Steiplera hemoroidektomiju salīdzinošs novērtējums

Abstract

Darba mērķis: salīdzinoši novērtēt steiplera hemoroidektomiju un steiplera hemoroidektomiju kombinācijā ar ārējo hemoroidālo mezglu ekscīziju un ieteikt optimālo metodi klīniskai pielietošanai. Materiāli un metodes: novērtēti 507 pacientu ārstēšanas iznākumi. Laikā periodā no 1998. gada janvāra līdz 2007. gada martam veiktas 83 (16,37%) steiplera hemoroidektomijas un 424 (83,63%) steiplera hemoroidektomijas kombinācijā ar ārējo hemoroidālo mezglu ekscīziju. Rezultāti: pēcoperācijas asiņošana attīstījas 8,43% pacientu pēc steiplera hemoroidektomijas un 1,4% pēc steiplera hemoroidektomijas kombinācijā ar ārējo hemoroidālo mezglu ekscīziju (p<0,001). Pirmā gada laikā recidīvs attīstījas 12,33% pacientu pēc steiplera hemoroidektomijas un 1,7% pacientu pēc steiplera hemoroidektomijas kombinācijā ar ārējo hemoroidālo mezglu ekscīziju (p<0,001). Secinājumi: mūsu pētījums rāda, ka steiplera hemoroidektomija var tikt efektīvi pielietota kombinācijā ar ārējo hemoroidālo mezglu ekscīziju. Tādas metožu kombinācijas pielietošana saistīta ar zemu recidīvu un komplikāciju biežumu.

Purpose: to compare stapled haemorrhoidectomy and stapled haemorrhoidectomy combined with excision of external haemorrhoids. Methods: we analyzed treatment results of 507 patients. In the period from January 1998 to March 2007 the were performed 83 (16,37%) stapled haemorrhoidectomies and 424 (83,63%) stapled haemorrhoidectomies combined with excision of external haemorrhoids. Results: postoperative haemorrhage developed in 8,48% of patients after stapled haemorrhoidectomy and in 1,4% of patients after stapled haemorrhoidectomy combined with excision of external haemorrhoids (p<0,001). The first year recurrence rate was 12,33% for stapled haemorrhoidectomy and 1,7% for stapled haemorrhoidectomy combined with excision of external haemorrhoids (p<0,001). Conclusions: our experience shows that stapled haemorrhoidectomy can be effectively performed in combination with excision of external piles. This kind of combination is associated with low rate of complications and recurrences.
