzi svarīgi diagnostikā biopsijas morfoloģiskās izmaiņas. Ir vairākas vadlīnijas, kuru mērķis, lai ārstiem palīdzētu izvēlēties pareizu biopsijas protokolu. Balstoties uz vadlīnijām visā pasaulē un arī Latvijā tiek ieviests standarts, kā tiek identificēts atrofiskais gastrīts, intestinālā metaplāzija un adenokarcioma. Pētījuma mērķis: Novērtēt, vai ambulatorās medicīnas iestādēs tiek ievēroti vadlīniju standarti gastroskopijas veikšanā un biopsijas paraugu ņemšanā.

Problem: Gastric cancer is still a frequent pathology. For a successful treatment an early diagnostics is essential. Chronic gastritis as well as other microscopic changes in the mucous membrane of stomach is essential in an early diagnostics of stomach. The endoscopy of the upper alimentary tract is a golden standard in the diagnostics of the gastric cancer. However, to reveal chronic changes that can prevent the gastric cancer it is important to reveal morphological changes in biopsy when performing a diagnostics. There are various guidelines with the aim to help doctors to choose the correct protocol of biopsy. According to the guidelines the standard is being introduced in the whole world and Latvia as well. This standard establishes the way of identifying the atrophic gastritis, intestinal metaplasia and adenocarcinoma.
