Toksīnus producējošu Clostridium difficile izraisītu slimību diagnostikā izmantoto metožu salīdzinoša izpēte un Clostridium difficile antibakteriālās jutības raksturojums

Pasaulē pieaug Clostridium difficile izraisītu infekciju (CDI) izplatība un smaguma pakāpe, virulentu C.difficile celmu izplatības dēļ. Terapijā un CDI kontrolē ir būtiska ticama toksinogēnu C.difficile diagnostika (TCDD) un informācija par to antibakteriālo jutību. Darba mērķis bija izvērtēt TCDD metožu ticamību, tās salīdzināt un raksturot C.difficile antibakteriālo jutību, nosakot minimālo inhibējošo koncentrāciju (MIC). Tika salīdzināts, Rīgas Austrumu klīniskajā universitātes slimnīcā, klīnika „Gaiļezers” (RAKUS KG), TCDD izmantotu metožu specifiskums un jutīgums, raksturota C.difficile izolātu antibakteriālā jutība pret metronidazolu un vankomicīnu. VIDAS BioMerieux imūnfermentatīvās metodes jutīgums bija nepietiekams (74%), bet specifiskums augsts (100 %). Reālā laika PCR (BD MAX CDIFF) uzrādīja augstu jutīgumu un specifiskumu (98,8 %), bet nebija iespējams izšķirt vai baktērija ir dzīva. Ieviesta C.difficile izolēšana, kā diagnostikas papildmetode un izmainīts diagnostikas algoritms. C.difficile metronidazola MIC bija 0,03 – 0,50 µg/ml, dominēja 0,06 µg/ml (39,7 %) un 0,12 µg/ml (32,1 %). Vankomicīna MIC bija 0,12 – 0,50 µg/ml, dominējošā – 0,25 µg/ml (65,4 %). C.diffcile antibakteriālā jutība RAKUS KG ir līdzīga kā citur Eiropā. Vankomicīna un metronidazola rezistenti C.difficile celmi netika konstatēti. Worldwide are increasing distribution and severity of Clostridium difficile infections (CDI), due to the emergence of virulent strains. The credible diagnostic of toxinogenic C.difficile (DTCD) and information of antibacterial susceptibility are an essential for control and treatment of CDI. The aim of this work was to evaluate the credibility of methods for DTCD, compare them and perform characterization of C.difficile susceptibility by detection of minimum inhibitory concentration (MIC). There were compared specificity and sensitivity of methods for DTCD used at the Riga East clinical university hospital, clinical centre „Gaiļezers” (RECUH CCG) and characterized susceptibility of C.difficile isolates against metronidazole and vancomycin. The enzymatic immunoassay method (VIDAS BioMerieux) was not sensitive enough (74 %), but specificity was high (100 %). Real-time PCR (BD MAX CDIFF) sensitivity and specificity were high (98,8 %), but it was not able to distinguish whether the bacterium is alive, as a supplementary method of diagnostic was introduced isolation of C.difficile and changed the diagnostic algorithm. MICs of metronidazole were 0,03 – 0,50 µg/ml, prevailed - 0,06 µg/ml (39,7 %) and 0,12 µg/ml (32,1 %). MICs of vancomycin were 0,12 – 0,50 µg/ml, prevailed - 0,25 µg/ml (65,4 %). Antibacterial susceptibility of C.difficile at REUH CCG were similar as elsewhere in Europe. The vancomycin and metronidazole resistant strains of C.difficile were not found.
