Promocijas darbs izstrādāts kā praktisku risinājumu komplekss uz pierādījumiem
balstītu infekciju kontroles pasākumu ieviešanai, ar mērķi samazināt nozokomiālu
bakteriēmiju incidenci. Vairāku gadu laikā intensīvās terapijas nodaļā ieviesti uz
pierādījumiem balstīti infekciju kontroles pasākumi, izvērtēta šo pasākumu ieviešanas
efektivitāte un veikti analītiski pētījumi to ietekmes izvērtēšanai uz bakteriēmiju incidenci.
Ieviešot uz pierādījumiem balstītu pasākumu kopumu, izdevās samazināt ar katetru
lietošanu saistītu bakteriēmiju incidenci no 13,3 līdz 2,1 gadījumiem uz 1000 gultdienām. Tas
uzlabo pacientu ārstēšanas rezultātus, saīsina hospitalizācijas ilgumu un arī ārstēšanas
izmaksas. Pētījumu laikā iegūtie rezultāti liecina, ka sākotnējie roku higiēnas atbilstības
rādītāji ir ļoti zemi (8 %), bet, īstenojot mērķtiecīgus pasākumus, tos ir iespējams uzlabot pat
piecas reizes. Summary
During the development of the thesis work, several consequent studies have been
implemented in Pauls Stradins’ Clinical University Hospital in order to characterize the
frequency of nosocomial bacteremia, risk factors and effectiveness of preventive measures.
During several years evidence based infection control measures have been
implemented at the intensive care unit, the effectiveness of these measures has been evaluated
and analytical studies carried out to evaluate its influence on the incidence of bacteremia. By
implementing the set of evidence based measures, the success was the reduced incidence of
catheter use related bacteremia from 13.3 till 2.1 cases per 1000 bed days. The results
obtained during the study show that at the beginning hand hygiene compliance indicators
were very low (8 %), but after implementing targeted measures, these results could be
improved even five times.
